Cross-sectional investigation of pneumococcal vaccination patterns in HIV patients at a tertiary care hospital in Riyadh, Saudi Arabia.

BACKGROUND: People living with HIV (PLWH) are recognized as a high-risk population for opportunistic infections, including community-acquired pneumonia (CAP). Therefore, the United States Centers for Disease Control and Prevention (US-CDC) and Saudi Ministry of health (MOH) advocate for pneumococcal vaccination among all PLWH as a crucial preventive measure against pneumococcal infections. Hence, this study aims to investigate the prescribing patterns of pneumococcal vaccines for PLWH at a tertiary care hospital. As PCV15 and PCV20 were not available until 2022, this study focuses on the presc

An Epidemiological and Clinical Study of Monkeypox in Changsha, China: A Retrospective Analysis of HIV-Infected and Non-HIV-Infected Patients from June to December 2023.

BACKGROUND: The World Health Organization (WHO) declared Human Monkeypox (mpox) as a public health emergency of international concern (PHEIC) in July 2022. Due to border quarantine and isolation measures implemented from January 2020 to December 2022, Beijing did not report its first locally transmitted case of mpox until May 31, 2023, which represented a delayed occurrence compared to other countries. The aim of this observational analysis is to describe demographical data, symptoms presentation and clinical course till outcome of patients diagnosed with monkeypox (mpox) from June to December

A model for predicting poor survival in patients with cirrhosis undergoing portosystemic shunt embolization.

BACKGROUND AND AIM: Portosystemic shunt embolization (PSSE) is a promising treatment for hepatic encephalopathy (HEP) and gastric varix (GV) in cirrhotic patients with a spontaneous portosystemic shunt. However, PSSE may worsen portal hypertension causing hepatorenal syndrome, liver failure, and mortality. This study aimed to develop and validate a prognostic model that helps identify patients with a risk of poor short-term survival after PSSE. METHODS: We included 188 patients who underwent PSSE for recurrent HEP or GV at a tertiary center in Korea. To develop a prediction model for 6-month

VascuFit: vascular effects of non-linear periodized exercise training in sedentary adults with elevated cardiovascular risk - protocol for a randomized controlled trial.

BACKGROUND: Early vascular aging (EVA) is increasingly prevalent in the general population. Exercise is important for primary cardiovascular prevention, but often insufficient due to ineffective training methods and a lack of biomarkers suitable to monitor its vascular effects. VascuFit will assess the effectiveness of non-linear periodized aerobic exercise (NLPE) in a non-athletic sedentary population to improve both established and promising biomarkers of EVA. METHODS: Forty-three sedentary adults, aged 40-60 years, with elevated cardiovascular risk will either engage in 8 weeks of ergomete

Primary amenorrhea in adolescent girls: normal coitus or not? Always take a look in the physician's office.

BACKGROUND: Primary care physicians are frequently faced with the challenge of evaluating primary amenorrhea in adolescent girls. Approximately 15% of these women have abnormal genital examination, with Müllerian agenesis being the second most frequent cause. We report two cases of adolescents with Müllerian agenesis that presented to a tertiary adolescent medicine center with primary amenorrhea and the very rare sexual phenomenon of urethral coitus. The aim of this report is to emphasize the importance of performing a genital examination in girls who present with amenorrhea in the primary car

Esophageal atresia and tracheoesophageal fistula.

Esophageal atresia, with or without tracheoesophageal fistula, is a fairly common congenital disorder that family physicians should consider in the differential diagnosis of a neonate who develops feeding difficulties and respiratory distress in the first few days of life. Esophageal atresia is often associated with other congenital anomalies, most commonly cardiac abnormalities such as ventricular septal defect, patent ductus arteriosus or tetralogy of Fallot. Prompt recognition, appropriate clinical management to prevent aspiration, and swift referral to an appropriate tertiary care center h

Evaluation, treatment, and transport of pediatric patients with shock.

Early recognition and correct treatment of shock remain the most important keys to preventing the death and disability frequently caused by this condition in children. The pediatrician plays a vital role in this process and in referral of the patient for transport to tertiary care centers, where shock is best managed. The transport environment creates special challenges in initial stabilization and ongoing treatment of shock. Discussion centers on clinical clues to recognition, on simple measures available to increase tissue oxygenation, and on the issues of pretransport and transport treatmen

Cataracts and cataract operations (1).

Cataract operations are commonly performed in either a primary-care or a tertiary-care center. A plethora of information (some of which is unfounded) has been disseminated in the lay news media about the advances in the surgical techniques of cataract removal. This article reviews some of the recent concepts about the formation of cataracts and the modern approaches for their extraction. It is directed at the nonophthalmologic primary-care physician who refers patients for evaluation or performs the ancillary tests in preparation for a cataract operation. Space limitations prevent a complete r

[Antibiotic treatment for traumatic brain injury patients without positive bacterial cultures].

Prophylactic administration of antibiotics is prevalent for traumatic patients including head injury, when the patients have contaminated wounds, CSF leakage, and multiple injuries. In cases with prolonged fever and inflammatory signs, other antibiotics must be selected without confirming the infections by cultures. Usually, 1st or 2nd generation cefalosporins are selected as empiric therapy for traumatic patients, but, successive antibiotics are usually selected according to the situation. In this study, we analyzed 60 cases of head injured patients with Glasgow Coma Scale under 12, in terms

The global burden of COPD: epidemiology and effect of prevention strategies.

Chronic obstructive pulmonary disease (COPD) affects more than 400 million people, continues to be the third cause of death worldwide, and has substantial regional variations in prevalence, risk factors, and temporal trends. COPD is associated with major socioeconomic and health consequences, with low-income and middle-income countries contributing the most to its growing burden. COPD prevalence among women is projected to increase, while that among men will decrease. This difference is possibly because tobacco consumption among women is decreasing at a slower rate than among men. Also, women
