Subjective Shoulder Value for Sport Is a Simple, Reliable, and Valid Score to Assess Shoulder Function in Athletes.

PURPOSE: To validate the subjective shoulder value for sport (SSV-Sport) by measuring its correlation with existing patient-reported outcome measures (PROMs) and defining its psychometric properties. METHODS: Between May 2021 and May 2022, we established 2 patient groups. Group 1 included those (1) aged 18 years or older, who were (2) consulting for the first time for any shoulder condition, (3) regularly participated in sports, and were capable of accessing a questionnaire independently. There were asked to rate their SSV and SSV-Sport at admission and 2 weeks later; they also were asked to

Posterior Glenoid Bone Loss and Instability: An Evidence-based Approach to Diagnosis and Management.

Posterior glenohumeral instability is an underappreciated cause of disability that may necessitate surgical intervention to allow functional glenoid restoration. However, posterior glenoid bone abnormalities, when sufficiently severe, may contribute to persistent instability despite a well-performed capsulolabral repair. Recognition and understanding of these lesions is critical to both surgical decision making and execution of the surgical plan. Numerous procedures have been described to address posterior instability including recent developments in arthroscopic grafting techniques. The purpo

Implications for the design of a Diagnostic Decision Support System (DDSS) to reduce time and cost to diagnosis in paediatric shoulder instability.

BACKGROUND: Currently the diagnosis of shoulder instability, particularly in children, is difficult and can take time. These diagnostic delays can lead to poorer outcome and long-term complications. A Diagnostic Decision Support System (DDSS) has the potential to reduce time to diagnosis and improve outcomes for patients. The aim of this study was to develop a concept map for a future DDSS in shoulder instability. METHODS: A modified nominal focus group technique, involving three clinical vignettes, was used to elicit physiotherapists decision-making processes. RESULTS: Twenty-five physiothe

[The causes of Latarjet surgery failure and the revision surgeries].

OBJECTIVE: To review the causes of Latarjet surgery failure and various revision surgeries, in order to provide the reference for the revision of treatment options for Latarjet surgery failure. METHODS: Literature on the causes of Latarjet surgery failure and revision surgeries was extensively reviewed and analyzed. RESULTS: Latarjet surgery is widely used in clinical practice for recurrent anterior dislocation of shoulder with glenoid defects, especially for the defects of more than 25%. The main reasons for its failure are ununion, bone resorption, graft dislocation, trauma, and graft frac

[Research progress of surgical treatment for anterior shoulder dislocation and combined injuries].

OBJECTIVE: To summarize the research progress of surgical treatment for anterior shoulder dislocation and combined injuries. METHODS: The related literature was reviewed, and the surgical treatment options for the anterior shoulder dislocation and its combined injuries were summarized. RESULTS: Anterior shoulder dislocation can combine with anteroinferior capsular ligament complex injury (Bankart injury), bony Bankart defect, and Hill-Sachs lesion. For Bankart and bony Bankart injuries, arthroscopic repair or coracoid osteotomy combined with bony graft reconstruction can be performed. For Hi

Editorial Commentary: Distal Tibia Allograft as an Option for Glenoid Reconstruction in Recurrent Shoulder Instability-It's All About the Bone?

Bony defects in recurrent shoulder instability can lead to the failure of soft tissue reconstruction. Many techniques have been developed to address glenoid defects in an attempt to prevent recurrent instability. However, the high complication rates with the Latarjet procedure have led surgeons to identify other sources of bone graft, including the distal tibia allograft (DTA). The DTA appears to be a suitable option for anterior glenoid reconstruction, highlighting the importance of reconstructing all bony defects and the versatility and efficacy of allograft bone blocks.

Instability of the shoulder following seizures.

