Consensus statement on the treatment of massive irreparable rotator cuff tears: a Delphi approach by the Neer Circle of the American Shoulder and Elbow Surgeons.

BACKGROUND: Management of massive irreparable rotator cuff tears (MIRCTs) remains controversial owing to variability in patient features and outcomes contributing to a lack of unanimity in treatment recommendations. The purpose of this study was to implement the Delphi process using experts from the Neer Circle of the American Shoulder and Elbow Surgeons to determine areas of consensus regarding treatment options for a variety of MIRCTs. METHODS: A panel of 120 shoulder surgeons were sent a survey regarding MIRCT treatments including arthroscopic débridement and partial cuff repair, graft aug

Critical shoulder angle does not influence retear rate after arthroscopic rotator cuff repair.

PURPOSE: The critical shoulder angle (CSA) has been implicated as a potential risk factor for failure following arthroscopic rotator cuff repair (RCR). However, there is conflicting evidence regarding the clinical usefulness of this measurement. Given these discrepancies and limited comparisons to clinical outcomes, the aim of the current study was to determine whether higher CSAs correlated with an increased retear rate after arthroscopic rotator cuff repair and to determine if any association between CSA and patient-reported outcomes (PROs) exists. It was hypothesized that there would be no

Retinoic Acid Receptor Agonists Suppress Muscle Fatty Infiltration in Mice.

BACKGROUND: The infiltration of fat tissue into skeletal muscle, a condition referred to as muscle fatty infiltration or fatty degeneration, is regarded as an irreversible event that significantly compromises the motor function of skeletal muscle. PURPOSE: To investigate the effect of retinoic acid receptor (RAR) agonists in suppressing the adipogenic differentiation of fibroadipogenic progenitors (FAPs) in vitro and fatty infiltration after rotator cuff tear in mice. STUDY DESIGN: Controlled laboratory study. METHODS: FAPs isolated from mouse skeletal muscle were cultured in adipogenic dif

A new approach to superior capsular reconstruction with hamstring allograft for irreparable posterosuperior rotator cuff tears: a dynamic biomechanical evaluation.

BACKGROUND: Superior capsular reconstruction (SCR) treatment of massive, symptomatic, irreparable rotator cuff tears (RCTs) has become a more recently used procedure. However, there is a lack of consensus surrounding optimal graft choice for the SCR technique, and current dermal grafts have increased cost and are technically challenging because of a need for multiple implants. The purpose of this study was to biomechanically investigate a biological lower-cost alternative as a support for the superior capsule reconstruction concept: an isolated semitendinosus tendon (STT) allograft and a combi

Clinical outcomes of rotator cuff repair in patients with concomitant glenohumeral osteoarthritis.

BACKGROUND: Glenohumeral osteoarthritis (OA) is a common comorbidity in patients with rotator cuff tears. However, the efficacy of rotator cuff repair in patients with concomitant glenohumeral OA is still heavily debated. Thus, the purpose of this study was to compare the clinical and functional outcome measures following arthroscopic rotator cuff repairs in patients with concomitant glenohumeral OA vs. those without glenohumeral OA. METHODS: A retrospective review of 206 consecutive patients who underwent arthroscopic supraspinatus repairs (both isolated and with accompanying infraspinatus a

Grip and shoulder strength correlation with validated outcome instruments in patients with rotator cuff tears.

HYPOTHESIS/BACKGROUND: The ability to better define preoperatively the extent of rotator cuff (RC) dysfunction is desired. The study's purpose was to prospectively examine the relationships between absolute and percentage loss (affected compared to unaffected) of grip and shoulder strength, and RC dysfunction. METHODS: Forty-seven consecutive patients with proven RC tears participated in this study. Prior to surgery, bilateral strengths of grip, shoulder abduction, and shoulder external rotation (ER) were measured with a handheld dynamometer, and subjective outcome measures were gathered. RC

External versus endoscopic ultrasound: Non-inferiority assessment for visualization of various structures of interest in the shoulder.

INTRODUCTION: Certain structures and pathologies can be difficult to reveal under videoscopy alone during arthroscopic surgery. Ultrasound can be a useful contribution in arthroscopic diagnostic and therapeutic procedures. The main aim of the present study was to assess equivalence between endoscopic and external ultrasound for shoulder exploration. Secondary objectives comprised qualitative assessment of endoscopic ultrasound images and comparative assessment of acquisition time between the two techniques. MATERIAL AND METHODS: An anatomic non-inferiority study was conducted on 6 shoulders f

[Superior capsule reconstruction in massive and irreparable rupture of the rotator cuff].

INTRODUCTION: Massive and irreparable rotator cuff injury is a clinical problem that results in loss of shoulder function and merits timely intervention that helps preserve it as long as possible before arthropathy. The option of repair of the superior capsule with autograft of Fascia Lata in the massive and irreparable rupture of the rotator cuff, is a technique proposed by Mihata, which has demonstrated an improvement of function in the short and medium term. CASE REPORT: Female of 51 years old with irreparable tear of supraspinatus and infraspinatus, failure in conservative treatment, clin

Effect of a poloxamer-based thermosensitive gel on rotator cuff repair in a rabbit model: a controlled laboratory study.

