Cranio-Orbital Pretemporal Approach for Resection of Right Superior Orbital Fissure/Orbital Renal Cell Metastasis: 2-Dimensional Operative Video.

Orbital metastatic lesions are rare entities1-3 best treated with radical surgical resection with preservation/improvement of neurological and visual function.1-9 Renal cell metastases, in particular, respond less favorably to radiation.9 To our knowledge, an operative video of microsurgical resection of a renal cell carcinoma metastasis to the superior orbital fissure and orbit has not been reported. A patient presented with worsening right eye vision as demonstrated on preoperative visual field testing and found to have a 3 × 1 × 1 cm lesion in the orbit and superior orbital fissure. The pat

Neurosurgery Technical Procedure: Surgeon's Position during Microscopic Transsphenoidal Surgery.

The surgeon's position in relation to the patient has been modified throughout the years for transsphenoidal surgery (TSS). Nowadays, if a microscopic approach is used, most centers place the surgeon at the side of the patient's head with the patient facing him. In this paper, we propose a more ergonomic and time-proven setup for this type of surgery. Since the early 1980s, our neurosurgical section has been using an approach where the surgeon stands behind the patient's head with the microscope tower on the surgeon's back. This position is rarely mentioned in the literature. We want to refres

Anatomical meningo-orbital band evaluation and clinical implications: a cadaveric dissection study.

BACKGROUND: The meningo-orbital band (MOB) is a dural structure which runs around the superior orbital fissure (SOF) tethering the frontotemporal basal dura to the periorbita, through the SOF. MOB division is important to expose and remove the anterior clinoid process, to access to proximal carotid artery and cavernous sinus area. The aim of this study was to measure how the MOB could be safely incised without cranial nerves and cavernous sinus injuries. METHODS: Anatomical dissections and extradural exposure of the anterior clinoid process was performed on 20 cadavers (40 sides). Measurement

[A study of the relations of foramen rotundum structure direction and the approach of percutaneous puncturing of radiofrequency thermocoagulation for treating V2 of primary trigeminal neuralgia].

Objective: To investigate the relations of foramen rotundum structure direction and surrounding structure systematically in order to choose the best approach of percutaneous puncturing of radiofrequency thermocoagulation for treating V2 of primary trigeminal neuralgia. Methods: A total of 122 patients with V2 of primary trigeminal neuralgia for radiofrequency thermocoagulation were enrolled from August 2012 to May 2017 at the First Hospital of Jiaxing. CT scan images were observed retrospectively, to find the inside and outside of the foramen rotundum. The direction of foramen rotundum were re

The unusual orbitosphenoid of the snakelike lizard Bachia bicolor.

We studied the morphology of the chondrocranial orbitotemporal region in the snakelike gymnophthalmid lizard Bachia bicolor and its relation to other structures such as the ophthalmic division of the trigeminal nerve and eye muscles, to show its particular morphology, and discuss the homology of some skeletal structures relative to other squamates. We used three-dimensional computer reconstructions from serial histological sections; additionally we studied the embryonic and postembryonic development of the orbitosphenoid bone using cleared and double-stained whole-mount skeletal material. The …

A unique case of an epidermoid cyst of the pterygopalatine fossa and its management.

Epidermoid cysts are rare benign tumors that are derived from the development of abnormally situated ectodermal tissue. Although they may originate from any part of the body head and neck region is occasionally affected. Epidermoid cyst originating from pterygopalatine fossa has not been reported previously in the literature. We report the first case of epidermoid cyst of pterygomaxillary fossa in this study.

Role of surgery in the management of pituitary tumors.

Growth characteristics and size, irrespective of endocrine activity, predict the nonendocrine clinical presentation of a pituitary adenoma. Anatomic classification by degree of sellar destruction (grade) and extrasellar extension (stage) has value in determining prognosis and in designing therapy, and magnetic resonance imaging provides optimal diagnostic information. The transsphenoidal approach is the preferred surgical technique for most pituitary adenomas and the treatment of choice for those secreting growth hormone (acromegaly) or adrenocorticotropic hormone (Cushing's disease, Nelson's

Nasal septum after sublabial transseptal transsphenoidal pituitary surgery.

Transseptal transsphenoidal approach to the pituitary fossa is a well-described and effective procedure. This article relates our experience with this procedure with specific emphasis on the nasal septum, both before and after surgery. It has been our experience that this surgery has minimal local complications in the nose and it would appear to improve septal alignment with subjective improvement in nasal function as reported by the patient. A total of 55 patients undergoing a sublabial transseptal transsphenoidal approach to the pituitary fossa were included in this study. All other approach

CT-Based Timeline for Closure of Sphenoid Body Synchondroses and Foramina in Children.

