BACKGROUND: Intraoperative localization of rib abnormalities identified on bone scan can be deceptively difficult. Previously used techniques have had limited sensitivity and accuracy. The gamma probe can help localize these bone scan "hot spots."
METHODS: Over the past 17 months, 5 patients underwent gamma-probe-directed limited rib resections following intravenous administration of Tc99m-MDP. Three patients required biopsies for suspected malignancy, and the other 2 underwent therapeutic resections for pain. The device was easy to work with following minimal training.

Experimental scoliosis with the potential for marked progression was treated by rib resection on the concave side of the curve, and the alterations of the rib cage and vertebrae in the transverse plane were investigated. A 43-year-old man was pointed out an abnormal shadow in the left hilar lesion with no symptom. Chest CT and MRI showed that the mass might be a dumbbell-shaped neurogenic mediastinal tumor next to the 8th thoracic intervertebral foramen. The resection of the chest wall and the thoracic vertebrae including the tumor was carried out, so it was revealed chondrosarcoma arising from the 8th rib by the operative findings and histological examination. The dumbbell-shaped chondrosarcoma of the rib in the posterior mediastinum is very rare, and a brief review was made from the literature.

In a retrospective study we evaluated the data regarding 126 patients (96 male/30 female) hospitalized with rib fractures between 1986 and 1989. Patients with multiple rib fractures often suffer from severe pain that impairs their respiratory performance. The effect of interpleural administration of bupivacaine (20 ml 0.25% every 4 h) for pain management was evaluated in ten patients. The initial interpleural injection resulted in significant pain relief and improvement of arterial oxygen tension. We present two cases of delayed, massive, life-threatening hemothorax due to intercostal hemorrhage in association with fractured ribs and severe blunt chest trauma (SBCT), a combination we have not seen described in the literature. Blunt chest trauma is not benign. Significant intrathoracic injuries are frequent although usually not life threatening. We have found the technique described herein to be advantageous to previously described techniques for resection of the first rib. The vital structures--the brachial plexus, subclavian artery and vein--are all readily visualized with the anterior approach and, therefore, subject to less risk of injury intraoperatively.

This paper reports on 41 immature patients whose scoliosis was treated by fixation of ribs on the convexity. All had progressive scoliosis, the curve deteriorating at more than 10 degrees p.a., or the rib-vertebra angle difference being greater than 20 degrees. Of the 24 with infantile idiopathic scoliosis examined 5 years after operation, 10 maintained the improvement obtained at operation and in a further 19 the rate of deterioration had been slowed.
