BACKGROUND: Thoracic surgery without general anesthesia can be traced back to the First World War, and thoracic epidural block was used to complete the operation due to a large number of patients with gunshot wounds who needed emergency thoracic surgery. By reducing the intraoperative opioid dose, intraoperative and postoperative opioid-related adverse events such as respiratory depression, nausea and vomiting, delirium, hyperalgesia, and other side effects can be reduced to the benefit of patients.
METHODS: A 72-year-old male patient was admitted to the hospital with a 5-day history of multi

Birt-Hogg-Dubé syndrome (BHDS) is an autosomal dominant inherited disorder caused by mutation in a tumour suppressor gene, FLCN, leading to skin tumours (fibrofolliculomas), renal tumours and pulmonary cysts. Lung involvement is predominantly observed in 70% of the cases of BHDS, manifesting in the form of recurrent primary spontaneous pneumothorax. This video tutorial showcases the surgical management of recurrent right primary spontaneous pneumothorax in a young adult with a history of multiple episodes of bilateral pneumothorax managed by surgical intervention previously. The study undersco

Current guidelines recommend VATS as the first-line treatment for stage II empyema. Patients with concomitant cardiorespiratory failure are considered ineligible for guideline-recommended management. Alternative treatment options are needed. This case report pertains to an obese man in his early 60 s with respiratory failure and decompensated heart failure. He developed an emphyema, which was refractory to the standard antibiotic therapy, thoracocentesis and other chest drainage. As patient was disqualified from surgical treatment, new approach was implemented. Minimally invasive approach of i

BACKGROUND: Resection of giant anterior mediastinal masses (AMMs) has traditionally required median sternotomy as the limited operative space and visibility of conventional video-assisted thoracoscopic surgery (VATS) pose significant technical challenges. In this study, we describe a percutaneous suspension technique using a Tian-Ping sternal lifter, to facilitate uniportal VATS for the complete resection of a giant teratoma. CASE PRESENTATION: A 28-year-old Chinese female presented with a three-month history of intermittent chest pain and cough. Computed tomography (CT) revealed a 17.0 × 9.0

Pleural infection remains a major cause of morbidity and mortality despite advances in antimicrobial therapy, imaging, and surgical techniques. Management is frequently complicated by delayed diagnosis, heterogeneous disease progression, and uncertainty regarding optimal timing of escalation. This article integrates contemporary medical and surgical perspectives on pleural infection, emphasizing early diagnosis, structured reassessment, and timely multidisciplinary decision-making. We discuss the roles and limitations of antibiotics, chest tube drainage, intrapleural enzyme therapy, and surger

RATIONALE: Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital vascular anomaly that often remains undetected in asymptomatic patients. While most cases are hemodynamically insignificant, failure to recognize PAPVC preoperatively may lead to intraoperative misidentification and surgical complications. Coexistence of PAPVC and lung cancer, particularly on the ipsilateral side, is extremely rare and poses challenges for surgical decision-making.
PATIENT CONCERNS: A 55-year-old man, asymptomatic and uncharacteristic in his past medical history, presented with an abnormal s

Bilateral spontaneous pneumothorax is a rare condition that accounts for approximately 1% of all pneumothorax cases. Most cases are attributed to underlying lung disease or prior thoracic surgery, as congenital pleuro-pleural communication is rare. We report the case of a 15-year-old girl with bilateral pneumothorax who underwent video-assisted thoracoscopic surgery. Intraoperative air leak testing unexpectedly revealed congenital pleuro-pleural communication. A retrospective review of preoperative computed tomography suggested a potential bilateral pleural communication between the esophagus

Extramedullary hematopoiesis (EMH) is a rare condition characterized by proliferation of hematopoietic stem cells outside the bone marrow, usually as a compensatory response to hematological disease. Although EMH primarily occurs in the liver and spleen, it can manifest in atypical locations, such as the mediastinum. We herein describe an asymptomatic 66-year-old man with incidentally discovered posterior mediastinal EMH. A 28- × 32-mm mass was detected during a routine examination. Laboratory findings were within normal limits. Computed tomography revealed a well-defined enhancing mass with a