BACKGROUND AND PURPOSE: The sphenoid bone forms from multiple ossification centers. Its body develops through the fusion of presphenoid and postsphenoid cartilages separated by the intersphenoid synchondrosis. Variations in ossification can lead to persistent craniopharyngeal duct remnants, potentially associated with pituitary dysfunction or tumors. We aimed to determine the timeline of closure of these synchondroses and associated foramina in children without skull base abnormalities on CT scans. MATERIALS AND METHODS: This retrospective study analyzed CT scans of children aged 0-6 years fr…

Anterior clinoid mucocele coexisting with sphenoid sinus mucocele.

An anterior clinoid mucocele, known to be extremely rare, can lead to visual complications due to its proximity to the optic nerve. We report a patient who developed visual disturbance due to an anterior clinoid mucocele. Interestingly, the anterior clinoid mucocele coexisted with a sphenoid sinus mucocele. When an anterior clinoid mucocele coexists with a sphenoid sinus mucocele, more deliberate diagnostic and therapeutic approaches must be considered according to our first experience.

Undifferentiated sarcoma of the sphenoid sinus.

Paranasal sinuses sarcomas are rare and no treatments have been established. We report a young-adult case of sphenoid sinus sarcoma treated by carbon-ion radiotherapy. The patient presented with progressive left-sided visual impairment. A tumor was then identified and partial resection by transnasal approach was performed. The resected mass showed typical morphology of mesenchymal tumor, and morphological and molecular analyses ruled out a predominant-differentiation phenotype. The pathological diagnosis was undifferentiated sarcoma. The residual lesion was treated with carbon-ion radiotherapy

Meningel intracranial epithelioid hemangioendothelioma: case report and literature review.

Intracranial epithelioid hemangioendothelioma (EH) is a very rare vascular neoplasm. We describe a case of meningeal intracranial tumor in a 58 year old man, located in the greater wing of the sphenoid bone. The tumor was completely excised via pterional approach and the pahological analysis showed a hemorrhagic lesion with capillary-sized vessels lined by atypical-appearing endothelial cells that expressed CD31, CD34 and Factor VIII antigens. A diagnosis of EH was established, and one year after surgery the patient is symptom-free, without tumor recurrence or residual tumor. A review of the l…

[Complications of endoscopic sinus surgery].

Endoscopic sinus surgery has become popular during recent years. Here is a report of complications of endoscopic sinus surgery among 150 patients. Seven patients had complications, including one nasolacrimal duct injury, one temporary blindness, one orbital hematoma as a result of trauma to the lamina papyracea, two orbital infections, one CSF leakage and one death probably due to laceration of the internal carotid artery. The complication rate was 4.6%, and the mortality rate 0.6%. We conclude that the knowledge of anatomy, good training, and meticulous surgical technique are very important,

Primary intraosseous meningioma mimicking osteosarcoma: case report.

With an incidence rate of 15-20%, meningiomas are one of the most common brain tumors among benign intracranial tumors. They are distributed as follows in intracranial localizations: parasagittal and falx (25%), convexity (18%), sphenoid wing (18%), parasellar (12%), posterior fossa (10%), intraventricular (2%), intraorbital (1%), and extracranial (1%). The most common extradural localizations are the paranasal sinuses, nasal cavity, skin, neck, glands, and intraosseous space. Intradiploic meningiomas are generally localized in the frontoparietal and orbital regions. Due to their low incidence

Factors affecting survival of patients with tumors of the anterior skull base.

Fifty-four patients with primary neoplasms of the anterior skull base were treated by craniofacial resection with curative intent. The most common tumor was esthesioneuroblastoma (24), followed by squamous cell carcinoma (10). The overall 2-year and 5-year survivals were 75% and 49%, respectively. High-grade tumors, grades 3 and 4, had a poorer prognosis. Tumor size, dural involvement, sphenoid sinus involvement, age, and sex had no significant influence on survival when examined by multivariate survival analysis. Sixteen complications were noted in the postoperative period. Cerebrospinal flui

An IgD extramedullary plasmacytoma involving the sphenoid sinus at onset: an immunohistochemical study.

A 60-year-old man developed a left VIth-nerve paralysis and underwent biopsy of an extramedullary plasmacytoma in the sphenoid sinus. Immunohistochemistry demonstrated cytoplasmic IgD and lambda determinants. Subsequent specimens from left clavical and thoracic epidural lesions also showed a plasma cell neoplasm with IgD and lambda determinants. No abnormal plasma cells were found in bone marrow specimens. Lambda Bence-Jones protein and monoclonal IgD-lambda protein in the serum were detected several months after onset. The patient died with disseminated plasmacytoma 1 year after diagnosis. Th

Orbital meningocele in two case studies.

BACKGROUND: Orbital meningocele is a rare congenital malformation characterized by herniation of the meninges into the orbit through a congenital defect in the orbital bones. Much less commonly, it occurs at the site of natural openings (e.g., optic foramen and sphenoidal fissure) or can be attributed to trauma. CASES DESCRIPTION: We report two patients with progressive proptosis found to have orbital meningoceles, respectively, attributed to congenital and traumatic lesions. The computed tomography scan in one case documented a traumatic orbital bony defect, but in the other case, led the mi
