BACKGROUND: The objective of this study was to evaluate factors associated with postoperative opioid use after open treatment of distal radius fractures.
METHODS: The Humana insurance claims database was queried for open treatment of distal radius fractures by Current Procedural Terminology codes. The search was further refined to identify patients who filled an opioid prescription within 6 weeks after their surgery. The study's outcomes were: (1) limited postoperative opioid use, defined as filling a prescription once in the 6-week to 6-month period after surgery; and (2) persistent postoper…

BACKGROUND: The medical community has recognized overweight as an epidemic negatively affecting a large proportion of the pediatric population, but few studies have been performed to investigate the relationship between overweight and failure of conservative treatment for distal radius fractures (DRFs). This study was performed to investigate the effect of overweight on the outcome of conservative treatment for DRFs in children.
METHODS: We performed a retrospective study of children with closed displaced distal metaphyseal radius fractures in our hospital from January 2015 to May 2020. Close…

CASE: Longitudinal follow-up of a 53-year-old woman with osteogenesis imperfecta (OI) Type III and concurrent bilateral distal radius fractures is reported. She had a right-sided displaced fracture and a left-sided minimally displaced fracture. Operative management of the displaced fracture showed adequate healing in 10 weeks. The minimally displaced fracture failed nonoperative management but did adequately heal after operative intervention.
CONCLUSION: Early surgical intervention of distal radius fractures may be considered in patients with severe OI, even in fractures with minimal displace…

AIMS: The aim of this study was to identify the origin and development of the threshold for surgical intervention, highlight the consequences of residual displacement, and justify the importance of accurate measurement.
METHODS: A systematic review of three databases was performed to establish the origin and adaptations of the threshold, with papers screened and relevant citations reviewed. This search identified papers investigating functional outcome, including presence of arthritis, following injury. Orthopaedic textbooks were reviewed to ensure no earlier mention of the threshold was pres

PURPOSE: The purpose of this study was to evaluate surgical outcomes following fixation of peri-implant distal radius fractures.
METHODS: A retrospective chart review was conducted of peri-implant distal radius fractures treated surgically at a large academic practice over 18 years. Patients were included if they had previously undergone open reduction and internal fixation of a distal radius fracture; subsequently sustained a fracture at, or adjacent to, the existing hardware; and then undergone revision fixation with the removal of hardware. Fractures were categorized into 3 groups: type A …

INTRODUCTION: Certain type of injury of the triangular fibrocartilage complex associated with distal radius fracture can result in distal radioulnar joint instability (DRUJ). Untreated DRUJ instability may lead to poor result in the treatment of acute distal radius fractures. The aim of this study was to evaluate DRUJ instability in distal radius fractures through dorsal stress radiography comparing the affected and unaffected wrists intraoperatively.
MATERIALS AND METHODS: 49 patients with a distal radius fracture who were operatively treated with a volar locking plate were included. Dorsal …

BACKGROUND: There is no consensus in the literature about the ideal classification of the distal radius fracture for the clinical practice. The traditional Melone classification system divides the distal radius into four basic components, the shaft, radial styloid, dorsal medial fragment, and volar medial fragment. The aim of this study was to identify fracture lines in comminuted distal radius fractures using three-dimensional mapping of computed tomography (CT) images to test the hypothesis that fracture fragments can be divided according to the Melone classification.
METHODS: Fifty-nine co…

BACKGROUND: The haematoma block (HB) has been successfully used to minimise pain prior to otherwise painful closed reduction manoeuvres for distal radius fractures. However, the invasive manner of the procedure, which technically produces an open fracture, still raises the question, whether HBs increase the risk of infection compared to conventional intravenous analgesia (IA). The purpose of this study was to assess complication rates and safety of the HB procedure for the closed reduction of surgically treated distal radius fractures.
METHODS: We included 176 distal radius fractures in 170 p…

STUDY DESIGN: Prospective cohort.
INTRODUCTION: Clinical studies that evaluate the correlation between associated lesions of the triangular fibrocartilage complex (TFCC) and outcome of distal radius fractures expressed with the patient-rated disability are missing. The purpose of this study was to evaluate the outcomes of distal radius fractures associated with or without an injury of the TFCC.
METHODS: Patients undergoing operative treatment for distal radius fracture were prospectively enrolled (n = 70). TFCC was examined by wrist arthroscopy and injuries classified according to Palmer. Co…

Volar locking plates used for treating distal radius fractures can damage the flexor tendon. Fractures distal to the watershed line can be fixed using volar rim locking plates. The aim of our study was to report the incidence of flexor tendon lesions when using these plates. Our case series included 15 patients (7 women, 8 men), aged 59 years on average (23-88) who underwent distal radius fixation using a volar rim plate (Extradistale®, Newclip Technics™, Haute Goulaine, France). Eight patients had complications: seven cases of synovitis treated by synovectomy with a satisfactory final outcome…

PURPOSE: Malnutrition is known to negatively affect outcomes after arthroplasty, hip fracture, and spine surgery. Although distal radius fracture surgery may be considered in a similar patient cohort, the effect of malnutrition in this scenario is unknown. We hypothesized that admission serum albumin level, as a marker for malnutrition, would correlate with the rate of postoperative complications following surgery for distal radius fracture.
METHODS: We performed a retrospective cohort study of the American College of Surgeons National Surgery Quality Improvement database. Patients undergoing

Displaced unstable extra-articular radius fractures are common and frequently treated with open reduction and internal fixation with a fixed-angle volar plate. Although this treatment yields good results, it might be more invasive than necessary for management of this often relatively simple fracture. In this article, we present the technique of using a cannulated self-drilling, self-tapping (CSDT) construct that can be employed in a minimally invasive fashion. The CSDT offers a minimally invasive alternative to volar fixed-angle plating for unstable extra-articular distal radius fractures. It…

We describe a novel technique in correcting distal radius fractures deformed with significant shortening or coronal plane translation in both the acute or chronic setting. The technique involves using a modified push-pull device to assist the surgeon in correcting length and/or translation of the articular block without the use additional hardware outside of the volar plate.