BACKGROUND: A common complication after rotator cuff repair is postoperative stiffness, which can be reduced by a simple application of an anti-adhesive agent. However, anti-adhesive agents may affect rotator cuff healing by preventing fibrosis. This experimental animal study evaluated the effect of the application of a poloxamer-based thermosensitive anti-adhesive gel and its influence on the healing of an acute rotator cuff repair in a rabbit model. METHODS: Acute rotator cuff repair (supraspinatus tendon) was performed using a transosseous suturing method. One shoulder on a randomly select

Development of a novel automatable fabrication method based on electrospinning co electrospraying for rotator cuff augmentation patches.

Rotator cuff tear is one of the most common shoulder diseases. Rotator cuff augmentation (RCA) is trying to solve the high retear failure percentage after the surgery procedures (20-90%). The ideal augmentation patch must provide a temporal mechanical support during the healing process. In this work, we proposed a simple method for the fabrication of synthetic RCA patches. This method combines the use of electrospraying to produce poly-L-lactic-co-ε-caprolactone (PLC) films in an organogel form and electrospinning to produce poly(lactic) acid (PLA) nanofibers. The device consists in a combinat

Early postoperative magnetic resonance imaging findings after arthroscopic rotator cuff repair: T2 hyperintensity of the capsule can predict reduced shoulder motion.

INTRODUCTION: The purpose of this study was to investigate whether postoperative shoulder magnetic resonance imaging (MRI) findings correlate with postoperative shoulder range of motion (ROM) at about 4 months after arthroscopic rotator cuff repair (ARCR). MATERIALS AND METHODS: Signal-intensity changes of the capsule, pericapsular soft tissue at the axillary recess, and subcoracoid fat triangle, as well as the thickness of the capsule at the axillary recess and coracohumeral ligament were assessed on preoperative and postoperative MR images of 232 patients. The ROM was evaluated preoperative

Editorial Commentary: Partial (Shoulder Rotator) Cuff Repair: May the Force (Couple) Be With You.

Partial repair of a shoulder rotator cuff tear, in the face of an otherwise massive or "irreparable" tear, is an effective treatment for reducing patient pain and improving function. Partial repair restores balance to the force couple composed of the infraspinatus and teres minor posteriorly and the subscapularis anteriorly. A balanced force couple maintains centralization of the humeral head on the glenoid during shoulder range of motion and, in nonarthritic patients with massive or irreparable tears, is better than no repair at all.

Arthroscopic lateral acromion resection (ALAR) optimizes rotator cuff tear relevant scapula parameters.

BACKGROUND: The acromion index (AI), critical shoulder angle (CSA) and lateral acromion angle (LAA) are predictive for degenerative rotatory cuff tears. Their unfavorable values are associated with a suboptimal deltoid force vector. The aim of this study was to evaluate whether an optimization of the radiological parameters could be achieved through a specific arthroscopic lateral acromion resection (ALAR). MATERIALS AND METHODS: The procedure was performed in eight fresh frozen cadaver shoulders. True a.p. and axial radiographs were taken before and after the intervention for radiological ev

Evaluation of muscular activity duration in shoulders with rotator cuff tears using inertial sensors and electromyography.

Shoulder disorders, including rotator cuff tears, affect the shoulder function and result in adapted muscle activation. Although these adaptations have been studied in controlled conditions, free-living activities have not been investigated. Based on the kinematics measured with inertial sensors and portable electromyography, the objectives of this study were to quantify the duration of the muscular activation in the upper trapezius (UT), medial deltoid (MD) and biceps brachii (BB) during motion and to investigate the effect of rotator cuff tear in laboratory settings and daily conditions. [Evolution of the technique of arthroscopic reinsertion of the rotator cuff. Our experience from the years 1998 to 2008].

PURPOSE OF THE STUDY: A rotator cuff tear is a relatively frequent cause of pain and restricted motion of the shoulder. Some orthopaedists believe that any attempt at rotator cuff reconstruction will fail. The aim of this paper is to present our experience with arthroscopic reconstruction of rotator cuff tears. MATERIAL AND METHODS: Between January 1998 and December 2008, 319 patients with an early diagnosis of rotator cuff rupture were treated. The group included 67 women and 252 men, with an average age of 37 years (range, 24 to 71 years) at the time of surgery. The patients indicated for a

Management of the failed rotator cuff surgery: causation and management.

Rotator cuff repair is a common orthopedic procedure. Techniques have evolved from open procedures to an arthroscopic (assisted) procedure in many patients. Tendon healing is anticipated, but complications may occur. There is approximately 90% patient satisfaction with index surgery, but imaging studies reveal defect recurrence in approximately one-third of the larger tears. For patients who are limited by pain, revision surgery is considered. Newer techniques of stabilizing the damaged structures, combined with delay in rehabilitation, improved the number of successful surgeries. Revision sur

Elasticity of the supraspinatus tendon-muscle unit is preserved after acute tendon tearing in the rabbit.

Supraspinatus tendon tearing is one of the most common causes of the shoulder pain and dysfunction, which often requires a surgical repair. In this situation, proximal tendon stump is usually retracted medially from its original insertion. For successful reduction of the retracted tendon stump to its original insertion, the elasticity of the tendon-muscle unit should be preserved by the time of surgery. The purpose of the present study was to clarify the chronological changes in the elasticity of the supraspinatus tendon-muscle unit after acute tendon tearing to determine the optimal timing fo
