A case of spontaneous rupture of the liver in pregnancy, fatal to mother and child, is described. The patient, a multiparous, 36-year-old diabetic, presented in the 27th week of pregnancy with upper abdominal pains resembling biliary dyskinesia. After 8 hours' observation in the hospital she suddenly developed hypovolaemic shock and fetal heart tones disappeared. On acute cesarean section a dead fetus was delivered. Hemoperiteoneum was present, arising from a huge subcapsular hematoma of the right lobe of the liver, with rupture of Glisson's capsule. The hematoma was evaluated and tears in the…

A 14-month prospective study of patients with premature rupture of the membranes was performed. The purpose of the study was to determine the effect of different therapeutic regimens for management of premature rupture of the membranes (PROM) on perinatal morbidity and mortality, as well as on maternal morbidity. Maternal and fetal risk factors and predictive factors in pregnancy outcome were prospectively defined. In patients with PROM whose gestational age was 34 weeks or more, induction within the first 12 hours of membrane rupture resulted in minimal maternal and fetal morbidity and mortal…

Rupture of the female urethra is a rare disorder, but perhaps more liable to occur in childhood than in adult life. As in the male, it can follow a fractured pelvis, but the level of the rupture is not simply in the membranous region, and is frequently much higher with destruction of the urethra over a considerable length. The vagina is always involved, so that almost inevitably there is a urethra-vaginal fistula. In the low lesions the bladder and urethral function may be adequate, but stenosis of the vaginal introitus can result in a considerable hydrocolpos, leading to a spurious form of in…

At least one additional cardiac lesion was present in 18 consecutive patients with ruptured aneurysms of the sinuses of Valsalva who were investigated between 1956 and 1973 at the University of Alberta Hospital. Clinical diagnosis was made in 78% (14/18) of the patients. Confirmation at cardiac catheterization, operation or autopsy was obtained in all but one. The main sites of rupture were the right ventricle (seven cases), right atrium (five) and left ventricle (five). Fifty percent (9/18) are alive and well following prompt operative repair, an average of 8.2 years later (range, six months …
