Assessment, diagnosis and management of syphilis: considerations for nurses.

Syphilis is a preventable and curable sexually transmitted infection that remains a significant public health challenge despite the availability of effective antibiotic treatment. Following a decline in the late 20th century, syphilis has re-emerged globally, with rising rates across diverse populations and a particularly concerning increase in congenital infection. The varied and often subtle presentations of syphilis, alongside its asymptomatic stages, can contribute to delayed diagnosis and ongoing transmission. Since many cases are now identified outside specialist sexual health services,

Nursing and nursing education in Haiti.

Haiti has long had the largest proportion of people living in poverty and the highest mortality level of any country in the Americas. On January 12, 2010, the most powerful earthquake to hit Haiti in 200 years struck. Before the earthquake, half of all Haitians lacked any access to modern medical care services. Health care professionals in Haiti number around one-fourth of the world average and about one-tenth the ratio present in North America. The establishment of new primary care services in a country where half of the people had no access to modern health care prior to the earthquake requi

Metro Atlanta responds to West Nile virus: a coordinated public health response.

Three and a half million people live in metropolitan Atlanta, in multiple counties with varying population bases, resources, issues and separate boards of health. Historically, public health issues have been managed within each county, with very little sharing of information among counties. The 1999 West Nile virus (WNV) outbreak in the Northeast caused public health officials in Atlanta to recognize the potential for the disease to spread to Georgia and the need to develop a coordinated, multi-jurisdictional response plan. This plan would need to address a new disease with little scientific d

Movement is muscle in hospitalized adults.

Physical inactivity is a major public health concern, but for hospitalized adults, the results of immobility are even more alarming. The "trauma of hospitalization" is a syndrome that refers to the collective impact of immobility, sleep deficits, and malnutrition associated with hospitalization and contributes to functional deficits. Functional decline is a modifiable and preventable risk factor. Nursing, at the center of patient care, is poised to coordinate the patient's mobility activities. Multiple steps to stave off functional decline to improve health outcomes for older adults are in the

In a Spirit of Restoration: A Phenomenology of Nursing Practice and the Criminal Justice System.

Conditions within jails and prisons are a public health crisis, necessitating critical reform measures. An innovative collaboration between a Midwestern College of Nursing and Cook County Department of Corrections provides students with the opportunity to develop health education for both those detained in the jail and the corrections officers. A phenomenological approach, recognizing the importance of intuitive and cognitive understanding, is offered as a framework for practice in complex environments. Principles of restorative justice provide a bridge between primary, secondary, and tertiary

Early COVID-19 Interventions Failed to Replicate 1918 St. Louis vs. Philadelphia Outcomes in the United States.

The Coronavirus disease 2019 (COVID-19) pandemic has elicited an abrupt pause in the United States in multiple sectors of commerce and social activity. As the US faces this health crisis, the magnitude and rigor of their initial public health response was unprecedented. As a response, the entire nation shutdown at the state-level for the duration of a ~1-3 months. These public health interventions, however, were not arbitrarily decided, but rather, implemented as a result of evidence-based practices. These practices were a result of lessons learned during the 1918 influenza pandemic and the ci

Learning From History About Reducing Infant Mortality: Contrasting the Centrality of Structural Interventions to Early 20th-Century Successes in the United States to Their Neglect in Current Global Initiatives.

UNLABELLED: Policy Points Current efforts to reduce infant mortality and improve infant health in low- and middle-income countries (LMICs) can benefit from awareness of the history of successful early 20th-century initiatives to reduce infant mortality in high-income countries, which occurred before widespread use of vaccination and medical technologies. Improvements in sanitation, civil registration, milk purification, and institutional structures to monitor and reduce infant mortality played a crucial role in the decline in infant mortality seen in the United States in the early 1900s. The c

Trichinellosis Outbreak Linked to Consumption of Privately Raised Raw Boar Meat - California, 2017.

On January 15, 2017, a hospital physician notified the Alameda County Public Health Department (ACPHD) in California of a patient with a suspected diagnosis of trichinellosis, a roundworm disease transmitted by the consumption of raw or undercooked meat containing Trichinella spp. larvae (1). A family member of the initial patient reported that at least three other friends and family members had been evaluated at area hospitals for fever, myalgia, abdominal pain, diarrhea, and vomiting. The patients had attended a celebration on December 28, 2016, at which several pork dishes were served, incl

Rabies death attributed to exposure in Central America with symptom onset in a U.S. detention facility - Texas, 2013.

On June 7, 2013, a man was diagnosed in a Texas hospital with rabies. He had been detained in a U.S. detention facility during his infectious period. To identify persons exposed to rabies who might require rabies postexposure prophylaxis (PEP), CDC and the Texas Department of State Health Services (DSHS) conducted investigations at four detention facilities, one medical clinic, and two hospitals. In all, 25 of 742 persons assessed for rabies exposure were advised to receive PEP. Early diagnosis of rabies is essential for implementation of appropriate hospital infection control measures and for

Logistics of control for an influenza pandemic.