Dislocation of the shoulder may occur during seizures in epileptics and other patients who have convulsions. Following the initial injury, recurrent instability is common owing to a tendency to develop large bony abnormalities of the humeral head and glenoid and a susceptibility to further seizures. Assessment is difficult and diagnosis may be missed, resulting in chronic locked dislocations with protracted morbidity. Many patients have medical comorbidities, and successful treatment requires a multidisciplinary approach addressing the underlying seizure disorder in addition to the shoulder pa

The microscopic pathoanatomy of acute anterior shoulder dislocations in a simian model.

PURPOSE: The purpose of this study was to determine the extent of capsular injury that occurs at a microscopic level in association with acute anterior shoulder dislocations. TYPE OF STUDY: The study was an anatomic, cadaveric study using a simian model. The microscopic slide review was blinded regarding the presence or absence of dislocation. METHODS: Three cadaveric monkey shoulder specimens were dissected free of muscle tissue but left undislocated as controls. Eleven cadaveric monkey shoulder specimens were dislocated with an abduction and external rotation force and subsequently evaluat

Shoulder proprioception in baseball pitchers.

We examined proprioceptive differences between the dominant and nondominant shoulders of 21 collegiate baseball pitchers without a history of shoulder instability or surgery. A proprioceptive testing device was used to measure kinesthesia and joint position sense. Joint position sense was significantly (P =.05) more accurate in the nondominant shoulder than in the dominant shoulder when starting at 75% of maximal external rotation and moving into internal rotation. There were no significant differences for proprioception in the other measured positions or with kinesthesia testing. Six pitchers

Arthroscopic Posterior Articular Coverage and Shift (PACS) Procedure for Treatment of Preosteoarthritic Constitutional Static Posterior Shoulder Instability (Type C1).

BACKGROUND: Different joint-preserving techniques for treatment of preosteoarthritic, constitutional static (type C1) posterior shoulder instability (PSI) have been proposed, including posterior glenoid open wedge osteotomy and bone graft augmentation. However, the techniques are demanding, the reported complication and reoperation rates are high, and posterior decentering cannot reliably be reversed. PURPOSE: To assess the clinical and radiological longitudinal outcomes of patients with type C1 PSI after arthroscopic posterior articular coverage and shift (PACS) surgery. STUDY DESIGN: Case

Evidence for strength imbalances as a significant contributor to abnormal synergies in hemiparetic subjects.

Abnormal synergies in the paretic shoulder and elbow of hemiparetic subjects were quantified during maximal voluntary contractions (MVCs) in 27 subjects with a history of stroke and 8 age-matched control subjects. A six-axis load cell allowed simultaneous measurement of the primary torque the subject was attempting to maximize and the secondary torques at other joint actions. For example, during MVC of shoulder flexion, shoulder flexion is the primary torque and the secondary torques are internal/external rotation, abduction/adduction, and elbow flexion/extension. In general, the stroke subjec

Shoulder 3D range of motion and humerus rotation in two volleyball spike techniques: injury prevention and performance.

Repetitive stresses and movements on the shoulder in the volleyball spike expose this joint to overuse injuries, bringing athletes to a career threatening injury. Assuming that specific spike techniques play an important role in injury risk, we compared the kinematic of the traditional (TT) and the alternative (AT) techniques in 21 elite athletes, evaluating their safety with respect to performance. Glenohumeral joint was set as the centre of an imaginary sphere, intersected by the distal end of the humerus at different angles. Shoulder range of motion and angular velocities were calculated an

Biomechanical Testing of Scapular Spine Autograft for Anterior Glenoid Bone Augmentation.

BACKGROUND: Augmentation of anterior glenoid defects with bone graft can improve shoulder stability and reduce the risk of redislocation. Several characteristics of the scapular spine may make it a suitable harvest site, avoiding the disadvantages associated with other glenoid augmentation procedures. PURPOSE: To evaluate the capacity of scapular spine autograft to restore the stabilizing joint-reaction forces of the shoulder in simulated scenarios of bony anterior shoulder instability. STUDY DESIGN: Controlled laboratory study. METHODS: We obtained 6 matched pairs of fresh-frozen cadaveric
