2026 North American Spine Society Patient Safety Position Statement on Education, Training, and Competency for Spine Interventions.

The rapid expansion of minimally invasive, endoscopic, percutaneous, and implant-based spine interventions has outpaced standardized training requirements. This statement provides recommendations based on current training pathways, contemporary practice, and expert consensus, with patient safety as the guiding principle. These recommendations are not intended to define or restrict scope of practice; credentialing decisions remain the responsibility of institutions and governing bodies. Formal surgical spine training currently occurs through orthopaedic surgery residency with spine fellowship o…

Global Spine Outreach (GSO): how to safely establish a sustainable short-term pediatric spine deformity outreach program.

PURPOSE: There are presently no published reports unifying the safe delivery of pediatric spine care in low- and middle-income countries (LMICs). Global Spine Outreach (GSO) is a 501(c)(3) pediatric healthcare nonprofit founded in 2013 with the mission of providing free, transformative surgical care to children with severe spinal deformities worldwide. This study aims to present the GSO model that other organizations can use to initiate, maintain, and grow short-term surgical mission trips (STSMs). METHODS: We retrospectively reviewed 56 consecutive STSMs from 2013 to 2024. GSO evaluated the …

Spine Surgery First Is Associated with Greater 1-year HOOS Improvement in Patients with Hip-spine Syndrome.

STUDY DESIGN: Retrospective Cohort Study. OBJECTIVE: This study aimed to: (1) compare demographic differences between spine-first and hip-first patients; (2) identify differences in perioperative outcomes; (3) determine whether surgical sequencing between hip and spine surgery influences patient reported outcomes (PROMS). SUMMARY OF BACKGROUND DATA: Hip-spine syndrome refers to coexisting lumbar degenerative stenosis and hip osteoarthritis. Surgical counseling remains challenging due to the symptomatic overlap between hip and spine disease, and there is no clear consensus on which disease sh…

Complications in Minimally Invasive Spine Surgery (2013-2024): A Proportional Meta-Analysis -Uniportal Endoscopic Spine Surgery (UESS).

STUDY DESIGN: Systematic review and proportional meta-analysis. OBJECTIVE: To evaluate overall and specific complication rates associated with lumbar uniportal endoscopic spine surgery (UESS) over the past decade. SUMMARY OF BACKGROUND DATA: UESS offers reduced tissue damage, faster recovery, and improved outcomes for lumbar spinal pathologies. However, its steep learning curve may contribute to variable complication rates, reported between 0% and 30%, highlighting the need for a pooled analysis. METHODS: We registered this review in the International Prospective Register of Systematic Revi…

Development of a finite element biomechanical whole spine model for analyzing lumbar spine loads under caudocephalad acceleration.

Background:Spine injury risk due to military conflict is an ongoing concern among defense organizations throughout the world. A better understanding of spine biomechanics could assist in developing protection devices to reduce injuries caused by caudocephalad acceleration (+Gz) in under-body blasts (UBB). Although some finite element (FE) human models have demonstrated reasonable lumbar spine biofidelity, they were either partial spine models or not validated for UBB-type loading modes at the lumbar functional spinal unit (FSU) level, thus limiting their ability to analyze UBB-associated occup…

Comparison of PA spine, lateral spine, and femoral BMD measurements to determine bone loss in ankylosing spondylitis.

To evaluate bone loss in the early- and late-stage ankylosing spondylitis (AS) patients using posteroanterior (PA) and lateral lumbar and femoral bone mineral density (BMD) measurement methods. Eighty-six AS patients and 50 control subjects were enrolled. PA spine, lateral spine, and femur BMD values of patients and controls were measured. The presence of any syndesmophytes or compression fractures was determined. Patients were divided as early (<10 years) and late stage (≥10 years) according to the onset of the inflammatory pain. Mean PA spinal BMD was similar in patients and controls (p = 0.…

Congenital absence of the lumbar spine and sacrum: one-stage reconstruction with subsequent two-stage spine lengthening.

This is a case report of a child born with severe, type IV lumbosacral agenesis. At age 6 years, she had surgical reconstruction of her absent spine and thoracic-pelvic instability with Harrington rods and autogenous bone obtained from simultaneous bilateral knee disarticulations. This arthrodesis healed solidly, and at the age of 10 years 6 months and again at age 12 years 6 months, she underwent spine lengthening by osteotomy and distraction instrumentation, the first time with Moe rods and the second time with Cotrel-Dubousset rods. Both lengthening osteotomies healed without difficulty. Th…

Layer-specific spine remodeling and synaptopodin-linked spine resilience in the mouse dentate gyrus following entorhinal denervation.

Following brain injury, neurons receiving afferent input from the site of injury become denervated. An established model for studying denervation-induced changes of neurons is the denervation of dentate granule cells following unilateral transection of the perforant pathway. In response to denervation, granule cells show a characteristic sequence of spine loss followed by spine recovery. Notably, some spines are more resistant to denervation than others, and the basis of this resilience is unknown. As in vitro data suggest that the actin-modulating protein synaptopodin stabilizes denervated sp…

Hip-Spine and Knee-Spine Syndrome: Is Low Back Pain Improved After Total Hip and Knee Arthroplasty?

INTRODUCTION: Hip and knee osteoarthritis (OA) and low back pain (LBP) are prevalent diseases that can negatively impact daily activities. The concurrent existence of lumbar spine disorders with hip or knee issues forms two syndromes: hip-spine syndrome (HSS) and knee-spine syndrome (KSS). The primary objective of this study is to evaluate the relationship between hip and knee OA and LBP, as well as the changes to LBP after total hip arthroplasty (THA) and total knee arthroplasty (TKA). The secondary objective is to identify the cause of LBP among patients with hip and knee OA. MATERIAL AND M…

Advantages of Revision Transforaminal Full-Endoscopic Spine Surgery in Patients who have Previously Undergone Posterior Spine Surgery.

BACKGROUND: Revision lumbar spine surgery via a posterior approach is more challenging than primary surgery because of epidural or perineural scar tissue. It demands more extensive removal of the posterior structures to confirm intact bony landmarks and could cause iatrogenic instability; therefore, fusion surgery is often added. However, adjacent segment disease after fusion surgery could be a problem, and further exposure of the posterior muscles could result in multiple operated back syndrome. To address these problems, we now perform transforaminal full-endoscopic spine surgery (TF-FES) as…

Outcome of spine surgery in patients with depressed mental states: a Canadian spine outcome research network study.

BACKGROUND CONTEXT: Depression is higher among spine patients than among the general population. Some small studies, but not others, have suggested that depression may be a predictor of worse outcome after surgery. PURPOSE: Determination whether there is an association between depression and worse response to surgery among spine patients. STUDY DESIGN/SETTING: The national, prospective, Canadian Spine Outcome Research Network (CSORN) surgical outcome registry. PATIENT SAMPLE: All patients in the CSORN registry who received surgery for thoracic or lumbar degenerative deformity, stenosis, spo…

Influence of subaxial cervical spine surgery on the sagittal alignment of the cervical and global spine. A prospective observational study.

INTRODUCTION: Sagittal balance is essential for posture, horizontal gaze and avoiding overloading the posterior segment of the spine. RESEARCH QUESTION: Does single- or double-level anterior cervical discectomy and fusion (ACDF) alter (1) cervical and global sagittal parameters and (2) outcomes in patients with kyphotic versus non-kyphotic alignment? MATERIAL AND METHODS: In a prospective cohort, 103 adults underwent 1-2-level ACDF. Standing radiographs were obtained pre-operatively and 24 months post-operatively. Analysed were six cervical parameters, thoracolumbar curves and spinopelvic an…
