Postoperative stability on lateral radiographs in the surgical treatment of pertrochanteric hip fractures.

BACKGROUND: Fixed-angle sliding hip-screw devices are commonly used to treat pertrochanteric fractures. The controlled impaction between the head and neck fragment and the femoral shaft fragment is crucial. However, the poor quality of fracture reduction can intercept controlled impaction and lead to excessive sliding. We hypothesized that excessive sliding occurs when most of the impaction is placed on the fragile posterior cortex of the fracture site. METHODS: This retrospective study included 128 AO/OTA type 31-A1 or 31-A2 fractures treated with fixed-angle sliding hip-screw devices. Cases

Giant Cell Tumor of the Proximal Femur with Pathological Fracture of Femoral neck.

INTRODUCTION: Giant cell tumor (GCT) of bone is a locally aggressive, benign neoplasm, accounting for approximately 20% of all bone tumors. While the distal femur and proximal tibia are the most common locations for GCTs, with the majority arising in the epiphyseal regions, their occurrence in the proximal femur is relatively rare, representing only 5.5% of cases. These tumors pose significant management challenges due to their tendency to cause pathological fractures, aggressive local behavior, and involvement of critical weight-bearing bones. Effective treatment requires careful consideratio

3D printed patient-specific fixation plates for the treatment of slipped capital femoral epiphysis: Topology optimization vs. conventional design.

Orthopedic plates are commonly used after osteotomies for temporary fixation of bones. Patient-specific plates have recently emerged as a promising fixation device. However, it is unclear how various strategies used for the design of such plates perform in comparison with each other. Here, we compare the biomechanical performance of 3D printed patient-specific bone plates designed using conventional computer-aided design (CAD) techniques with those designed with the help of topology optimization (TO) algorithms, focusing on cases involving slipped capital femoral epiphysis (SCFE). We establish

Rigid External Le Fort I Distraction Followed by Secondary Bone Grafting for Maxillary Advancements in Patients With Cleft Lip and Palate.

INTRODUCTION: Maxillary hypoplasia after cleft lip and palate (CLP) repair can result in significant functional and aesthetic impairments. Le Fort I osteotomy & advancement and Le Fort I distraction osteogenesis are standard treatment options for individuals with CLP-associated midface retrusion. However, both of these modalities continue to be associated with a high relapse rate. This study describes surgical outcomes of a 2-stage technique utilizing distraction osteogenesis combined with bone grafting and rigid fixation, which may optimize skeletal stability by reducing relapse. METHODS: A

Biomechanics and finite element analysis of a novel plate designed for posterolateral tibial plateau fractures via the anterolateral approach.

Surgical management of posterolateral tibial plateau (PLTP) fractures is challenging. One reason for this challenge is the lack of suitable internal fixation devices. Our aim was to introduce a novel plate via the anterolateral approach for managing PLTP fractures. The biomechanical testing and finite element analysis (FEA) were performed. PLTP fracture models were created using synthetic tibias (n = 10 within each group). These models were randomly assigned to three groups (groups A-C) and fixed with the lateral locking plate, the posterior buttress plate, and the novel plate, respectively. T

Percutaneous Spine Fusion Combined with Whole-Body Traction in the Acute Surgical Treatment of AO A- and C-Type Fractures: A Technical Note.

BACKGROUND: A diminished level of pain following the operation and shortened hospitalization are the distinct advantages of minimally invasive spine surgery (MISS). However, manipulating the spine with additional MISS tools (e.g., distraction and compression devices) is often cumbersome. Our paper draws attention to a cost-free, fast, indirect decompression method that can be used in the acute treatment of thoracolumbar spine fractures. The presented method involves ligamentotaxis by whole-body traction in the operating room combined with percutaneous spine fixation. METHODS: Fifteen patients

Complications After Hip Nailing for Fractures.

Pertrochanteric fractures in elderly patients represent a major health issue. The available surgical options are fixation with extramedullary devices, intramedullary nailing, and arthroplasty. Intramedullary nailing for hip fractures has become more popular in recent years. Advantages of intramedullary nailing for hip fracture fixation include a more efficient load transfer due to the proximity of the implant to the medial calcar, less implant strain and shorter lever arm because of its closer positioning to the mechanical axis of the femur, significantly less soft tissue disruption and perios

Treatment of distal biceps tendon rupture: why, when, how? Analysis of literature and our experience.

