Alcohol use among spinal cord-injured patients.

Patients with spinal cord injury (SCI) are at greater risk than others for alcohol abuse because they face physical, psychological and vocational difficulties that include sensory impairment, pain, depression and reduced opportunities to participate actively in society. This study assesses the utility of using a brief screening interview questionnaire, the CAGE (cut, annoyed, guilty and eye opener), to further evaluate patients with SCI for alcohol use, including alcoholism using retrospective data. The study explores patterns of alcohol consumption among SCI patients and investigates the rela…

Missed injuries associated with spinal cord trauma.

Spinal cord trauma frequently results from high energy vehicular accidents which produce multisystem trauma. Because of the priorities of resuscitation, other injuries may escape early diagnosis. This study was undertaken to examine the extent and implications of "missed injuries" associated with spinal trauma. We reviewed the charts of 24 patients (23 men, one woman) with spinal cord injuries, who presented during a consecutive 9-year period. The median age was 31 years. Two patients died. There were 13 cervical, 10 thoracic, and one lumbar injuries. Blunt trauma was responsible for injuries …

Sacral agenesis and caudal spinal cord malformations.

Thirty-three children and one adult with sacral agenesis (SA) were studied by computed tomographic myelography and/or magnetic resonance imaging and were monitored for a mean period of 4.7 years. Four children had the OEIS (concurrent omphalocele, cloacal exstrophy, imperforate anus, and spinal deformities) complex, and three others had VATER (vertebral abnormality, anal imperforation, tracheoesophageal fistula, and renal-radial anomalies) syndrome. All patients shared some of the characteristic features of SA, namely, a short, intergluteal cleft, flattened buttocks, narrow hips, distal leg at

Pyrexia in hospitalised spinal cord injury patients.

To discover the frequency of pyrexia in spinal cord injury (SCI) patients in the Hospital for Medicine of the Locomotor System (HMLS/SARAH), a review study was undertaken. For a period of 3 months, all new episodes of fever were recorded. During the study 221.3 patients/month were observed. The age ranged from 14 to 60 years and time of lesion from 1 to 6,264 days: 85 patients were paraplegia or paraparetic, and 44 tetraplegic or tetraparetic. At the beginning of the study 10.8% of the patients had fever. During follow up, we observed a rate of 33.9 new incidences per 100 patients/month. In 77…

[Magnetic resonance tomography in spinal cord trauma].

A total of 44 patients with sequelae of spinal injuries in the periods of 2 months to 46 years were studied by X-ray, neurological and NMR techniques and divided into 3 groups: (1) those with open injuries, (2) those with closed injuries concurrent with compression of the spinal marrow and/or roots of the cauda equina; 3) those with closed injuries without compression. It was shown that NMR tomography enabled the status of the vertebral canal and spinal marrow to be assessed and indications for surgical interventions to be specified. Spinal compression may in later periods result in post-traum

Spinal cord injury due to suicide attempts.

During 1965 to 1987 45 patients were admitted to the Rehabilitation Hospital in Hornbaek, Denmark because of spinal cord injury (SCI) due to suicide attempts. The proportion of traumatic SCI due to this cause increased significantly during the period: 1965-74: 3.5%, 1975-84: 8%, 1985-87: 13%. The median age at injury was 31 years, and the female to male ratio 1.6. In 38 instances (84%), SCI was caused by jumps from buildings. The neurological level of the lesion was: 7 cervical, 8 thoracic, 22 lumbar; the remaining 8 had minor spinal cord/cauda equina lesions. At the time of the SCI 62% were u…

Rerouting peripheral nerves for spinal cord lesions.

To provide control of paralyzed limb muscles following spinal cord lesions, peripheral nerves containing motor axons from motoneurons located above a spinal cord lesion could potentially be rerouted to nerves containing motor axons located below the spinal cord lesion. To test this hypothesis in rats, the distal end of a cut tibial nerve, innervated by the L4-6 spinal level, was anastomosed or rerouted to the central end of the cut femoral nerve, innervated by the L3-4 spinal level. Appropriate controls were used. Recovery of lower hind limb motor function was followed at regular intervals, me…

[Cardiovascular changes in cervical spinal cord lesions].

