A 12-month evaluation of health-related quality of life outcomes of methadone maintenance program in a rural Malaysian sample.

This paper focuses on the evaluation of addiction program effectiveness which involves changes in health-related quality of life (HRQoL) profile. This study was conducted from 2007 until 2010 at a rural methadone maintenance treatment center in Malaysia to assess HRQoL outcomes before and after treatment. Fifty-seven respondents completed the WHOQOL-BREF at baseline, 6 months, and 12 months postintervention. Data were analyzed using nonparametric techniques (SPSS 15). Significant and positive HRQoL impacts were demonstrated. Future studies with larger sample are encouraged. This study was supp

Health behaviour and attitude towards screening examinations in an Austrian urban and rural population: gender aspects - screening and sex.

BACKGROUND: Both sexes are at different risks for cancers and gender-dependent fears and barriers might influence their attitude towards screening. The aim of the present study was to assess gender issues in demographic variables which could influence the use of screening examinations. METHODS: A standardized questionnaire was used to assess data of 513 Austrians, with urban and rural location of residence, aged 35-65 years. An elderly age group was additionally assessed in the urban region. RESULTS: Women of both age groups showed lower screening rates for bowel cancer. Living in partnershi

Disability and self-rated health among older women and men in rural Guatemala: the role of obesity and chronic conditions.

Unprecedented population aging in poorer settings is coinciding with the rapid spread of obesity and other chronic conditions. These conditions predict disability and poor self-rated health and often are more prevalent in women than men. Thus, gender gaps in obesity and other chronic conditions may account for older women's greater disability and worse self-rated health in poor, rural populations, where aging, obesity, and chronic conditions are rapidly emerging. In a survey of 604 adults 50 years and older in rural Guatemala, we assessed whether gender gaps in obesity and other chronic condit

The association of fat patterning with blood pressure in rural South African children: the Ellisras Longitudinal Growth and Health Study.

BACKGROUND: Hypertension is one of the major causes of death in developed and underdeveloped nations. Essential hypertension and obesity may have their inception in childhood, with little data in African children to support these findings. Objectives were to determine the prevalence of overweight and hypertension in rural children in South Africa. Additionally, the association between fat-patterning ratios and blood pressure (BP) was investigated. METHODS: Data were collected from 1884 subjects (967 boys and 917 girls), aged 6-13 years, participating in the Ellisras Longitudinal Study. Height

[Management of allergic children in rural and urban environments. Results of our experience in a health network setting in the High Pyrenes].

The allergic children management at school seems to be more and more difficult, especially in rural areas. Far from the Universitary Hospital centres how to achieve the right diagnosis? In the country side school, inaccessible for the Emergency services, how to manage an efficient interventional group? The "Hauts Pyrénéens" practitioners have decided to set up a Health care Network to answer this requirement. A managing team includes the DDASS Inspector Physician (MISP), the Academy Inspector technical advisor Physician (MRDCT) and an Allergologist Practitioner. Two commissions have been set u

Variation in tertiary prevention and health service utilization among the elderly: the role of urban-rural residence and supplemental insurance.

OBJECTIVES: Tertiary prevention seeks to reduce chronic disease progression and illness-related dysfunction. Using the Aday-Andersen model, we evaluated the impact of predisposing, need, and enabling factors on tertiary prevention, hypothesizing that urban-rural geographic differences in delivery would be detected. METHODS: A population-based telephone survey was conducted evaluating six common chronic indicator conditions: arthritis (n = 488), hypertension (n = 414), cardiac disease (n = 185), diabetes mellitus (n = 125), peptic ulcer disease (n = 125), and chronic obstructive pulmonary dise

A Descriptive Analysis of a Rural Telehospital Medicine Program in the Veterans Health Administration: A Multi-Facility, Mixed-Methods Evaluation.

PURPOSE: The Veterans Health Administration (VHA) reports provider shortages in up to 86% of their hospitals, with rural VHA Medical Centers (VAMCs) disproportionately affected. To address this, the VHA's Office of Rural Health launched the Telehospital Medicine Program to provide hospitalist coverage in understaffed facilities. This evaluation describes adoption and expansion of this program across six rural VHA hospitals. METHODS: Using a mixed-methods approach guided by the Reach, Effectiveness, Adoption, Implementation, Maintenance framework, we evaluated implementation across six VHA fac…

The financial toxicity of ageing: a longitudinal analysis of the health and functional determinants of household OOP spending in rural South Africa.

BACKGROUND: In sub-Saharan Africa, the epidemiological transition has created a double burden of chronic disease and functional decline. While the relationship between non-communicable diseases (NCDs) and out-of-pocket (OOP) health expenditure is well-established, less is known about the financial burden of physical frailty and cognitive decline. This study investigates the "financial toxicity" of ageing, distinguishing between the costs of disease diagnosis and those of functional disability. METHODS: We analysed longitudinal data from the Health and Ageing in Africa: A Longitudinal Study (H

Disparities in Lung Cancer Health Outcomes and Access to Lung Cancer Screening Between Rural and Urban Areas in the U.S.

