A case involving a bovine aortic arch contributing to blunt thoracic aortic injury of triple rupture close to the pericardial reflection.

A blunt thoracic aortic injury is a fatal condition that often requires emergency surgery. Although injuries to the aortic isthmus are common in trauma patients, the proximal aortic arch often remains intact. Here we describe the case of an 80-year-old man with a bovine aortic arch who presented with an aortic injury including three free rupture sites and multiple bone fractures caused by a motorcycle accident. He underwent successful total arch replacement and the frozen elephant trunk technique. The major rupture was situated at the proximal aortic arch with the tear continuing from the peri

A meta-analysis of the timing of endovascular repair for blunt thoracic aortic injury: Safety and efficacy of early vs. delayed treatment.

BACKGROUND: Blunt thoracic aortic injury (BTAI) carries a high risk of mortality. Thoracic endovascular aortic repair (TEVAR) is a standard treatment. However, the optimal timing of TEVAR remains debated. METHODS: Embase, Web of Science, PubMed, Cochrane, and Chinese databases were searched for studies comparing emergency (≤24h, n = 4233) and delayed (>24h, n = 1457) TEVAR. Meta-analyses were conducted using Stata 15.0. Subgroup and sensitivity analyses were performed. RESULTS: Among 5690 patients (13 studies), emergency TEVAR (n = 4233) showed higher in-hospital mortality (OR = 1.99

Incidental Thoracic Th1 Spina Bifida Occulta Detection Following Sports Injury: A Case Report with Comprehensive Radiological Insights.

INTRODUCTION: Spina bifida is a neural tube defect and the incidence in the thoracic region is extremely rare. We reported an incidental finding of thoracic spina bifida occulta at the first thoracic vertebra following a sport-related injury. CASE REPORT: A 28-year-old male presented to our emergency department; the patient complained of middle thoracic back pain without any sensory-motor deficits after a fall during a football game. An X-ray and a thoracic computed tomography (CT) scan were performed and highlighted an incomplete fusion of the Th1 spinous process and lamina. A follow-up magn

Central nervous injury risk factors after endovascular repair of a thoracic aortic aneurysm with type B aortic dissection.

BACKGROUND: Aortic dissection is the deadliest disease of the cardiovascular system. Type B aortic dissection accounts for 30%-60% of aortic dissections and is mainly treated by endovascular repair of thoracic endovascular aneurysm repair (TEVAR). However, patients are prone to various complications after surgery, with central nervous system injury being the most common, which seriously affects their prognosis and increases the risk of disability and death. Therefore, exploring the risk factors of central nervous system injury after TEVAR can provide a basis for its prevention and control. AI

Enhancement of motor functional recovery in thoracic spinal cord injury: voluntary wheel running versus forced treadmill exercise.

JOURNAL/nrgr/04.03/01300535-202503000-00028/figure1/v/2024-06-17T092413Z/r/image-tiff Spinal cord injury necessitates effective rehabilitation strategies, with exercise therapies showing promise in promoting recovery. This study investigated the impact of rehabilitation exercise on functional recovery and morphological changes following thoracic contusive spinal cord injury. After a 7-day recovery period after spinal cord injury, mice were assigned to either a trained group (10 weeks of voluntary running wheel or forced treadmill exercise) or an untrained group. Bi-weekly assessments revealed

Early Versus Delayed Thoracic Endovascular Aortic Repair for Blunt Traumatic Aortic Injury: A Systematic Review and Meta-Analysis.

Blunt aortic injury is the second most prevalent cause of patient fatalities post-trauma, closely following head injuries as the leading cause. In recent years, thoracic endovascular aortic repair (TEVAR) has evidently improved survival rates and reduced complications in patients suffering from blunt traumatic aortic injury (BTAI) in comparison to open surgery and non-operative management. It is difficult to characterize the appropriate criteria for the timing of TEVAR, whether early or delayed for BTAI, considering the discrepancies related to timing. Electronic databases, including PubMed, S

Prone thoracic endovascular aortic repair via the popliteal artery for inadvertent vascular injury during spondylectomy: a case report.

