There have been a wide variety of reported renal parenchymal diseases associated with inflammatory bowel disease, ranging from interstitial nephritis to amyloidosis to immune complex glomerulonephritis. Two pediatric cases of renal parenchymal pathology in association with Crohn disease are presented. The first is an 11-year-old child who presented with recurrent bouts of gross hematuria, biopsy-proven IgA nephropathy, and later developed Crohn disease 4 years after the initial presentation. In this simple high-performance thin-layer chromatographic technique for evaluating porphyrin excretion, porphyrins rapidly extracted from urine or feces are separated on reversed-phase octadecylsilyl (C18)-bonded silica thin-layer chromatography plates. We observed > 12 distinct bands of different porphyrins by viewing the plates under long-wave ultraviolet light. Intestinal obstruction proximal to a transition zone without an interposed physical barrier usually indicates Hirschsprung disease. The authors report one case of focal small bowel muscular thinning just distal to a transition zone that produced clinical and radiographic findings that simulated long-segment Hirschsprung disease in a 2-day-old infant.

A thin layer chromatographic method for the identification of phenothiazine derivative drugs was studied collaboratively by 8 laboratories. Twenty phenothiazine drugs were examined by each collaborator. The identification scheme depends on the color of the sprayed spots and the Rf values relative to the Rf of chlorpromazine (RCHL) in 4 solvent systems. A young man with pectus excavatum and Gilbert's syndrome was found to have an abnormal hepatic photoscan, repeatedly interpreted as showing a mass lesion in the left lobe. Although his habitus suggested that it was a normal anatomic variant, numerous diagnostic procedures did not convincingly exclude disease of the liver. Twelve patients with biliary-enteric anastomoses were examined with transhepatic cholangiography to evaluate the etiology of epigastric pain, cholangitis, abnormal liver function tests, pancreatitis, or transient or persistent jaundice. Results of 567 thin-needle aspiration biopsies of head and neck lesions are reviewed. Cases included lymph nodes, salivary glands, thyroid, soft tissue and bone lesions. For the entire series, 17 patients had unsatisfactory aspirations. There were 12 false negative reports and 4 false positive reports, rates of 2.1% and 0.7%, respectively. No radical treatment resulted from false positive diagnoses and no patient delay in treatment occurred because of false negative reports.