Lung cancer is one of the leading causes of mortality in the United States. Despite overall declines in incidence and mortality nationwide, rural communities continue to experience higher rates of lung cancer incidence and mortality than their urban counterparts, a disparity that has persisted over recent decades. This review synthesizes evidence from epidemiologic and clinical studies evaluating rural-urban differences in lung cancer incidence, mortality, diagnostic stage, access to screening, and treatment outcomes. Factors influencing these differences-tobacco use and environmental exposure

Pilot study of the DeStress for Health Program: a community-engaged stress reduction and cancer prevention intervention for rural communities.

BACKGROUND: Stress related to personal, social, and environmental factors has been linked to engaging in unhealthy behaviors. These behaviors may increase cancer risk. Most current prevention programs do not address stress related to social factors and related stresses that are driving unhealthy behaviors. The purpose of this study was to pilot the DeStress for Heath Program. METHODS: We conducted a pilot randomized controlled trial. Community members from two rural counties were randomized to the DeStress for Health Program group or a waitlist control group. The DeStress for Health Program,

From research on to research with: a bottom-up participatory approach for setting research agenda at a rural health research unit.

Public health research often relies on top-down approaches that do not adequately address local needs or engage end-users as active participants. This article narrates the implementation of a bottom-up health research approach at the Model Rural Health Research Unit (MRHRU) in Puducherry, India, aimed at prioritising community participation in health research. The model leverages a participatory approach to integrate local stakeholders into the research process. The study outlines a structured, democratic process for stakeholder engagement, beginning with the identification and network-buildin…

Congenital Genitourinary Anomaly Identification and Referral in Rural Nepal: A Qualitative Study on the Perspective of Female Community Health Volunteers.

BACKGROUND AND AIMS: Delayed presentation of congenital anomalies is common in Nepal, highlighting the importance of timely diagnosis. Female community health volunteers (FCHVs), who are integral to health care delivery in communities, may offer an opportunity for early identification and referral of these anomalies. This study explored the perceptions, willingness, and motivations of FCHVs in Nepal regarding their potential role in the identification of congenital external genitourinary (GU) anomalies. METHODS: Twelve FCHVs from Devdaha municipality, Rupandehi district, Nepal, participated i

Comparing the health implications of heat exposure in pregnant women in rural and urban Tamil Nadu: An exploratory cohort study.

BACKGROUND: Increased temperatures caused by climate change may have an adverse impact on pregnant women's health outcomes. Understanding the disparities in heat vulnerabilities between rural and urban pregnant women (RPW and UPW) could help prioritize and develop tailored interventions to protect the most vulnerable. METHODS: We conducted an observational cohort study from 2018 to 2021 among 680 pregnant women in Tamil Nadu from six districts. We measured the wet bulb globe temperature (WBGT) exposure and Heat Strain Indicators (HSI) such as urine specific gravity and core body temperature u

Sensory impairment and cognitive impairment among older adults in rural China: personal responsibility for health as a possible moderator?

BACKGROUND: Sensory impairment (SI) is often overlooked as a part of normal aging. In the unique context of rural China, there is a dearth of studies exploring the association between SI and cognitive impairment. Personal responsibility for health has been promoted as a positive concept aimed at maintaining and improving health by taking proactive actions, but whether it can modify the association between SI and cognitive impairment remains unclear. Here, we explored the association between SI and cognitive impairment and investigated whether personal responsibility for health is a moderator i

Maternal Perceptions and Practices in Trachoma Prevention: A Descriptive and Correlational Analysis Using the Health Belief Model in Rural Ethiopia.

BACKGROUND AND AIMS: Trachoma remains a leading cause of preventable blindness in Ethiopia, where maternal preventive practices are critical for interrupting transmission. However, the psychosocial and perceptual landscape shaping these practices in high-burden rural communities is poorly characterized. This study describes maternal health beliefs and examines their correlational structure using the Health Belief Model (HBM) to inform the content and delivery of health education. METHODS: A community-based cross-sectional study was conducted in June 2022 in the Andabet district, Northwest Eth

Anticipated effects of centralising complex gastrointestinal surgery in a rural area in Germany - perspective of health professionals: a qualitative study.

BACKGROUND: For centralising complex gastrointestinal surgery, minimum volume standards have been implemented in Germany. To increase centralisation and thus patient safety, the minimum volume standards for complex oesophageal and pancreatic surgery were set to rise in 2023 and 2025, respectively. Hospitals that do not meet these minimum volume standards will lose authorisation to perform these surgical procedures and are not eligible for reimbursement. Volume-outcome studies focus on patient outcomes at high or low hospital volumes. Other impacts on the service, professionals (e.g. medical tr
