BACKGROUND: Clot in transit (CIT) through a patent foramen ovale (PFO) with concomitant pulmonary embolism is a rare condition that carries high mortality and lacks established management guidelines.
CASE SUMMARY: A 67-year-old woman presented with 5 days of exertional dyspnea. Echocardiography showed a mobile thrombus traversing the atrial septum through a PFO with right ventricular dysfunction. Computed tomography pulmonary angiography demonstrated a saddle embolus, and brain imaging revealed a clinically silent paradoxical infarct. Compression ultrasonography showed no deep vein thrombosis

INTRODUCTION: Ischemia in extra-lobar pulmonary sequestration is rare and mostly occurs in childhood; it is uncommon in adults but can be progressive, necessitating surgical removal.
CASE PRESENTATION: A 37-year-old woman experienced sudden onset severe back pain and was initially diagnosed with pneumonia. Computed tomography revealed a 4.5 cm mass on the diaphragm and rapidly increasing pleural effusion. Emergency surgery confirmed a black mass that adhered to the diaphragm, which could be bluntly detached, but one was fixed in a cord shape that was detached by an energy device. Pathology sh

Paradoxical embolism refers to thromboemboli originating from the venous system or right heart that traverse an intracardiac or pulmonary shunt into the systemic circulation, causing end-organ embolization. It is hypothesized to occur when the right atrial pressure exceeds the left atrial pressure. To date, patent foramen ovale is the most common intracardiac shunt associated with paradoxical embolism. During acute pulmonary embolism, acutely elevated right atrial pressure can reopen a functionally closed patent foramen ovale, facilitating paradoxical embolism into the systemic circulation. Th

Occlusion of the main pulmonary artery to the right lung in a newborn infant produced peripheral (subpleural) infarction of the lung and cyst formation. Two older infants were found at autopsy to have subpleural cysts of the upper lobes. One had had surgery for repair of an atrial septal defect and a patent ductus arteriosus eight months prior to death, while the second had died suddenly of carbon monoxide intoxication. The authors suggest that pulmonary arterial occlusion may have been responsible for cyst formation in all three cases and may produce cysts in other infants that lead to idiopa

In order to trace the changes in blood oxygenation and disturbances of the acid-base equilibrium in myocardial infarction, investigations were carried out in 136 patients aged 39-83 years. The values of blood oxygenation and the acid-base status were determined on the 1st, 2nd and 3rd day of the disease, and then after one, two and three to four weeks. Besides this, in 38 patients determinations were performed 3, 6, 9 and 12 months after the onset of the disease. The determinations were careied out with a micro-Astrup unit "Radiometer". It was demonstrated that hypoxaemia is very frequent comp

Pulmonary arterial hypertension (PAH) can cause significant dilatation of the pulmonary arteries, which may externally compress the left main coronary artery (LMCA). We present 2 such cases successfully treated with percutaneous coronary intervention using drug-eluting stents. Case A was a 29-year-old woman with idiopathic group 1 PAH with severe LMCA stenosis due to main pulmonary artery compression, diagnosed during routine cardiac catheterization. Case B was a 23-year-old woman with PAH secondary to secundum atrial septal defect who presented with non-ST-segment elevation myocardial infarct

Anomalous origin of the left coronary artery from the pulmonary artery is associated with myocardial infarction, left ventricular dysfunction, mitral valve dysfunction and, occasionally, intracardiac congenital abnormalities. A technique that utilizes a flap of the anterior wall of the pulmonary artery to serve as a neocoronary artery to direct aortic flow from a created aortopulmonary window to the pulmonary artery orifice of the anomalous left coronary artery was used in five patients aged 2.5 months to 4.75 years. Two patients were less than 4 months of age at operation. There was one death

We assessed previously undiagnosed thrombophilia-hypofibrinolysis in 11 testosterone (T)-taking men, five of whom developed deep venous thrombosis (DVT), four pulmonary embolism, one spinal cord infarction, and one osteonecrosis 3.5 months (median) after starting T gel (50-160 mg/day) or T intramuscular (50-250 mg/week). In the order of referral because of thrombosis after starting T, thrombophilia-hypofibrinolysis was studied in 11 men, and, separately, in two control groups without thrombosis - 44 healthy normal male controls and 39 healthy men taking T. Nine men had DVT or DVT-pulmonary emb

A review summarizing recent findings on the causes of the development, pathogenesis, diagnosis and treatment of acute cardiac failure. It is a condition when the heart is unable to pump blood in amounts needed for the metabolic activity of tissues. It may be the first manifestation of disease or acute deterioration of chronic heart failure. The most frequent causes of acute left-sided failure include acute myocardial infarction, arterial hypertension, valvular defects, myocarditis, toxic damage or metabolic myocardial disorders. In right-sided failure pulmonary embolism, extensive affections o

Electrocardiographic (ECG) abnormalities are seen in 70%-80% of patients with acute pulmonary embolism (PE). Rarely, acute PE presents with ST-segment elevation (STE) in leads II; III and aVF and V1-3 mimicking ST-segment elevation myocardial infarction (STEMI). Herein, we describe a case of acute PE presenting with STE in II; III and aVF and V1-3.

We are reporting a case of Pemphigus vulgaris with extensive thrombosis of deep veins, pulmonary veins, and cardiac chambers complicated by myocardial infarction. The extensive skin lesions prohibited the administration of thrombolytics and coronary intervention. The patient was treated conservatively with heparin, and oral anticoagulation in addition to steroids and cyclosporine with significant resolution of thrombosis.

The study presents a case of a 48-year-old female patient, in whom first symptoms of subarachnoid hemorrhage were suggestive of acute pulmonary oedema, caused by acute left ventricular insufficiency, evoked by myocardial infarction. The authors discuss the course of illness, diagnostic difficulties and changes in circulatory system, which can appear as a result of subarachnoid hemorrhage.

Pulmonary vein thrombosis (PVT) is often not diagnosed because PVT has subtle symptoms and PVT is believed to be rare. The mechanism for the formation of PVT is unclear. In this case, I describe a small thrombus in a small branch of a pulmonary vein draining into a larger vein, the right lower pulmonary vein (RLPV). The patient was a 70-year-old male with angina pectoris, and he presented with chest pain. He had no symptoms of cerebral infarction. He previously had been treated with percutaneous coronary intervention and had four stents in the coronary arteries. A 64-slice multidetector comput

HISTORY AND CLINICAL FINDINGS: During a screening examination a pulmonary mass in the left lower lobe was detected in a 39-year-old man. The patient was asymptomatic and had hitherto been healthy. CT pulmonary angiography revealed an intralobar pulmonary sequestration with an afferent arterial branch from the thoracic aorta.
DIAGNOSIS, TREATMENT AND COURSE: Thoracotomy with resection of the sequestration was performed. On the first postoperative day hemiparesis of the left side occurred because of an infarction of the arteria cerebri media. After aspiration of the thrombus and intraarterial t

The patient was a 69-year-old man with a 3-year history of diabetes mellitus accompanied by cerebral infarction and arteriosclerosis obliterans of the lower extremities. After squatting to clean a car, the patient began to experience dyspnea. A blood gas analysis was performed upon admission, and the patient's PaCO2 and PaO2 levels were found to be low. Examination of a blood sample revealed elevated FDP, FDP-D dimer and platelet factor 4 levels. A perfusion lung scan showed multiple defects in both lungs, but no abnormal findings were detected on a ventilation scan. A pulmonary angiogram show