INTRODUCTION: The rupture of the distal biceps tendon is a relatively uncommon lesion. Even if conservative treatment may be an option in low demanding patients, young and active subjects may benefit from an early surgical reinsertion. Many techniques and fixation devices have been described, but in the literature, there are no clinical evidences that show the superiority of any of these. In this article, we report an analysis of the "state of the art" and our case series of surgical reinsertion with the double approach transosseous technique. MATERIALS AND METHODS: Between 2003 and 2013, 26

Patient Reported Outcomes for Maisonneuve Fractures using PROMIS Physical Function Scores.

BACKGROUND: Limited research exits surrounding Maisonneuve fractures due to rarity. Additionally, even less data has been published evaluating the patient reported outcomes of surgically treated Maisonneuve fractures using the Patient-Reported Outcomes Measurement Information System (PROMIS) Physical Function (PF) and Pain Intensity (PI) score. The aim of this study was to put forth a cohort of patient reported outcomes using PROMIS PF and PROMIS PI. METHODS: Maisonneuve fracture patients with minimum 12-month follow-up treated at a level-1 trauma center from 2006 to 2020 completed PROMIS PF

Correction of metatarsus primus varus with an opening wedge plate: a review of 18 procedures.

UNLABELLED: The opening base wedge osteotomy is a safe and useful surgical alternative for correction of moderate to severe hallux valgus deformities with substantial metatarsus primus varus. The authors combine the modified McBride bunionectomy with a proximal first metatarsal opening base wedge osteotomy. Osteotomy stabilization was achieved without bone grafting with a titanium fixation plate specifically designed for opening proximal osteotomies. To assess outcomes achieved by the use of this fixation device, we reviewed the records of 18 procedures (16 patients). Preoperative and postoper

Revision surgery of spinal dynamic implants: a literature review and algorithm proposal.

INTRODUCTION: Dynamic stabilization of the spine has been performed since the 1990s with the double purpose of restoring spinal segmental stability and allowing residual movement at the operated level. When we take into account the different motion-preserving devices and the spinal areas where they are applied, we can identify three categories of spinal implants: anterior cervical, anterior lumbar, and posterior lumbar. However, as in all prosthetic procedures performed in orthopedic surgery, the life span of a joint replacement device is a central topic of discussion, and this is true also fo

Removal of bent intramedullary nail in diaphyseal segmental femoral fracture with intra-articular distal femoral fracture after below-knee amputation.

Refracture accompanied by nail bending in healed or unhealed femoral fractures is a rare complication, and its removal is challenging intraoperatively. A man in his mid-20s presented to the emergency department with a deformed thigh with a puncture wound in a below-knee amputee limb following a road traffic accident, and radiographs revealed a diaphyseal segmental femur fracture with an intra-articular distal femur fracture with a bent femur nail in situ. Attempts to remove a bent femoral nail frequently fail non-invasively and may cause iatrogenic damage. Cutting the bent femur nail using a c

Pediatric gastrostomy tubes and techniques: making safer and cleaner choices.

BACKGROUND: Gastrostomy tube placement is a common procedure that can be accomplished with a variety of techniques, each with its attendant complications. In an effort to standardize practice at our institution, we retrospectively evaluated complications including early dislodgement requiring operative repair, leaks, and granulation tissue to determine the optimal technique. MATERIALS AND METHODS: A retrospective cohort study (June 2008-July 2014) evaluating children (<18) receiving gastrostomy tubes was completed. We recorded demographic data, placement technique, and postoperative complicat

T-fastener migration after percutaneous gastropexy for transgastric enteral tube insertion.

BACKGROUND/AIMS: To determine the prevalence and time-course of t-fastener migration after gastropexy deployment. METHODS: We reviewed our procedural database for all percutaneous gastrostomy and gastrojejunostomy tube insertions performed over a 14-month period using a widely accepted t-fastener kit for gastropexy (Kimberly-Clark). Of 201 patients, 71 (41 males, 30 females; mean age, 56 years) underwent subsequent abdominal computed tomography (CT) imaging. The location and associated findings of each t-fastener were retrospectively recorded for each CT scan performed after the tube insertio

Use of a ventral cervical retractor system for minimal access transforaminal lumbar interbody fusion: technical case report.

OBJECTIVE: Interbody lumbar fusion techniques have become increasingly popular because of improved rates of fusion, restoration of disc and foraminal height, and promotion of lordosis. Morbidity has been associated with the exposure for these fusions, potentially leading to muscle injury, trunk muscle weakness, and pain. Minimal access surgery has been advocated to limit the exposure related morbidity. Tubular retractors are available, which allow dilation of the muscle through a small incision. The downside of these dilators is the expense required to use them and a generally cumbersome desig