BACKGROUND: Vascular injury during spinal surgery is a dreaded complication associated with high morbidity and mortality. Repositioning the patient following such an injury could result in significant time delays and haemorrhage. Endovascular repair via popliteal access has never previously been described in the literature. A novel prone thoracic endovascular aortic repair (TEVAR) technique is described here as a safe alternative to manage vascular injury during posterior spinal surgery. CASE DESCRIPTION: Here we describe a 63-year-old male where endovascular repair of vascular injury to the

Proinflammatory and Immunomodulatory Gene and Protein Expression Patterns in Spinal Cord and Spleen Following Acute and Chronic High Thoracic Injury.

INTRODUCTION: In addition to paralysis and loss of sensation, high-level spinal cord injury (SCI) causes sympathetic dysfunction that can lead to autonomic dysreflexia (AD) and chronic immune suppression involving splenic leukopenia. Evidence has shown that treatment with either gabapentin or blockade of TNFα mitigates maladaptive plasticity and the underlying hemodynamic dysfunction, spleen atrophy, and immune dysfunction associated with AD. Because significant improvements long term was noted following treatments only during acute stages of recovery, we sought to systematically examine chang

Road traffic accident-related thoracic trauma: Epidemiology, injury pattern, outcome, and impact on mortality-A multicenter observational study.

BACKGROUND: Thoracic trauma is a major cause of death in trauma patients and road traffic accident (RTA)-related thoracic injuries have different characteristics than those with non-RTA related thoracic traumas, but this have been poorly described. The main objective was to investigate the epidemiology, injury pattern and outcome of patients suffering a significant RTA-related thoracic injury. Secondary objective was to investigate the influence of serious thoracic injuries on mortality, compared to other serious injuries. METHODS: We performed a multicenter observational study including pati…

Blunt thoracic aortic injury resulting in free rupture into the pleural space and cardiac arrest, managed successfully with endovascular stenting.

Blunt thoracic aortic injury (BTAI) is an uncommon yet serious diagnosis in trauma patients, with high on-scene mortality. BTAI typically occurs from rapid deceleration such as in motor vehicle collisions or high-altitude falls shearing the aorta just proximal to the ligamentum arteriosum. We report a case of a man in his 50s falling from a height of 15 m who presented hypotensive with retrosternal chest pain. Mobile chest X-ray showed a widened mediastinum with left-sided haemothorax. CT revealed a contained free aortic rupture just inferior to the origin of the left subclavian artery with bl

A Randomized Controlled Trial of Early versus Late Surgical Decompression for Thoracic and Thoracolumbar Spinal Cord Injury in 73 Patients.

Convincing clinical evidence exists to support early surgical decompression in the setting of cervical spinal cord injury (SCI). However, clinical evidence on the effect of early surgery in patients with thoracic and thoracolumbar (from T1 to L1 [T1-L1]) SCI is lacking and a critical knowledge gap remains. This randomized controlled trial (RCT) sought to evaluate the safety and efficacy of early (<24 h) compared with late (24-72 h) decompressive surgery after T1-L1 SCI. From 2010 to 2018, patients (≥16 years of age) with acute T1-L1 SCI presenting to a single trauma center were randomized to r

Thoracic hyperextension injury with opening wedge distraction fracture in DISH -consideration of surgical strategy based on intraoperative pathological findings.

Diffuse idiopathic skeletal hyperostosis (DISH) is a well-recognized disease characterized by calcifications and ossifications of the entheses mainly in the spine. Patients with DISH are prone to sustaining spinal injuries even after minor trauma because of the long-lever arm mechanism induced by any type of force acting on the rigid yet brittle spine. The number of cases of trauma in DISH-affected spines is predicted to increase during the coming decades because of an increase in DISH-related comorbidities. Generally, posterior fixation with spinal instrumentation spanning three levels above

Do patients with blunt thoracic aortic injury present to hospital with unstable vital signs? A systematic review and meta-analysis.

BACKGROUND: Blunt thoracic aortic injury (BTAI) is an uncommon diagnosis, usually developing as a consequence of high-impact acceleration-deceleration mechanisms. Timely diagnosis may enable early resuscitation and reduction of shear forces, essential to prevent worsening of the injury prior to definitive management. Death is commonly due to haemorrhagic shock, but clinical features may be absent until sudden and massive haemorrhage. OBJECTIVES: The aim of this systematic review was to determine the proportion of patients with BTAI who present with unstable vital signs. METHODS: Manuscripts