BACKGROUNDS: With increase of patients with a small-sized lung cancer, there is an increasing need for minimally invasive lung segmentectomy that can preserve respiratory function. We perform S(9+)10 segmentectomy with retrograde dissection of the pulmonary vein, bronchus, pulmonary artery, in order, without interlober fissurelectomy and staple dissection of the peripheral lung parenchyma.
METHODS: Seven patients who underwent retrograde S(9+)10 segmentectomy between June, 2021 and May, 2022 in our hospital were retrospectively reviewed.
RESULTS: No patient was converted to the open thoracot

OBJECTIVES: The objective of this single-centre, open, randomized control trial was to compare the patients' satisfaction with local anaesthesia (LA) or general anaesthesia (GA) for video-assisted thoracoscopy.
METHODS: Patients with indication for video-assisted thoracoscopy pleural management, mediastinal biopsies or lung wedge resections were randomized for LA or GA. LA was administered along with no or mild sedation and no airway devices maintaining spontaneous breathing, and GA was administered along with double-lumen tube and one-lung ventilation. The primary end point was anaesthesia-r

BACKGROUND/NEED: Pleural empyemas carry a high morbidity and mortality. Some can be managed with medical treatment but most require some form of surgery with the goals to remove the infected material from the pleural space and to help re-expand the collapsed lung. Keyhole surgery by Video Assisted Thoracoscopy Surgery (VATS) is rapidly becoming a common approach to deal with early stage empyemas to avoid larger, more painful thoracotomies that hinder recovery. However, the ability to achieve those aforementioned goals is often hindered by VATS surgery due to the instruments available.

BACKGROUND: Surgery is crucial in the treatment of the potentially fatal pulmonary hemoptysis condition. Currently, most patients with hemoptysis are treated by traditional open surgery (OS). To illustrate the effectiveness of video-assisted thoracic surgery (VATS) for hemoptysis, we developed a retrospective study of surgical interventions for lung disease with hemoptysis.
METHODS: We collected and then analysed the data, including general information and post-operative outcomes, from 102 patients who underwent surgery for a variety of lung diseases with hemoptysis in our hospital between De

Diaphragm plication has recently become a standard operation for diaphragm eventration. The surgical experience for the patient has improved with the development of minimally invasive operations, but the operator's convenience has not improved significantly. We performed video-assisted thoracoscopic surgery for a diaphragm oblique resection using endostaplers in 10 patients. The total operation time was 39.5 minutes; all patients' symptoms disappeared postoperatively. This presentation will help more thoracic surgeons simplify this operation on the diaphragm.

Since the early 1990s, minimally invasive surgery has been introduced in many surgical fields. The progress of technology and increased interest in minimally invasive surgery has led to innovation from surgery with a large incision to complete thoracoscopic surgery in many facilities. We started thoracoscopic mediastinal surgery in 1997 and robot-assisted mediastinal surgery in 2021. Considering the approach for anterior mediastinal tumor resection, it is necessary to fully understand tumor size, tumor location, and the existence of the tumor invasion to other organs. Especially in the case of

Posterior basal (S10) segmentectomy is one of the most challenging (and uncommon) types of pulmonary segmentectomy. Here, we present two key tips for facilitating a uniportal operation. The first is a full understanding of the relative locations of the pulmonary vessels and bronchi (as revealed by preoperative three-dimensional computed tomography/broncho-angiography), and the other is the use of "suction-guided stapling" to dissect and divide the peripheral pulmonary vessels and bronchi. We describe the successful postoperative course of a patient who was so treated.

INTRODUCTION: Recurrent primary spontaneous pneumothorax (PSP) is often managed with a wedge resection (or blebectomy) and either pleurectomy or pleurodesis. There is a conflicting data regarding which approach is superior to reduce recurrence. Our objective is to evaluate the long-term recurrence rates following pleurectomy versus mechanical pleurodesis for recurrent PSP.
METHODS: The PearlDiver Mariner Patient Claims Database was queried for patients aged 10-25 who were presented with PSP and underwent either pleurectomy or mechanical pleurodesis between 2010 and 2020. The primary outcome w

RATIONALE: Pulmonary sequestration, which can be divided into 2 main types: intralobar pulmonary sequestration (IPS) and extralobar pulmonary sequestration, is an uncommon congenital condition for which surgical resection is usually indicated. Video-assisted thoracoscopic surgery, as compared with open thoracotomy, has increasingly become the preferred operative procedure in the treatment of PS, owing to less postoperative pain and faster recovery. This report describes a rare and challenging case with a giant IPS undergoing video-assisted thoracic lobectomy.
