Managing ankle injuries in the emergency department.

We review the anatomy and physiology of the ankle joint with attention to the structures most likely to be injured. We discuss the epidemiology of ankle injuries and their physical and radiographic evaluation, including the Ottawa Ankle Rules. Treatment, consultation, and pitfalls are followed by more specific discussions of tendonitis, Achilles tendon injury, tendonous subluxations, tarsal tunnel syndrome, sonovial impingement, and injury to the os trigonum. The references provide a guide for further reading.

[Accurate use of imaging in ankle sprain].

Imaging ankle trauma has two goals: to evaluate bone lesions and to appreciate the capsulo-ligamentous tears. Concerning bone lesions, Ottawa's criteria specify the clinical characteristics to perform radiography in the emergency conditions. These recommendations negate the need for nearly 30% of unnecessary radiographs initially performed. Four views of the tarsis have been selected: anteroposterior, lateral, internal rotation, external oblique. Immediate screening of the ligament is very accurately performed with ultrasound, which depicts the number of bundle concerned and the extent of the

Ankle joint arthroscopy for meniscoid lesions in athletes.

The meniscoid lesion is a frequent but not well known cause of persistent pain in the anterior part of the upper ankle in sports traumatology. It has been described as portions of hyalinized tissue following an inversion sprain of the ankle. Trapping of this formation between the lateral cheek of the talus and the fibula is supposed to be responsible for pain and other symptoms reported by the patient. In 59 arthroscopic procedures on the ankle joint in athletes, meniscoid lesions were seen in 19 cases. Only 1 of these 19 patients showed lateral and anterior instability, and frequent clinical

Is balance impaired by recurrent sprained ankle?

OBJECTIVE: To evaluate uninjured and recurrent sprained ankles during single leg standing, both with and without visual input, and the contribution of related proprioceptive feedback in this event. METHODS: A force measuring system was used for monitoring reaction forces in the anteroposterior and mediolateral directions during single leg standing. Differences between selected variables obtained in the uninjured and sprained ankles were analysed using two way analysis of variance. RESULTS: Foot-ground reaction forces in both anteroposterior and mediolateral directions were the same in normal

Evaluation and diagnosis of ankle injuries.

The evaluation of ankle injuries is a common challenge for family physicians. Diagnosis is made by using knowledge of the anatomy and function of the ankle joint to aid in taking an adequate history and performing an appropriate physical examination. The patient should be questioned about the mechanism of injury, previous injury, disability, treatment and pain. The ankle should be evaluated for ecchymosis, swelling, areas of tenderness and laxity. These measures help to determine what, if any, additional diagnostic procedures may be needed. Most ankle sprains are lateral, affecting the anterio

The role of fatigue in susceptibility to acute muscle strain injury.

We investigated the role of fatigue in muscle strain injuries using the extensor digitorum longus muscles of 48 rabbits. The muscles of the rabbits were fatigued by 25% or 50% then stretched to failure and compared with the contralateral controls. Three rates of stretch were used. The force to muscle failure was reduced in the fatigued leg in all groups (range, 93% to 97.4% compared with the controls). The change in muscle length in the fatigue groups was not different from the controls. The amount of energy absorbed in the fatigued muscle was 69.7% to 92% that of the energy absorbed in the co

Muscle strain during aerial combat maneuvering exercise.

BACKGROUND: Little is known about the in-flight muscular strain of fighter pilots. HYPOTHESIS: The purpose of this study was to measure fighter pilots' mean and peak muscular strain during aerial combat maneuvering exercises. The results obtained were compared against existing ergonomic recommendations. METHODS: Six pilots volunteered to serve as test subjects. Their mean age (+/- SD) was 28.5 +/- 5 yr, height 181 +/- 7 cm, and weight 75 +/- 10 kg. They performed one-to-one dog-fight exercises in the morning and in the afternoon. During the flights, the pilots' electromyographic activity (EM

Effects of a semirigid and softshell prophylactic ankle stabilizer on selected performance tests among high school football players.

No prophylactic ankle stabilizer performance study has been conducted to date using high school football players, ie., players who are at risk of ankle sprains. In addition, the effect that prophylactic ankle stabilizer use has on performance by player position is unknown. The purpose of this study was to determine the effect of a semirigid and softshell prophylactic ankle stabilizer on performance by football player position in events testing vertical jump, speed, and agility. Twenty-five high school football players participated in the study. All subjects denied any history of ankle injury w

Injuries of the distal radioulnar joint.

