Supernumerary extensor pollicis longus tendon masking an extensor pollicis longus rupture following non-surgical treatment of a distal radius fracture.

Extensor pollicis longus tendon pathology is a recognised complication following non-surgical treatment of non-displaced distal radius fract … inger metacarpophalangeal joint extension. An extensor indicis proprius to extensor pollicis longus tendon transfer was performed. At her final 6-month follow-up, she had painless full active thumb motion comparable to her contralateral side.

Pull-in suture: a novel reconstruction technique for tendon avulsion injury at the musculotendinous junction associated with forearm open fracture.

We present three cases of strong one-staged tendon reconstruction for musculotendinous junction avulsion tendon injuries, and called it a 'pull-in suture'. The clinical outcomes of this method are comparable to those of tendon transfer; it is an effective reconstruction method that should be considered as an initial treatment procedure.

Primary and secondary microvascular reconstruction of the upper extremity.

Tissue defects of the upper extremity may result from trauma, tumor resection, infection, and congenital malformation. Restoration of anatomy and functional integrity may require microsurgical free flap transfer for coverage of bones, nerves, blood vessels, or tendons. Microsurgical tissue transfer also may be required prior to secondary reconstruction, such as tendon transfers or nerve or bone grafts. This article addresses indications for upper extremity reconstruction using microsurgical tissue transfer flap selection and strategies including primary and secondary reconstruction.

Arthroscopic all-inside ATiFL's distal fascicle transfer for ATFL's superior fascicle reconstruction or biological augmentation of lateral ligament repair.

PURPOSE: Tendon grafts are often utilized for reconstruction of the lateral ligaments unamenable to primary repair. However, tendon and ligaments have different biological roles. The anterior tibiofibular ligament's (ATiFL) distal fascicle may be resected without compromising the stability of the ankle joint. The aim of this study is to describe an all-arthroscopic and intra-articular surgical technique of ATiFL's distal fascicle transfer for the treatment of chronic ankle instability. METHODS: Five unpaired cadaver ankles underwent arthroscopic ATiFL's distal fascicle transfer using a non-ab

Restoration of opposition of the fifth finger of the hand.

I describe a technique for surgical restoration of opposition of the fifth finger, which includes transfer of the extensor digiti quinti tendon between the fifth and fourth metacarpals towards the dorsoulnar aspect of the neck of the fifth metacarpal, where the transferred tendon is fixed to the opponens digiti quinti tendon. I have operated on 11 patients with paralysis of all the hypothenar muscles, all of whom were followed up from one to three years postoperatively. Ten were successful.

Function after correction of a clawed great toe by a modified Robert Jones transfer.

We carried out a cross-sectional study in 51 patients (81 feet) with a clawed hallux in association with a cavus foot after a modified Robert Jones tendon transfer. The mean follow-up was 42 months (9 to 88). In all feet, concomitant procedures had been undertaken, such as extension osteotomy of the first metatarsal and transfer of the tendon of the peroneus longus to peroneus brevis, to correct the underlying foot deformity. All patients were evaluated clinically and radiologically. The overall rate of patient satisfaction was 86%. The deformity of the hallux was corrected in 80 feet. Catchin…

Attritional rupture of the extensor pollicis longus tendon due to calcified extensor carpi radialis tendon group: a case report.

In this article, we report a case with spontaneous rupture of extensor pollicis longus tendon due to calcified extensor carpi radialis tendon group, which has not been published in the literature previously. Successful thumb extension was achieved in a 68-year-old male patient with the transfer of the extensor indicis proprius tendon at two months postoperatively. It appears that calcified extensor carpi radialis tendon group constitutes high risk for a possible attritional rupture of the extensor pollicis longus tendon.

Feasibility of the abductor pollicis longus hemitendon transfer for thumb opposition: An anatomical study.

INTRODUCTION: The abductor pollicis longus (APL) presents two muscle bellies and multiple accessory tendons available for transfer, amongst these an accessory tendon inserting on the thenar aponeurosis (APLTh). Edgerton described an opponensplasty using the whole APL tendon, but its results were limited due to the short size of the donor tendon. HYPOTHESIS: The purpose of this study is to assess the feasibility of transferring the APLTh for thumb opposition. MATERIAL AND METHODS: Eleven cadaver upper limbs were dissected to assess the presence of the two heads of the APL as well as their mai

Functional and Cosmetic Reconstruction of Floating Thumb: A Thumb-preserving Technique.

For the severely hypoplastic thumb, including floating thumb, it is difficult to acquire function while preserving the thumb. Pollicization using the index finger after removing the hypoplastic thumb is usually indicated for such cases. However, most parents of children undergoing surgery for deformed hands desire retaining the digit number, 1 thumb and 4 fingers. A floating thumb has a neurovascular band in its pedicle, and we can identify and divide the band with precision for preservation, although it seems impossible to retain it. For acquiring function while retaining the thumb, we develo

Anomalous Extensor Indicis Proprius Muscle Extending Beyond the Extensor Retinaculum: A Case Report.

CASE: During dissection of the upper limb of a cadaver in preparation for a first-year anatomy course, an extensor indicis proprius (EIP) variant was discovered with its muscle belly extending distal to the extensor retinaculum and beyond what has been previously described in the literature. CONCLUSION: EIP is commonly used as a tendon transfer for extensor pollicis longus rupture. Few anatomic variants of EIP have been reported in the literature, but such variants should be considered because of their consequences to the success of tendon transfer and potential implications for diagnosis of …

A histological study on the tendons at chiasma plantare in pes cavus.

INTRODUCTION: In the dysfunction of large muscles of the leg, tendon transfer surgery is found to be very helpful in restoring the normal function of these muscles. The tendons involved in the chiasma plantare play a major role in this regard. OBJECTIVE: The present cadaveric study has been carried out in cadavers presenting pes cavus. MATERIAL AND METHODS: Cadaveric feet presenting pes cavus were identified based on their foot prints. All these tendons and their interconnections were subjected to histological procedures. The sections of the tendons were stained with hematoxylin and eosin in…

Extensor tendon ruptures in rheumatoid wrists.

BACKGROUND AND AIMS: Rheumatoid arthritis is a chronic inflammatory disease. The associated involvement of hands and tendons is over 90% and impairs overall function. In the course of the disease, the joints are often operated on. During this operation, ruptures of the extensor tendons are found by chance without the patients noticing them. The aim of this retrospective study is the prevalence of extensor tendon rupture. Which tendon is destroyed most frequently? How can the functional outcome be measured after reconstruction? MATERIALS AND METHODS: From 1572 operations on rheumatoid wrists,

[Secondary repair of the extensor pollicis longus].

Repair of extensor pollicis longus, even when it is imperfect, generally gives fairly good results. This tendon is exposed to injury particularly as it leaves the pulley around Lister's tubercle and especially after a slightly displaced Pouteau-Colles fracture. Very marked retraction of the proximal part of the tendon is almost constant after three weeks and repair by secondary suture is rarely possible. For this reason, a palmaris longus graft or transfer of the intrinsic extensor of the index finger is generally required. The analysis of the results of 23 cases shows that, in order to obtain
