A case of nodular mesenteritis simulating a carcinoma of the sigmoid is presented. The unusual features of the case are analyzed. Emphasis is placed on the basic histologic reactions seen in nodular mesenteritis and on their possible implications. The questionable relationship to Weber-Christian disease is discussed.

BACKGROUND AND PURPOSE: Preoperative understanding of the vascular morphology is important for preservation of the distal sigmoid colon and intraluminal processing of the mesentery in laparoscopic proximal sigmoid colon cancer surgery. We report a case of laparoscopic proximal sigmoid colon cancer surgery using sophisticated surgical-assisted CT colonography (CTC). The surgical-assisted CTC was created by combining a CTC created using SYNAPSE VINCENT and a vascular 3D-CT.
PATIENT AND METHOD: The patient was a woman in 80s with proximal sigmoid colon cancer, cT1bN0M0, cStage Ⅰ. The patient had

Giant colonic lipomas, tumors that infrequently occur in the gastrointestinal tract, can manifest as bleeding, abdominal pain and, in few cases, obstruction with intussusception. Surgery is usually the treatment of choice. We report the case of a 78 years-old woman with abdominal pain, constipation, and bleeding due to a giant lipoma of the sigmoid colon causing intussusception. After an initial diagnostic colonoscopy, the patient underwent an endoscopic mucosal resection (EMR) without complications. Even if surgery is traditionally the primary therapeutic approach for giant colonic lipomas, s

We report the first case of sigmoid volvulus after laparoscopic surgery for sigmoid colon cancer. The patient is a 75-year-old man who presented with the sudden onset of severe abdominal pain. He had undergone laparoscopic sigmoidectomy for cancer 2 years before presentation. CT scan showed a distended sigmoid colon with a mesenteric twist, or "whirl sign." Colonoscopy showed a mucosal spiral and luminal stenosis with dilated sigmoid colon distally and ischemic mucosa. The diagnosis of ischemic colonic necrosis due to sigmoid volvulus was established. Resection of the necrotic sigmoid colon wa

A 52-year-old Afro-Caribbean female presented with a 2-week history of left iliac fossa pain and persistent fever. The working diagnosis was diverticulitis and she was treated with intravenous antibiotics. A CT scan of the abdomen showed a thickened sigmoid colon lying adjacent to the left bladder wall with inflammatory changes present. As the patient's condition showed no improvement, an explorative laparotomy was performed, which identified a mass in the sigmoid colon adherent to the bladder and left ovary. An anterior resection, partial cystectomy and a defunctioning loop-ileostomy were per

An 82-year-old man visited a medical clinic for dyschezia. His serum carcinoembryonic antigen (CEA) level was 16ng/ml. Abdominal computed tomography (CT) revealed an 8×7cm tumor in the sigmoid colon. Endoscopy showed that the sigmoid colon was insufficiently dilated but no mucosal lesions were present. Histological examination of biopsy specimens did not show malignant cells. Sigmoidectomy was performed due to suspected submucosal tumor. The final pathological diagnosis was mucoid carcinoma with features of a submucosal tumor which had no mucosal lesions. Mucoid carcinoma with features of a su

Laparoscopic colorectal surgery has well-known benefits. However, an abdominal incision, albeit much smaller than conventional surgery, is still needed. A transvaginal extraction of a sigmoid colon neoplasia with en bloc salpingo-oophorectomy and colorectal mechanical anastomosis is described. The technique is feasible and safe. The excellent recovery of the 86-year-old patient shows the potential future of the natural orifices endoscopic surgery.

We report a case of sigmoid colon cancer that was left untreated for a period of 4 years, because the patient declined treatment. A 59-year-old man was found to have an early carcinoma of the sigmoid colon measuring approximately 12 mm in diameter. The lesion, initially a flat cancer, increased in height and became sessile 4 months later. Subsequently, the central portion of the lesion became ulcerated, leaving an elevated ring along its periphery. The lesion eventually evolved into an ulcerated, invasive cancer. This sequence has not been observed with colonoscopy before.

Intussusception accounts for almost all cases of intestinal obstruction in children. In contrast, intussusception in adults is relatively rare. An 86-year-old Japanese female with rectal bleeding came to our hospital via ambulance. At first, colonoscopy findings revealed the sigmoid colon cancer. Ultrasonography showed a hypoechoic mass with a multiple concentric ring sign. Computed tomography showed a round fluid-filled cystic structure. Colon contrast studies demonstrated stenosis in the rectosigmoid colon. A laparotomy was performed. The sigmoid colon was intussuscepted to the rectosigmoid
