BACKGROUND: Non-battle injuries (NBIs) represent a substantial proportion of morbidity during military deployments, often exceeding combat-related injuries in frequency. NBIs, including superficial wounds, burns, blisters, and soft tissue infections, remain a significant source of lost duty time and operational disruption in deployed military populations. In austere environments with limited medical support, even minor injuries often progress to cellulitis or abscess, which require substantial logistical resources including irrigation fluids, medical dressings, and antibiotics, impairing opera…

BACKGROUND: Clostridium septicum is a highly virulent, gas-forming, anaerobic Gram-positive bacillus known to cause rapidly progressive necrotizing soft tissue infection that is often associated with gastrointestinal malignancy or immunocompromised states. Mortality remains extremely high, with many patients dying within the first 24 h of onset. Although aggressive surgical debridement is the cornerstone of treatment, cases requiring hemipelvectomy are exceedingly rare, and postoperative management of exposed pelvic organs has not been described in the relevant literature.

RATIONALE: Complicated skin and soft tissue infections (cSSTIs) are severe infections that may arise from abscesses, cellulitis, fasciitis, diabetic foot infections, trauma-related infections, and surgical site infections. Conventional treatment typically requires surgical drainage combined with systemic antibiotics. However, exploratory surgical drainage may cause tissue damage and increase inflammatory responses.
PATIENT CONCERNS: A 35-year-old male with diabetic ketoacidosis presented with progressive swelling, erythema, and pain in the right upper arm despite prior antibiotic therapy.

Aureobasidium melanogenum is an emerging pathogen of growing importance in medical literature. It is a dematiaceous fungus with a propensity to cause opportunistic infections in immunocompromised hosts. However, we present a case of an immunocompetent patient who developed a skin and soft-tissue infection caused by A. melanogenum. The pathogen was identified at a reference laboratory at the University of Texas (UT), San Antonio, Texas, using phenotypic and DNA sequencing techniques. The patient was successfully treated with a two-week course of intravenous micafungin. This case highlights the

INTRODUCTION: Skin and soft tissue infections are common because of exposure to aquatic environment, while severe infections caused by Aeromonas veronii are rare.
CASE PRESENTATION: We report a case of severe skin and soft tissue infection of the left upper limb caused by Aeromonas veronii. A 50-year-old Chinese woman, who had a history of cardiac disease and type 2 diabetes mellitus, accidentally injured her left thumb while cutting a fish. Early antibiotic therapy and surgical debridement was performed before the result of bacterial culture came back. Whole-genome sequencing was further per…

Roseomonas gilardii rarely causes infection in humans. We report that a patient with underlying rheumatoid arthritis and diabetes developed wrist septic arthritis and osteomyelitis due to Roseomonas after steroid joint injection. After antibiotic and surgical treatment, the condition of the patient improved. We reviewed previously reported cases of soft tissue, joint and bone infection related to Roseomonas to understand the characteristics of joint and bone infection of Roseomonas.

INTRODUCTION: Colostomy is usually proposed during the acute phase of Fournier Gangrene, nevertheless its impact on disease outcome remains still debated. We conducted a retrospective study in an academic center to determine the impact of fecal diversion on disease morbidity and specific survival.
METHODS: All medical charts of Fournier Gangrene cases in the past 30-years were reviewed. Mortality rate, hospitalization duration, time to complete healing and number of excision surgeries were compared between the stoma and the non-stoma groups. Time between initial diagnosis and stoma creation,

Prosthetic failure secondary to bearing fracture remains a potential complication in ceramic-on-ceramic total hip arthroplasty. We report the unusual presentation of a ceramic component fracture of a total hip arthroplasty performed 17 years ago that mimicked a periprosthetic joint infection. This case was managed based on the current guidelines and algorithms recommended for periprosthetic joint infection management. Histologic examination of periprosthetic tissue revealed an adverse inflammatory soft-tissue reaction to the ceramic fragments released from the fracture site. Our case highlight

BACKGROUND: Compared to other life-threatening infection survivors, long-term health-related quality of life (QOL) of patients surviving necrotizing soft-tissue infections (NSTI) and its determinants are little known. In this monocentric prospective cohort including NSTI survivors admitted between 2014 and 2017, QOL was assessed during a phone interview using the 36-Item Short-Form Health Survey (SF-36), the Hospital Anxiety and Depression (HAD), the activity of daily living (ADL), instrumental ADL (IADL) scales and the Impact of Event Scale-Revised (IES-R). The primary outcome measure was the

Necrotizing fasciitis is a highly invasive disease characterized by rapidly worsening inflammation of the fascia associated with necrosis of the subcutaneous tissue. It is a rare, life-threatening disease and needs early diagnosis through acute clinical awareness. It warrants urgent and aggressive surgical management. We report a rare and fatal case of spontaneous necrotizing soft tissue infection (NSTI) of the anterior abdominal wall caused by Finegoldia magna in a chronic diabetic patient. The initial presentation and radiological features suggested a pelvic abscess. Other acute abdomen diff

Non-tuberculous mycobacteria are ubiquitous pathogens causing infections in immunocompromised patients. Here, we describe a kidney transplant recipient who developed skin and soft tissue infection by Mycobacterium haemophilum, complicated by tenosynovitis and fluid collection, following an injury sustained to her right foot. Her immunosuppressant dose was reduced, and she underwent prolonged antimicrobial therapy followed by surgical debridement with a favorable outcome. Non-tuberculous mycobacteria should be considered as a potential etiology of subacute skin and soft tissue infections.

INTRODUCTION: Skin and soft tissue infections (SSTIs) present with variable severity. The American Association for the Surgery of Trauma (AAST) developed an emergency general surgery (EGS) grading system for several diseases. We aimed to determine whether the AAST EGS grade corresponds with key clinical outcomes.
METHODS: Single-institution retrospective review of patients (≥18 years) admitted with SSTI during 2012 to 2016 was performed. Patients with surgical site infections or younger than 18 years were excluded. Laboratory Risk Indicator for Necrotizing Fasciitis score and AAST EGS grade w…

OBJECTIVES: Little is known about procedural sedation use for anxiety and pain associated with skin and soft tissue infections (SSTIs) requiring incision and drainage (I&D). Our objectives were therefore (1) to characterize the use of procedural sedation use for SSTI I&D procedures in pediatric emergency departments (EDs), (2) to compare the frequency of procedural sedation for I&D across hospitals, and (3) to determine factors associated with use of procedural sedation for I&D.
METHODS: We performed a retrospective cohort study of pediatric EDs contributing to the Pediatric Health Informatio…

Skin and soft tissue infections (SSTIs) are among the most common infections treated by emergency department clinicians. The emergence of community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) as the cause of these infections prompted the Centers for Disease Control and Prevention and the Infectious Disease Society of America to publish guidelines for the outpatient management of SSTIs. This study describes the management and outcomes of emergency department patients treated for uncomplicated SSTIs who returned within 30 days of the initial visit. The study found that of 85…
