Outcomes of vitrectomy with or without internal limiting membrane peeling for the treatment of vitreomacular traction.

OBJECTIVE: Vitreomacular traction is a common presentation that can benefit from surgical intervention. The decision to peel the internal limiting membrane (ILM) is made at the discretion of the operating surgeon. We describe a large cohort of eyes that had surgery with or without ILM peeling to give more evidence for the decision-making process. DESIGN: A retrospective cohort study. PARTICIPANTS: Eyes with the diagnosis of vitreomacular traction (VMT) that underwent surgery between January 1, 2016, and January 1, 2025, at a group retina practice in Edmonton, Canada. METHODS: Eyes at a sing…

A 3-step dorsal approach with the extracorporeal ventral traction method for laparoscopic hemihepatectomy.

Major laparoscopic hepatectomy along the major hepatic veins requires accurate orientation to the transection plane and safe management of the hepatic venous branches. We developed an extracorporeal ventral traction (EVT) method combined with a 3-step dorsal approach (3SDA) to standardize laparoscopic hemihepatectomy. In EVT, a traction suture secured on the caudal edge of the transection line is brought out through the epigastrium and pulled extracorporeally, elevating the liver parenchyma ventrally and allowing dorsal access under the caudal laparoscopic view. In 3SDA, the transection plane …

Evolution of Tensionless Repair of Lateral Radial Meniscus Tear With the Use of the Traction Stitch.

BACKGROUND: Meniscus surgery remains one of the most common orthopaedic surgeries performed in the United States each year, with radial tear patterns being a common subset. In young, active patients with mechanical symptoms, surgery is often indicated. While management via partial meniscectomy predominates historically in tears of this type, recent success has been found in surgical repair. As repair techniques continue to evolve, this case demonstrates the use of the traction suture to maintain anatomic reduction throughout repair, ultimately resulting in a low-tension construct. INDICATIONS…

Effects of Positional Traction Integrated With Mobilization With Movement on Cervical Facet Joint Syndrome.

Background Cervical facet joint syndrome (CFJS) is a common source of neck pain and disability, often limiting functional activities. While conventional physiotherapy provides symptomatic relief, it may not always ensure optimal recovery. Positional traction, when combined with mobilization with movement (MWM), is a promising strategy aimed at enhancing cervical mobility and reducing pain. Objectives This study evaluated the effectiveness of combining positional traction with MWM in reducing pain, improving function, and increasing cervical range of motion (ROM) in individuals with CFJS. Metho…

Effects of a lumbar exoskeleton that provides two traction forces on spinal loading and muscles.

In workplaces with prolonged or repetitive stooping, disc herniation due to excessive pressure at the lumbar L5/S1 joints has been a difficult condition to prevent and treat in the field of lower back pain. Previous research on lumbar exoskeletons mainly focused on the impact of assistive torque on muscle activation, overlooking the fact that providing assistive torque is not the optimal approach when bending over with a low load. Instead, using traction force to reduce disc pressure is a more adaptable method to mitigate the risk of intervertebral disc herniation. In this paper, a novel lumba…

Intraoperative challenges and management of fibrovascular membrane with tractional retinoschisis in proliferative diabetic retinopathy.

BACKGROUND: In severe Proliferative Diabetic Retinopathy (PDR), fibrovascular membrane (FVM) causes macular tractional retinal detachment (MTRD) which threatens vision and eventually leads to blindness. Here we present a case of separation between the inner and outer retina in tractional retinoschisis, induced during intraoperative FVM delamination. CASE PRESENTATION: A 68-year-old woman presented with PDR in the right eye, characterized by a combined FVM and retinal detachment, for which a vitrectomy was performed. Multiple holes, large retinal detachment extending to all quadrants, and whit…

Tractional Retinal Detachment Related to Hemoglobin C Trait Retinopathy: A Case Report.

Hemoglobin C (HbC) disease is an uncommon disease that is generally considered benign, causing only occasionally painless hematuria, osteomyelitis, and dental abnormalities. Ocular manifestations have rarely been described in these patients. Here we report a novel ophthalmological manifestation of the disease. A 20-year-old woman presented with progressive visual loss in her right eye due to tractional retinal detachment. The left eye was apparently normal, but wide-field fluorescence angiography showed mild peripheral ischemia with multiple vascular abnormalities. Vitrectomy was performed and…

Mesenteric traction syndrome - Incidence, impact, and treatment: A systematic scoping review of the literature.

BACKGROUND: Mesenteric traction syndrome (MTS) is commonly seen during major abdominal surgery and is characterised by facial flushing, hypotension, and tachycardia 15 min into surgery. MTS also impacts the postoperative course, as severe MTS has been associated with increased postoperative morbidity. However, despite MTS being common and severe MTS causing increased postoperative morbidity, the gaps in the literature are not clearly defined. We aimed to examine the diagnostic criteria, incidence, intraoperative and postoperative impact, and potential preventative measures of MTS while highlig…

The novel traction-suturing technique in correction of caudal end deviation of the nasal septum.

BACKGROUND: Caudal end correction is one of the main challenges during septoplasty that needs extra steps and skills. This complex area affects the shape of the nose. Moreover, the caudal end represents the medial boundary of the internal nasal valve. Thus, any deviation may disturb this critical area causing nasal obstruction. This study aimed to evaluate the novel traction-suture technique to correct septal caudal end deviation and its postoperative impact. STUDY DESIGN: This prospective case-series study. SETTINGS: It was held in a tertiary referral university institute between January 20…

A novel technique for Endobronchial Watanabe Spigot placement for bronchopleural fistula: a traction method.

Empyema with bronchopleural fistula is a challenging condition that often requires open-window thoracostomy. Bronchial occlusion with Endobronchial Watanabe Spigot is an effective and less invasive procedure to close the fistula; however, delivering a spigot into the target bronchus requires high technical skills. Herein, we report a case of bronchopleural fistula occluded with a spigot using a new 'traction method' in which the spigot is tied as an anchor to a guidewire and pulled into the target bronchus. Our method allows selective and steady placement of the spigot for patients who undergo…

Use of an endoscopic flexible grasper as a traction tool for excision of polyps: preclinical trial.

Endoscopic submucosal dissection (ESD) is challenging in the right colon. Traction devices can make it technically easier. In this study, we evaluated a flexible grasper with articulating tip and elbow-like bending (IgE) through a double-balloon surgical platform (DESP), compared with an earlier generation grasper without elbow-like bending (Ig). The reach of Ig/IgE was investigated at eight locations using a synthetic colon within a 3D model. Using a fresh porcine colorectum, 4 cm pseudo-polyps were created at the posterior wall of the ascending colon. Fifty-four ESD procedures were performed…

A modern review of penile traction monotherapy and combination therapy for the treatment of peyronie's disease.

Peyronie's disease (PD)-related penile deformity is managed with multiple treatment modalities including oral medications, intralesional injections, and surgery. Penile traction therapy (PTT) is one such modality with purported benefits, albeit with notable differences in the characteristics of available traction devices and published study protocols. We provide a comprehensive review of the available data supporting PTT for PD treatment. We performed a rigorous database search to identify all studies pertaining to PTT for the treatment of PD through November 2019. Seventeen trials explored us…
