Primary and drug-induced disorders of water homeostasis in psychiatric patients: principles of diagnosis and management.

Psychotropic drugs, as well as some psychiatric disorders, can produce neurotoxic and life-threatening abnormalities of water and electrolyt … hrogenic diabetes insipidus. Clinical signs in these disorders range from lethargy and confusion to stupor, seizures, coma, and death. This overview provides a conceptual framework for differentiating among and safely managing these relatively common disorders.

Imidazoline intoxication in children.

A 15-month-old girl with rhinopharyngitis was treated with a nasal solution containing the imidazoline derivative naphazoline. She rapidly developed profound CNS depression with stupor, hypothermia, hypoventilation and bradycardia. All symptoms disappeared within 24 h. The symptomatology of 18 other paediatric cases of naphthylimidazoline exposure reported to the Belgian National Poison Centre, is also discussed. Imidazoline intoxication due to overdose or accidental ingestion but also after normal therapeutic usage is frequent in children. It can cause severe CNS depression, especially in ver…

Insulin-secreting pancreatic (islet cell) carcinoma in a cat.

A functional, insulin-secreting pancreatic (islet cell) carcinoma was diagnosed in a 17-year-old male Siamese cat. Diagnosis was made on the basis of clinical signs (i.e., seizures and stupor) that resolved temporarily after correction of hypoglycemia with feeding or intravenous administration of glucose, the finding of an inappropriately increased serum insulin concentration in the face of hypoglycemia, and prolonged resolution of hypoglycemia after surgical removal of the tumor. Primary islet cell tumor of the pancreas was confirmed by biopsy. The cat died 18 months later, and necropsy revea…

[Neurological manifestations in hemorrhagic colitis associated with Escherichia coli O 157: H 7].

From September through November 1990, an outbreak of hemorrhagic colitis associated with Escherichia coli O 157: H 7, occurred in a kindergarten in Saitama, Japan. Some of the patients suffered from neurological symptoms such as stupor, deep coma and/or convulsions in the acute stage, and/or action tremors, nystagmus, incontinence, phrenic nerve palsy in the later stage. Serum complements decreased more in the patients with neurological symptoms than in the patients without them. The verotoxin elaborated by E. coli O 157: H 7 was considered to cause the neurological symptoms, on the basis of t…

Extra-pontine myelinolysis secondary to hypernatremia induced by postpartum water restriction.

A young adult female with restricted water intake during the postpartum period presented with history of progressive weakness, dizziness and tendency to fall with generalized slowing of movement. On examination, patient was anaemic, febrile and stuporous. Investigations revealed hypernatremia, delta waves in electroencephalogram (EEG) and features suggestive of extra-pontine myelinolysis on magnetic resonance imaging (MRI) of brain. After correcting hypernatremia and instituting anti-cholinergic therapy, there was a gradual but steady improvement in neurological symptoms of the patient over a …

Nontraumatic "spindle coma": clinical, EEG, and prognostic features.

"Spindle coma" denotes the association of alteration of consciousness with electroencephalographic (EEG) activity resembling slow-wave sleep. Clinical, EEG, and pathologic data were reviewed for 36 patients with this syndrome, who were divided into two groups, based on the level of consciousness at the time of the initial neurologic evaluation. Fifteen patients were stuporous or semicomatose; none exhibited abnormalities of brainstem motor function, and 14 survived. Twenty-one patients were comatose; 20 had abnormalities of brainstem motor function, and only 4 survived. The outcome of nontraum…

[Adaptogenic effect of mebikar in emotional stress, physical exertion and hypoxia].

Adaptogenic effect of mebikar was revealed in experiments on cats, mice and rats. The drug was shown to decrease the emotional stress and to completely remove the stupor-like state in cats induced by stress situation and pain stimulation. The drug slightly increased the lifespan of animals under hypoxic hypoxia and raised the limits of maximum tolerable physical load. It lowered the accumulation of hypoxic lactate. Electron microscopy and biochemical analysis revealed the mebikar-induced increase in glycogen content in muscle cells of the myocardium and in skeletal muscles. The data obtained i…
