Management of end-stage dementia.

Dementia is a progressive and noncurable illness, and its management in late stages should follow a palliative care approach. However, many patients with advanced dementia sustain aggressive interventions that do not improve their survival and might hinder their comfort and quality of life. This is likely explained by a lack of research on this population; a lack of knowledge from health care providers, patients, and family members; and lack of communication between those caring for these patients. There is therefore an urgent need for research and education on this topic, as well as palliativ

Improving Self-Management Skills Among People With Spinal Cord Injury: Protocol for a Mixed-Methods Study.

BACKGROUND: Most people with spinal cord injury will develop secondary complications with potentially devastating consequences. Self-management is a key prevention strategy for averting the development of secondary complications and their recurrence. Several studies have shown that self-management programs improve self-management behaviors and health outcomes in individuals living with chronic conditions such as asthma, diabetes, hypertension, and arthritis. Given the burgeoning health care costs related to secondary complications, we developed an alternative electronic health-based implementa

Cervical cancer screening in high-risk patients: Clinical challenges in primary care.

The risk of developing cervical cancer is not equal across populations-individual health history, economic, political, and societal factors influence cervical cancer risk. Certain health conditions, including human immunodeficiency virus (HIV) infection, immunosuppression, and history of high-grade cervical dysplasia, are associated with higher cervical cancer risk and warrant distinct screening, surveillance, and management guidelines. It is imperative for clinicians to recognize high-risk groups and apply appropriate corresponding guidelines. However, this can be difficult in practice, as re

A Narrative Review of the Benefits of Board Games in Health.

Board games and gamification - the application of game-like elements to non-game contexts - have emerged as promising strategies in healthcare for enhancing education, promoting behavioral change, and fostering social interaction. While traditionally used for entertainment, these tools are increasingly being investigated as therapeutic and diagnostic interventions across the lifespan. This narrative review synthesizes current evidence on the use of board games in patient education, diagnosis, and treatment among pediatric, adult, elderly, and mixed-age populations. Literature was examined acro

How to Estimate Optimal Malaria Readiness Indicators at Health-District Level: Findings from the Burkina Faso Service Availability and Readiness Assessment (SARA) Data.

One of the major contributors of malaria-related deaths in Sub-Saharan African countries is the limited accessibility to quality care. In these countries, malaria control activities are implemented at the health-district level (operational entity of the national health system), while malaria readiness indicators are regionally representative. This study provides an approach for estimating health district-level malaria readiness indicators from survey data designed to provide regionally representative estimates. A binomial-hierarchical Bayesian spatial prediction method was applied to Burkina F

Pressure ulcer prevention, patient positioning and protective equipment.

Pressure ulcer (PU) prevention is a high priority for health professionals in all settings. While PU incidence is expected to rise given the ageing population in the UK and Europe, PUs can occur in people of any age. It is therefore imperative that all practitioners know about the causes and consequences of PUs and be aware of up-to-date guidance on the prevention and management in patients who have developed a PU or who are at risk of developing a PU. Health professionals should also be aware of preventive equipment highlighted in national and local guidance, prevention strategies and protoco

Individuals with intellectual disability and complex epilepsy: a case study of diagnostic overshadowing.

People with intellectual disability have a higher prevalence of epilepsy, care complexity, comorbidity and polypharmacy than the general population. These patients are often found to be on multiple medications without a clear rationale for ongoing prescriptions. Recent evidence has highlighted the ongoing exposure to avoidable health inequalities and premature death in this population group, in which medicines management can play an influential role. This article aims to identify practical interventions to mitigate the risk of diagnostic overshadowing and health inequalities in individuals wit

The consequences of armed conflict on the health of women and newborn and sexual reproductive health - A position statement by the European Board and College of Obstetrics and Gynaecology (EBCOG).

Armed conflict brings about significant health-related consequences to the non-combatant population especially the elderly, women, and children. The consequences for women extend further than battle-related deaths. One important consequence of conflict is the population displacement resulting from individuals and families seeking safety in non-conflict zones. This forced migration places women at risk of gender-based violence during their journey and for this reason the host countries should ensure that the migrants are given access to reproductive health services, including providing emergenc

Use and overuse of positron emission tomography for patients with metastatic colorectal cancer.

In all healthcare systems, and particularly those that are resource limited, using diagnostic techniques efficiently is essential. According to consensus guidelines, standard computed tomography (CT) imaging without positron emission tomography (PET) is usually sufficient for the management of metastatic colorectal cancer (mCRC). This retrospective national cohort study evaluated the use of PET in mCRC management among members of Clalit Health Services (CHS, the largest Israeli health maintenance organization covering >50% of the Israeli population), utilizing the CHS central database. The coh

The role of evidence-based algorithms for rotational thromboelastometry-guided bleeding management.

Rotational thromboelastometry (ROTEM) is a point-of-care viscoelastic method and enables to assess viscoelastic profiles of whole blood in various clinical settings. ROTEM-guided bleeding management has become an essential part of patient blood management (PBM) which is an important concept in improving patient safety. Here, ROTEM testing and hemostatic interventions should be linked by evidence-based, setting-specific algorithms adapted to the specific patient population of the hospitals and the local availability of hemostatic interventions. Accordingly, ROTEM-guided algorithms implement the

COVID-19 case management strategies: what are the options for Africa?

The ongoing coronavirus disease 2019 (COVID-19) pandemic has put a strain on health systems globally. Although Africa is the least affected region to date, it has the weakest health systems and an exponential rise in cases as has been observed in other regions, is bound to overwhelm its health systems. Early detection and isolation of suspected and confirmed COVID-19 cases are pivotal to the prevention and control of the pandemic. The World Health Organization (WHO) recommends that all laboratory-confirmed cases should be isolated and treated in a health care facility; however, where this is n