A study is performed on the alterations in blood pressure and heart rate in 36 patients with cervical spinal cord lesions with as evolution of at least 5 years. We have found fluctuations in blood pressure and heart rate when passing from lying to standing position, before urinating and from one day to another. The incidence of autonomic dysreflexia crisis is observed in 178 patients suffering cervical spinal cord lesions of the Hospital Nacional de Parapléjicos, Toledo. This is a scarcely known entity by physicians in general and the patient's life very often depends on its diagnosis and corr…

Spinal cord injury: a ten-year report.

In the past 10 years, the RSCICDV has had a unique opportunity to serve and expand the bounds of knowledge regarding this most devastating injury. The RSCICDV has collaborated with other model SCI systems in research regarding the incidence of respiratory complications, the value of removing bullet fragments lodged within the spinal canal, the survival/cause of death following spinal cord injury, the cost of spinal cord injury care, and the recovery of motor strength after quadriplegia. Key on-site research efforts have focused on preventing deep vein thrombosis and in documenting the course o…

Diencephalic syndrome following cervical spinal cord trauma.

The authors discuss about five cases of diabetes insipidus observed in patients affected by traumatic cervical spine fractures and/or dislocations, without either evident lesions of the cerebral structures at CT scan examination, or important craniocerebral trauma. In all patients polyuria and hyperthermia arose some days after the traumatic accident and regressed spontaneously or after exogeneous vasopressin administration. Vasopressin urinary levels confirmed the presence of a true diabetes insipidus, the origin of which is in largely obscure. A central medullary vasopressin mediated pathway

An autopsy case of stiff-man syndrome is presented. The patient was a 54-year-old man who had suffered from fluctuating rigidity and tonic s … ed. Although the patient was resuscitated, he died nine days after this episode. Examination of the spinal cord revealed loss and degeneration of nerve cells with marked gliosis in the medial motor nuclei of the anterior horns. Bipolar spinal cord stimulation for spasmodic torticollis.

Four years' experience with electrical stimulation of the cervical cord is reported for 18 patients suffering from spasmodic torticollis. Permanent 1,100-Hz stimulation of the C2-C4 level resulted in a marked improvement in 50% of the patients, in a satisfactory result in 27.8% and in an unsatisfactory result in 22.2%. Muscular tension and related pain were reduced within 8 days to 4 months postoperatively. A measurable improvement of head posture rendered objective by a mechanoelectrical measuring device could only be observed after a continuous stimulation over 8-12 months. The procedure is

Spinal cord compression secondary to Gaucher's disease.

A case of compression of the spinal cord and cauda equina secondary to collapse of the T-12 and L-3 vertebral bodies in a patient with type 1 Gaucher's disease is presented. Decompressive laminectomies were done at both the T-12 and L-3 levels with moderate improvement of the patient's neurologic findings. Histologic examinations confirmed the presence of Gaucher's cells in the bone. Review of the literature revealed only one previous report.

Combined brachial plexus and spinal cord trauma.

The clinical features of combined injuries of the brachial plexus and spinal cord in 31 patients are described. Diagnosis of the brachial plexus lesion was delayed in 17 instances, and the reasons for this are discussed. Early diagnosis is important so that definitive treatment and long-term plans can be instituted without delay. This dual lesion constitutes a very severe disability; as a direct result of their brachial plexus paralysis, 9 paraplegics did not achieve their predicted level of independence. Possible mechanisms of injury are suggested. Motorcycle accidents are particularly likely…

Polyarteritis nodosa affecting the spinal cord arteries.

Necrotising arteritis was found in the spinal arterial branches of the lumbar spinal cord and in skeletal muscles, bones, peripheral nerves, and most viscera, except lungs and brain, of a 41-year-old female who died after a protracted illness finally characterized by severe paralysis due to progressive peripheral neuropathy. Spinal cord vascular involvement in polyarteritis nodosa (PAN) is rarely reported despite the high incidence of neurological complications of this disease.

Acute spinal cord compression in Scheuermann's disease.

Acute myelopathy is a rare complication of Scheuermann's disease. Three patients are reported where spinal cord compression occurred at the apex of a kyphos. All were male, aged 14, 18 and 20 years, and each had a profound neurological defect associated with a short, sharp kyphos in the low thoracic region. Each patient underwent anterior decompression and all made an almost full recovery. It is deduced that factors which may influence the onset of cord compression include the angle of kyphosis, the number of segments involved, the rate of change of the angle, local anatomical variations, trau…
