Endoscopic resection of sinonasal malignancies.

BACKGROUND: In this study, we evaluate our experience and the outcomes of patients with sinonasal cancer treated with endoscopic resection. METHODS: Retrospective review of patients with sinonasal cancer who had endoscopic resection was conducted. The outcomes of interest included survival outcomes and surgical complications. RESULTS: Overall, 239 patients were included. Median follow up time was 46.6 months. Of the 239 patients, 167 (70%) had a pure endonasal endoscopic approach, while 72 (30%) had an endoscopic-assisted approach. Postoperative cerebrospinal fluid (CSF) leakage occurred in

Endoscopic Endonasal Transclival Resection of the Upper Clival Meningioma.

In recent years, endoscope use for the excision of brain stem lesions or those localized to the anterior part of the brain stem has started. In this paper, a case of upper clival meningioma resected through the endoscopic endonasal transclival route has been presented with illustrations, and live surgery videos demonstrating the surgery step-by-step. A 35-year-old male patient presented with dysphagia and impaired consciousness. Magnetic resonance imaging (MRI) showed a mass lesion with a wide base located at the clivus and anterior part of brain stem. Following surgical preparations, the mass

Frozen Section Can "Sharpen" or "Sand Off" the Surgeon's Knife: 2 Case Illustrations with Skull Base Meningioma Mimics.

BACKGROUND: Lesions at the skull base warrant careful evaluation of imaging and meticulous preoperative planning. Unnecessary radical resection of these lesions could add to surgical morbidity. Occasionally, dural-based inflammatory or infectious lesions at this site radiologically mimic meningiomas, which are commonly encountered entities at this site. Not all lesions require excision, and it could be harmful in a few cases. CASE DESCRIPTION: Two patients had skull base lesions that resembled a meningioma on imaging, whereas frozen section in both cases differed from the preoperative diagnos

Isolated intracranial Rosai-Dorfman disease mimicking petroclival meningioma in a child: Case report and review of the literature.

RATIONALE: Rosai -Dorfman disease (RDD) is a rare, idiopathic, and non-neoplastic histioproliferative disease with distinctive entity of unknown etiology. Central nervous system (CNS) RDD is uncommon, hence, isolated intracranial RDD is extremely rare. So far only 6 cases of CNS RDD with the lesions originating from petroclival region have been reported. We present a case of isolated intracranial RDD mimicking petroclival meningioma. PATIENT CONCERNS: A 14-year-old girl was admitted at our hospital with a 3-month history of dizziness, slowly progressing headache, and 2-month history of instab

Primary epithelioid hemangioendothelioma originating in the lower petroclival region: case report.

BACKGROUND: We report the first case of primary epithelioid hemangioendothelioma (EH) originating in the lower petroclival region. CASE DESCRIPTION: A 55-year-old female presented with a 45-year history of subclinical atrophy on the right side of her tongue and a 15-year history of hoarseness. Neuroimaging revealed an expansile, homogeneously enhanced intraosseous mass with bony shell and honeycomb configuration in the petroclival region. A right far lateral transcondylar approach was utilized for subtotal removal of the tumor, which demonstrated high vascularity in the petroclival bone. Hist

How to reliably evaluate middle ear diseases? Comparison of different methods of post-processing based on multislice computed tomography examination.

CONCLUSIONS: Multislice computed tomography (MSCT) virtual endoscopy was useful in evaluating mainly post-traumatic and postoperative cases. In other pathological conditions axial images and MPR reformations were most useful. OBJECTIVES: Evaluation of cross-sectional images, multiplanar 2-D reformations, 3-D reconstructions and virtual endoscopy (VE) in assessment of the middle ear in inflammatory diseases, trauma, otosclerosis and tumours. Comparison of each method and correlation with surgical findings. MATERIALS AND METHODS: Investigations were carried out in 80 patients with middle ear p

Giant cranionasal epithelioid haemangioendothelioma with invasive growth pattern mimicking a skull base chondrosarcoma.

Epithelioid haemangioendothelioma (EHE) is a rare low-grade vascular neoplasm that is composed of mostly epithelioid cells. EHE may arise as a solitary tumour or in the form of multiple body lesions, and commonly occurs in soft tissues, liver, pleura, lung, peritoneum, lymph nodes, breast, and many other sites. EHE in the cranionasal region is extremely rare. There are very few reports of cases of skull-base EHE. We discuss an extremely rare presentation of an aggressive EHE that originated from the sellar region. Based on literature review, our patient is the first reported case of a giant so

Endoscopic repair of high-flow cranial base defects using a bilayer button.

OBJECTIVES/HYPOTHESIS: Repair of the skull base still begins with a direct repair of the dural defect. We present a new button closure for primary repair of the dura for high flow defects. STUDY DESIGN: Retrospective review. METHODS: We reviewed our 20 cases of primary button grafts and compared the results to the previous 20 high-flow open-cistern cerebrospinal fluid (CSF) cases. Subjects were excluded if they had no violation of the arachnoid space or potential for low-flow CSF leak. The button is constructed so that the inlay portion is at least 25% larger than the dural defect, and the o

Transdural Cisternotomy in the Extended Middle Fossa Approach.

Dolenc's pretemporal transcavernous approach and Kawase's s anterior transpetrosal approach are the two primary extradural middle cranial fossa approaches (EMFAs) to the central skullbase. The extended extradural phase of the surgery may potentially result in temporal lobe edema, compromising the surgical exposure and sometimes a postoperative temporal lobe venous infarction. The authors describe an alternative technique of cerebrospinal fluid (CSF) drainage called "transdural cisternotomy" in EMFA. In this, CSF is released from the chiasmatic cistern during the extradural stage of surgery. Tw

Imaging Progress and Treatment of Malignant Triton Tumor of the Skull Base.

BACKGROUND: Reporting the clinical course and management of a skull base malignant triton tumor in a child. METHODS: An 11-year-old female patient initially presented at the age of 7 with left parotid gland swelling, exhibiting symptoms similar to parotitis. Magnetic resonance imaging (MRI) of the head performed at an external hospital suggested a neoplastic lesion in the left parotid gland and the left parasellar region. Six months later, a parotid gland biopsy was conducted. Postoperative pathology confirmed the diagnosis of embryonal rhabdomyosarcoma. Following the biopsy, the patient unde

[Endoscopic transnasal resection of clival meningiomas].

BACKGROUND: Surgical treatment of ventral and ventrolateral meningiomas of posterior cranial fossa is difficult in modern neurosurgery. This is due to peculiarities of approach to these areas and concentration of critical structures (cranial nerves and great vessels). Currently, endoscopic transnasal approach to these meningiomas allows partial, and in some cases, total resection. However, this technique is not widespread. OBJECTIVE: To analyze the world literature data on postoperative outcomes in patients with clival and petroclival meningiomas after endoscopic transnasal resection. MATERI

Prognostic factors and outcomes in minimal access resections of skull base and sinonasal epithelial malignancy.

BACKGROUND: Sinonasal epithelial malignancies are uncommon tumours but represent a challenge to treatment given their close proximity to the orbit, brain and cranial nerves. Traditional external surgical approaches have potential for significant functional and cosmetic morbidity. An endoscopic approach provides the surgeon with good access for tumour removal and enables surveillance postoperatively. This study aimed to assess outcomes of an endoscopic approach to sinonasal epithelial malignancy and evaluate factors that may influence its utility. METHODS: A case series was performed involving
