Delayed-onset spinal subdural hematoma after kyphoplasty.

BACKGROUND: Spinal subdural hematoma (SDH) is a very rare complication of percutaneous balloon kyphoplasty. Here, a 66-year-old male developed delayed-onset spinal SDH following kyphoplasty. CASE DESCRIPTION: A 66-year-old male with a history of atrial fibrillation on Eliquis developed a SDH on day 4 following a T3 single-level kyphoplasty for an osteoporotic compression fracture. The patient presented with progressive lower extremity motor/sensory paralysis that eventually ascended to the cervical region. The magnetic resonance (MR) imaging revealed a large dorsal subdural hematoma extending

[A human bite wound caused a severe life threatening infection].

Within 24 hours after a human bite trauma, a 34-year-old male patient became septic and ended up at an intensive care unit. The combination of a local damage of the skin or mucosa, aggressive micro-organisms, local host factors, and some bad luck resulted in necrotizing fasciitis for this patient. This is a very rapid progressive infection which can spread via fat tissue and the muscle fascia. The occurrence of extensive necrosis releases a lot of cytokines and acute-phase proteins, resulting in a systemic reaction. The consequential vasodilatation makes it necessary for the heart to pump hard

[Neck phlegmon with an odontogenic cause].

BACKGROUND: When an odontogenic infection extends into the neck, a life-threatening situation may develop as a result from airway obstruction, sepsis or mediastinitis. However, this is quite rare. Prompt treatment is important to minimise these complications. CASE DESCRIPTION: A 49-year-old woman presented to the oral and maxillofacial surgery outpatient clinic of a general hospital with extensive abscesses in the neck after a molar extraction. Because of the aggressive course of the disease, the patient was transferred to a university hospital. After several surgeries, during which the neck

Fulminant necrotising fasciitis by community-acquired methicillin-resistant Staphylococcus aureus.

Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) is a rare cause of necrotising fasciitis (NF), and is usually not fulminant as in group A Streptococcus (GAS), the archetypal aetiology. We report an unusually fulminant case of NF by CA-MRSA in an immunocompetent patient. A 52-year-old man presented to the emergency department with 1 week of progressive left thigh pain and swelling. The patient had ecchymoses, bullae and hypoesthesia of the involved skin, and CT scan revealed extensive fascial oedema. He was immediately started on broad spectrum antibiotics. Within 12 h

[Intussusception in infant with diagnostic botulism: A case report].

Botulism is an important public health problem in Argentina. It is a potentially fatal disease, and its diagnosis may be difficult. There are rare presentation forms of the disease, such as acute abdomen. We present a 4-month baby with a 3-day constipation condition, associated with weakness and abnormal eating attitude in the last 12 hours. The baby presented preserved muscle tone, with no changes in sucking or deglutition according to the mother's observations. Altered sensorium and acute abdomen were found; the patient was entered into the operating room with presumptive diagnosis of intuss

Imported human rabies in a U.S. Army soldier - New York, 2011.

On August 19, 2011, a male U.S. Army soldier with progressive right arm and shoulder pain, nausea, vomiting, ataxia, anxiety, and dysphagia was admitted to an emergency department (ED) in New York for suspected rabies. Rabies virus antigens were detected in a nuchal skin biopsy, rabies virus antibodies in serum and cerebrospinal fluid (CSF), and rabies viral RNA in saliva and CSF specimens by state and CDC rabies laboratories. An Afghanistan canine rabies virus variant was identified. The patient underwent an experimental treatment protocol but died on August 31. The patient had described a do

Endodontic treatment of a Patient with Huntington's disease.

Huntington's disease is a lethal autosomal dominant disorder that affects men and women equally. The prevalence of Huntington's disease in North America is 5 to 10 per 100000 people. It is characterized by regular personality changes, dementia, and choreiform (dance-like) movements. Literature concerning dental treatment of patients who have been diagnosed with this type of disease is scarce. Continuous ingestion of medications to control the symptoms of this disease leads to xerostomia, which, maximized by unrestrained movements, often leads to progressive tooth decay, irreversible pulpitis,

Patch reconstruction of the mitral annulus for active infective endocarditis with annular abscess.

Active infective endocarditis is associated with high operative mortality and morbidity. The outcome may be improved by operating at the optimal time using the most appropriate surgical procedure. We describe a 15-year-old man with active infective endocarditis of the mitral valve associated with an annular abscess. Disseminated intravascular coagulation was associated with progressive decrease in the platelet count. The abscess was debrided, the mitral annulus was patch-reconstructed using bovine pericardium treated with glutaraldehyde, then the mitral valve was replaced. The patient was disc

Perspective of healthcare professionals on barriers and facilitators in exploring end-of-life care preferences of patients with pulmonary fibrosis: A qualitative study.

BACKGROUND: Progressive pulmonary fibrosis is a lethal disease with a survival of 3-5 years with optimal medication treatment. Palliative care and advance care planning are therefore receiving increasing attention in the literature. However, structural implementation in clinical practice is still lacking. The aim of this study was to explore the needs, facilitators, and barriers for communication on the topic of end-of-life preferences of patients from the perspective of healthcare professionals. METHODS: A generic qualitative study was performed with focus groups and individual semi structur

Ethical and Clinical Considerations in the Workup of HIV-Associated Neurocognitive Decline.

Human immunodeficiency virus (HIV) can invade the central nervous system, leading to a spectrum of neurocognitive disorders known as HIV-associated neurocognitive disorder (HAND). Despite the widespread use of combination antiretroviral therapy (cART), patients remain at risk for HAND and other neurologic complications, including progressive multifocal leukoencephalopathy (PML) and HIV encephalitis (HIVE). Differentiating among these conditions is crucial, given their distinct etiologies, prognoses, and management strategies. We describe a 37-year-old man with a six-month history of untreated

Perioperative management of pulmonary thromboendarterectomy in a patient with life-threatening acute-on-chronic pulmonary thromboembolism secondary to nephrotic syndrome: A case report.

A previously healthy 44-year-old man presented with acute severe dyspnea. Laboratory evaluation revealed severe hypoalbuminemia, elevated low-density lipoprotein cholesterol and D-dimer levels, and marked proteinuria, suggesting nephrotic syndrome (NS). Imaging demonstrated bilateral pulmonary emboli with right ventricular strain. Since he developed progressive shock, veno-arterial extracorporeal membrane oxygenation (VA ECMO) and thrombolytic therapy were initiated. Persistent thrombi and failed ECMO weaning due to poor oxygenation prompted urgent pulmonary thromboendarterectomy on day 8, con

Rapidly Progressive Subcutaneous Emphysema and Sequential Bilateral Pneumothoraces Following Pediatric Tracheocutaneous Fistula Closure: A Postoperative Air-Leak Syndrome Recognized in the Post-anesthesia Care Unit (PACU).

Tracheocutaneous fistula (TCF) closure is commonly performed in pediatric patients following successful decannulation from long-term tracheostomy dependence. Although the procedure is generally considered safe, postoperative air-leak complications, including subcutaneous emphysema, pneumomediastinum, and pneumothorax, have been reported. Early recognition is critical because progression may lead to significant respiratory compromise. We report the case of a three-year-old child who developed rapidly progressive subcutaneous emphysema in the post-anesthesia care unit (PACU) following TCF closur