Injuries of the distal radioulnar (DRU) joint are common. If the joint is unstable or discongruous, attempts should be made to restore anatomic alignment and stability. Although most acute injuries of the DRU joint are easily treated, they are often overlooked or misdiagnosed because they usually occur in association with other major injuries of the upper limb. Acute and chronic abnormalities are described briefly and the treatment for each is discussed. Appropriate diagnosis and management of the acute injury will yield a much higher success rate than reconstructive procedures to correct a ch

Effects of the direction and distance of horizontal arm movements on local muscular strain--a fundamental study on the teller workplace in bank.

This study was conducted to examine the relative differences in the muscular strain of the hand-arm-shoulder system in the repetitive arm movements which would directly depend on the layout of devices at teller counter. The subjects were five young male adults. The manual handling task was standardized and the repetition frequency was in self-pace. The arm movements were characterized by the different direction measured from the frontal plane of the subject between 0 degrees to 150 degrees combined with the distance measured from the acromion to the fixed points of 300, 500 and 700mm. The move

Tarsal tunnel syndrome caused by strain of an anomalous muscle: an MRI-specific diagnosis.

Tarsal tunnel syndrome (TTS) is a compression neuropathy of the posterior tibial nerve in the ankle, commonly caused by a mass lesion such as ganglion, tumor, fracture, or post-traumatic fibrosis. Although anomalous muscle is relatively common in the ankle, there has been only one report to date implicating it in TTS. This case demonstrates MR findings of a strained flexor digitorum accessorius longus muscle as an unusual cause of TTS.

[An analysis of overstrain injuries in rock climbing].

Between spring and autumn 1990 a study was performed with the goal of recording and classifying overstrain injuries due to rock-climbing and to define their causes. Of the 332 climbers participating in the study, 114 (34.4%) had suffered from at least one overstrain injury. The degree of climbing skill proved to be the main risk factor; with increasing climbing skills of the observed persons the percentage of injuries increased very substantially. The degree of climbing skill also was the only significant difference between injured and non-injured persons--injured persons had a climbing skill

Wrist and hand injuries in platform diving.

To permit a smooth entry into the water, platform divers assume a special position with their hands. An older method consists of hitting the water with both fists closed while holding one thumb in the palm of the other hand. In a newer technique fingers and wrists are hyperextended, the forearms pronated, and the hands overlapped. In this series of 21 divers, the injuries included 2 fractures and 5 sprains treated by conservative methods. Two divers required surgery: one with loose bodies in the radiocarpal articulation, the other with chronic rupture of the trapeziometacarpal ligament and syn

Flight helmet weight, +Gz forces, and neck muscle strain.

The aim of this study was to investigate the effect of flight helmet weight on cervical erector spinae muscle strain under high +Gz forces. Two helmets of different weight were compared by recording inflight neck muscle activities with a portable surface-integrated EMG (IEMG) device. The obtained IEMG activities were normalized by comparing them with activities representing maximal voluntary contraction (MVC) of the muscles. Two test pilots carried out a total of 16 flights consisting of a series of different maneuvers. The results indicate that a lighter flight helmet may--at least in some pi

Arthroscopic treatment of anterolateral impingement of the ankle.

We studied 31 patients (17 females, 14 males; average age, 34) with more than 2 years of followup who had chronic anterolateral ankle pain following inversion injury. All had failed to respond to at least 2 months of conservative treatment and had negative stress radiographs to rule out instability. On physical examination, tenderness was localized to the anterolateral corner of the talar dome. Magnetic resonance imaging was the most useful diagnostic screening test, showing synovial thickening consistent with impingement in the anterolateral gutter. At an average of 24 months after injury, al

Partial carpal arthrodesis for third degree carpal sprains. A review of 45 carpi.

The efficacy of partial carpal arthrodesis was evaluated retrospectively in 39 dogs (45 carpi) with severe sprains of the middle carpal joints, the carpometacarpal joints, or both. The carpometacarpal joint was the most frequently injured joint. Jumping or falling from heights was the cause of injury in 49% of these animals. Of the 25 owners who responded to a mailed questionnaire (mean follow-up, 32 months), all stated their animal had improved or greatly improved after partial carpal arthrodesis and all were pleased or very pleased with the final surgical result. Hyperextension persisted in

Talar impingement by the anteroinferior tibiofibular ligament. A cause of chronic pain in the ankle after inversion sprain.

Seven patients who had pain in the anterior aspect of the ankle were found to have a thickened distal fascicle of the anteroinferior tibiofibular ligament. Each patient had a history of an inversion sprain of the ankle followed by chronic pain in the anterior aspect of the ankle. The thickened distal fascicle was resected without loss of stability of the ankle. Five patients needed débridement of an area of abraded hyaline cartilage on the anterolateral aspect of the talus. Six patients were followed for a mean of thirty-nine months (range, twenty-four to fifty-nine months). Four of them had n