Antiviral drugs will initially be the mainstay of pharmaceutical intervention in an influenza pandemic. Used primarily for therapeutic treatment, they can also lower transmission in the community. We use mathematical modelling to study the impact of different antiviral coverage strategies, with a limited stockpile. Aggressive coverage offers several advantages, even if this should exhaust the stockpile prematurely. However, where a sub-group of the population has a high mortality risk, a strategy to minimise deaths must be carefully balanced, and is not always in agreement with prioritising tr

USPSTF recommendations for STI screening.

Since 2000, the U.S. Preventive Services Task Force (USPSTF) has issued eight clinical recommendation statements on screening for sexually transmitted infections. This article, written on behalf of the USPSTF, is an overview of these recommendations. The USPSTF recommends that women at increased risk of infection be screened for chlamydia, gonorrhea, human immunodeficiency virus, and syphilis. Men at increased risk should be screened for human immunodeficiency virus and syphilis. All pregnant women should be screened for hepatitis B, human immunodeficiency virus, and syphilis; pregnant women a

Household structure and urban services: neglected targets in the control of visceral leishmaniasis.

Visceral leishmaniasis (VL) caused by Leishmania chagasi is a growing public-health problem in many parts of the New World. Although several studies have focused on the consequences of environmental damage, human migration and land occupation on the incidence of VL, the effects on the disease of the substandard living conditions that often result from the process of urbanization have not been investigated in detail. The present study was based in the Brazilian city of Teresina, where, since 1980, there have been two large outbreaks of VL (one in 1981-1985 and the other in 1993-1996), each invo

Sexually transmitted infection control with sex workers: regular screening and presumptive treatment augment efforts to reduce risk and vulnerability.

Sex workers have high rates of sexually transmitted infections (STIs), many of them easily curable with antibiotics. STIs as co-factors and frequent unprotected exposure put sex workers at high risk of acquiring HIV and transmitting STIs and HIV to clients and other partners. Eliminating STIs reduces the efficiency of HIV transmission in the highest-risk commercial sex contacts--those where condoms are not used. This paper reviews two STI treatment strategies that have proven effective with female sex workers and their clients. 1) Clinical services with regular screening have reported increase

[Recent data on prion diseases].

The transmissible spongiform encephalopathies (TSEs) or "prions diseases", provokes some controversies about their origins and possible transmission from animals to humans. Important advances were obtained recently on the genetical TSEs with the genotypic diagnosis methods based on the discovery of numerous mutations and inclusions in the PRNP gene coding for prion protein. The possible connection between the bovine spongiform encephalopathy (BSE) and the newvariant of Creutzfeldt-Jakob disease (nv-CJD) ask numerous questions in terms of public health. Lastly, the important problem of biologic

Disease eradication as a public health strategy: a case study of poliomyelitis eradication.

Disease eradication as a public health strategy was discussed at international meetings in 1997 and 1998. In this article, the ongoing poliomyelitis eradication initiative is examined using the criteria for evaluating candidate diseases for eradication proposed at these meetings, which covered costs and benefits, biological determinants of eradicability (technical feasibility) and societal and political considerations (operational feasibility). The benefits of poliomyelitis eradication are shown to include a substantial investment in health services delivery, the elimination of a major cause o

Neonatal jaundice, animal-induced injuries and disease, and immunizations.

The past years' literature on topics of interest to the general practitioner in the areas of neonatal jaundice, animal-induced injury and disease, and immunizations is reviewed. Highlighted are current practice implications of recent publications. As always, the pursuit of excellence in clinical care involves balancing new information with experienced practice. Public health policy, child advocacy, and managed care demands additionally influence the current practice of pediatrics and modify our behavior as pediatricians. However, it is at the bedside that we must strive to provide the most ben

Social Stigma Toward Persons With Prescription Opioid Use Disorder: Associations With Public Support for Punitive and Public Health-Oriented Policies.

OBJECTIVE: Prescription opioid use disorder and overdose have emerged as significant public health challenges in the past 15 years. Little is known about public attitudes toward individuals who have developed a prescription opioid use disorder and whether these attitudes affect support for policy interventions. This study examined social stigma toward individuals with prescription opioid use disorder and tested whether stigma was associated with support for various policy interventions. METHODS: A nationally representative Web-based survey was conducted from January 31 to February 28, 2014. T

Best practices for after-action review: turning lessons observed into lessons learned for preparedness policy.

All-hazards preparedness and response planning requires ongoing individual, organisational and multi-jurisdictional learning. Disaster after-action reviews are an established emergency management practice to acquire knowledge through a process of analysing what happened and why, to improve the emergency response before the next crisis. After-action reviews help individuals and organisations learn, and are an essential step in the preparedness cycle. Human and animal health authorities have begun to employ after-action reviews for disaster preparedness and response among public health and Veter
