The effectiveness of collaborative/therapeutic assessment for psychotherapy consultation: a pragmatic replicated single-case study.

This pragmatic study evaluated the effectiveness of a collaborative assessment intervention as an approach to midtherapy consultation, which has yet to be empirically tested. Ten adult participants in ongoing psychotherapy with a variety of presenting concerns, primarily consisting of general mood and adjustment issues, received a brief assessment-based intervention based on Finn's (2007) Therapeutic Assessment model. Following the collection of assessment questions and the administration of a multimethod assessment battery, clients and therapists participated in a joint feedback session with

Conversation analysis of the two-chair self-soothing task in emotion-focused therapy.

OBJECTIVE: Despite an increasing recognition of the relevance and significance of self-compassion processes, little research has explored interventions that seek to enhance these in therapy. In this study, we examined the compassionate self-soothing task of emotion-focused therapy involving two-chair work, with seven clients. METHOD: Conversation analysis was used to examine client-therapst interaction. RESULTS: The analysis yielded a detailed description of interactional practices and processes involved in the accomplishment of self-soothing, drawing on Goffman's concept of the participatio

Socially anxious people reveal more personal information with virtual counselors that talk about themselves using intimate human back stories.

In this paper, we describe our findings from research designed to explore the effect of virtual human counselors' self-disclosure using intimate human back stories on real human clients' social responses in psychological counseling sessions. To investigate this subject, we designed an experiment involving two conditions of the counselors' self-disclosure: human back stories and computer back stories. We then measured socially anxious users' verbal self-disclosure. The results demonstrated that highly anxious users revealed personal information more than less anxious users when they interacted

Psychological treatment for panic disorder with agoraphobia: a randomized controlled trial to examine the role of therapist-guided exposure in situ in CBT.

OBJECTIVE: Cognitive-behavioral therapy (CBT) is a first-line treatment for panic disorder with agoraphobia (PD/AG). Nevertheless, an understanding of its mechanisms and particularly the role of therapist-guided exposure is lacking. This study was aimed to evaluate whether therapist-guided exposure in situ is associated with more pervasive and long-lasting effects than therapist-prescribed exposure in situ. METHOD: A multicenter randomized controlled trial, in which 369 PD/AG patients were treated and followed up for 6 months. Patients were randomized to 2 manual-based variants of CBT (T+/T-)

If transmen can have babies, how will perinatal nursing adapt?

Reproductive technology has made it possible for those born biologically female and who have partially transitioned to the male gender via hormones to become pregnant and give birth. This article explores the role of the perinatal nurse in providing care during the obstetric experience for a transgender male and his significant other. A carefully executed plan of care can assist the nurse in providing nonjudgmental, nondiscriminatory physical and emotional nursing care, and ensure that the family's healthcare needs are met and that their transition into parenthood is effective.

Intersecting losses: working with the inevitable vicissitudes in therapist and client lives.

Therapists working with clients with severe trauma histories over long periods of time may find themselves managing traumatic events in their own lives-illness, divorce, death-at the same time as they are assisting their clients to live productively with their painful pasts. While a few accounts exist describing how therapists deal with single overwhelming events, very little has been written about how therapists manage ongoing or prolonged episodes of severe stressors with a busy clinical practice of clients with severe trauma histories. Yet, we know that support for therapists is crucial to

Intimidation at the helm: superego and hallucinations in the analytic treatment of a psychosis.

On the basis of the development in the course of analytic treatment of a young male patient in a state of hallucinatory terror, the author discusses in this paper the nature of the psychotic superego. He shows how the process of recomposition of the self after a psychotic breakdown involves the transition from a terrorising and destructuring type of superego to one more reminiscent of that seen in depressive illness. The author describes this process against the background of the variations in the auditory hallucinations that began during the analysis. In the first phase of the analysis the pa

Processing narratives of self-destructive behavior in routine medical encounters: health promotion, disease prevention, and the discourse of health care.

This study asked how patients and doctors process self-destructive behaviors that warrant preventive action, such as smoking, substance use, and sexual practices. Although many suggestions have appeared about how physicians should encourage health promotion and disease prevention by counseling patients about self-destructive personal habits, few studies have examined actual encounters to assess how patients and doctors communicate about these matters, and prior research rarely has considered the social context of self-destructive habits. The present research has been influenced by a growing re

The meaning of activity: day care for persons with Alzheimer disease.

The purpose of this study was to gain understanding of the meaning of the daily routines and activities at a day-care center for persons with Alzheimer disease, as experienced by the staff. With the use of the qualitative research techniques of participant observation and ethnographic interviewing, data were collected for 4 weeks at a small adult day-care center for persons with mid- to late-stage Alzheimer disease or related disorders. Analysis of the data revealed that the foremost guiding principle for all activities during the day was prevention, that is, to prevent participant behavior th

Psychotherapeutic management of obsessive-compulsive patients.

Psychotherapy of the obsessive-compulsive patient requires a modified psychoanalytic approach utilizing the concepts of a short-term therapy, which include focusing on relevant issues with limited goals. The therapist must be active, confronting, and must deal with here-and-now issues. The use of psychopharmacological agents to reduce anxiety and depression should be encouraged.

Patients' reactions to the suicide of their psychiatrist.

The five patients who were in psychotherapy with a psychiatrist who killed herself were interviewed 10 months after the suicide about their reactions to the event. At the time of the suicide most patients felt lost and angry as well as demoralized and alienated from the clinic where they had been treated. To some degree each patient felt responsible for her death and several experienced considerable difficulty related to the suicide in their subsequent psychotherapy. Suggestions for management are offered.

A-B status and sex stereotyping among psychotherapists and patients. Toward a model for maximizing therapeutic potential.

Previous studies indicate that mental health professionals, when evaluating the mental health of males and females, sex role stereotype to the disparagement of females. Some feminists have further assumed that male therapists in particular discourage self-actualization of females, and have argued that female patients should be seen by female therapists only. This study examined the major hypothesis that among both male and female therapists and among both male and female patients, there may be identifiable kinds of therapists and patients who sex role stereotype more than others. A total of 24

Establishing standing and walking behavior in a physically handicapped, retarded child.

A physical therapist using reinforcement procedures worked for three months with minimal success on pulling to standing and pushing off to walk as target behaviors for a physically handicapped retarded child. The physical therapist then changed his role and used an attendant staff to work on the target behaviors in a two-and-one-half month treatment program. Pretreatment and posttreatment time sampling of the child's on-feet behaviors following the treatment program. The results of this one case suggest that the consultation role of the physical therapist might provide a more efficient treatme

Experiences of Disabled Women With Perinatal Care Providers: A Qualitative Study.

BACKGROUND: Despite growing attention to patient-provider relationships in the general population, there is a notable gap in the literature regarding the quality of these relationships in the context of maternal disability. OBJECTIVE: This qualitative study explored the experiences of pregnant and recently pregnant women with disabilities in their interactions with health care providers during the perinatal period. METHODS: As part of a larger study examining the effects of psychosocial stress on maternal and neonatal outcomes among women with disabilities, we conducted semi-structured inter
