Guidelines for high-flow nasal cannula oxygen therapy in neonates (2022).

High-flow nasal cannula (HFNC) oxygen therapy, which is important in noninvasive respiratory support, is increasingly being used in critically ill neonates with respiratory failure because it is comfortable, easy to setup, and has a low incidence of nasal trauma. The advantages, indications, and risks of HFNC have been the focus of research in recent years, resulting in the development of the application. Based on current evidence, we developed guidelines for HFNC in neonates using the Grading of Recommendations Assessment, Development and Evaluation (GRADE). The guidelines were formulated aft

Deep vein thrombosis in pregnancy.

This article provides a review of the incidence, pathophysiology, and treatment of deep vein thrombosis (DVT) in pregnancy, a rare but serious complication of pregnancy. The incidence of DVT in pregnancy varies widely, but it is a leading cause of maternal morbidity in both the United States and the United Kingdom. Risk factors during pregnancy include prolonged bed rest or immobility, pelvic or leg trauma, and obesity. Additional risk factors are preeclampsia, Cesarean section, instrument-assisted delivery, hemorrhage, multiparity, varicose veins, a previous history of a thromboembolic event,

Ventilatory support of the trauma patient with pulmonary contusion.

Pulmonary contusion is a common clinical condition seen in patients with thoracic trauma. The mortality rate from isolated pulmonary contusion is low, but when combined with other severe injuries patients with pulmonary contusion can have mortality rates as high as 10% to 57%. Early aggressive mechanical ventilatory support can prevent progressive atelectasis and worsening of arterial oxygenation. Careful titration of ventilatory support to physiologic end points is a reasonable approach. Unfortunately, data comparing the means of achieving the end points are lacking and no published prospecti

Medical care at the 1982 US Festival.

The 1982 US Festival, a three-day outdoor rock music festival, attracted close to 410,000 people. Through a private contractor, a group of physicians participated in the design, administration, and implementation of medical care for this event. Basic ambulatory care was provided by a multitiered approach, using emergency medical technicians, registered nurses, medical health counselors, and physicians. A total of 2,623 encounters required examination and evaluation by health professionals. Thirty-eight percent (1,014 cases) involved minor surgical trauma. Extreme environmental conditions, incl

Clinicians' Experiences in Responding to Mental Health Crisis Among People Experiencing Homelessness in Ireland.

INTRODUCTION: Homelessness is rising in Ireland, and people experiencing homelessness face disproportionately poor mental health outcomes due to intersecting structural and psychosocial factors. Access to mental healthcare remains limited, leaving generalist services to manage increasing need, particularly during crises. AIM: To explore healthcare professionals' experiences of managing mental health crisis episodes among people experiencing homelessness. METHOD: A descriptive qualitative study informed by critical realism. We used semi-structured interviews with 14 healthcare professionals,

Experience of Health Professionals in Caring for Patients With Chest Trauma: A Qualitative Study.

BACKGROUND: The variation in the implementation of chest trauma (CT) management's guidelines led to inconsistent and mixed experiences toward CT management by the health care team. Moreover, there is a scarcity of studies exploring factors that enhance CT management experiences worldwide and in Jordan. OBJECTIVES: The aims of this study were (1) to explore emergency health professionals' attitudes and experiences toward CT management and (2) to understand factors that affect the care for patients with CTs among emergency health professionals. METHODS: A qualitative exploratory approach was a

Chinese expert consensus on prevention, diagnosis, and management of venous thromboembolism in adult burn patients (2024).

Venous thromboembolism (VTE) management in adult burn patients has become a crucial focus in China. The intricate nature of VTE necessitates specialized anticoagulation strategies due to the unique challenges posed by burn injuries. To address this pressing issue, the Burn and Trauma Branch of the Chinese Geriatric Medical Association and Critical Care Group of Burn Surgery Branch of the Chinese Medical Association organized a panel of domestic experts in burn surgery, critical care medicine, vascular surgery, nursing, and health statistics and methodology from Chinese hospitals to discuss VTE

Post-traumatic eye observations.

Retrobulbar haemorrhage after facial trauma or surgery is an uncommon but well-documented complication. The assessment and management of this condition is variable. We asked 288 maxillofacial surgeons in the UK about the signs and symptoms, incidence, and management of retrobulbar haemorrhage. We also enquired about the regimen that they used for eye observations, the patients who, in their opinion, required observation, and the method of communication of instructions to the nursing staff. A total of 185 responded (64%). Each surgeon had seen a mean of 1.3 cases of retrobulbar haemorrhage, of

Femoral shaft fracture treated with Hoffmann external skeletal fixator in young teens.

External skeletal fixation has been proved to be an effective method in treating complex femoral shaft fracture in adult patients with multiple trauma, extensive soft tissue injury or vascular injury which need reconstructive procedures. This method has the advantages of less extensive surgical exposure, easy wound access, and early ambulation. Twenty-two cases of femoral shaft fracture, between the ages of 10 and 15, were treated with the Hoffmann external skeletal fixator in Chang Gung Memorial Hospital from 1981 to 1985. The average time for union and removal of fixator was 72 +/- 8 days (f

External fixation in the management of severe musculoskeletal trauma.

External fixation was employed in 33 open extremity fractures in 29 patients. These high energy injuries were characterized by massive soft tissue injury, contamination, instability, comminution, and/or segmental bone loss. Initial management consisted of irrigation and debridement, and application of the external fixator. In an attempt to obtain limb salvage, 92 additional procedures were performed to attain soft tissue coverage and insure bony stability. Six patients required subsequent below knee amputation at the level of the fracture site. All other cases achieved soft tissue coverage, bo

Characteristics of undertriaged older patients in the emergency department: Retrospective study.

BACKGROUND: Older patients are more likely to be undertriaged as they often suffer from multiple diseases and complain of non-specific symptoms. Therefore, it is necessary to identify the characteristics of undertriaged older patients in emergency departments. METHODS: This descriptive study retrospectively reviewed and analyzed the electronic medical records of older patients who visited the emergency department of a general hospital in Seoul between January and December 2019. RESULTS: Approximately 29 % (n = 4,823) of older patients who visited the emergency department during the study per

Quantitative Ergonomic Comparison of Traditional Versus Endoscopic-Assisted Tonsillectomies: A Prospective Case-Control Study.

INTRODUCTON: Low-trauma, osteoporotic fractures among older men are associated with a significant increase in morbidity and mortality. Despite effective therapies for osteoporosis, several studies have demonstrated that management and treatment after a low trauma fracture remains inadequate, especially among men. Fracture liaison services have been shown to significantly improve osteoporosis evaluation and treatment. However, such programs may be less feasible and accessible in rural areas, with limited availability of specialty services. The study objective was to evaluate a centralized, elec

When demography challenges policy: the rising burden of proximal femur fractures as a role model and driver for geriatric trauma care in Germany.

PURPOSE: Germany's hospital reforms will substantially reshape service delivery and capacity planning. Proximal femur fractures (PFFs), the most frequent geriatric fractures, are highly sensitive to demographic aging and therefore serve as a model condition to assess future healthcare system requirements. METHODS: Nationwide inpatient data from the German Federal Statistical Office (2000-2023) were analyzed for proximal femur fractures (ICD-10-GM S72.0-S72.2). Demographic projections up to 2050 were combined with Poisson regression, exponential smoothing, and autoregressive integrated moving

Trauma transitional care coordination: A mature system at work.

BACKGROUND: We have previously demonstrated effectiveness of a Trauma Transitional Care Coordination (TTCC) Program in reducing 30-day readmission rates for trauma patients most at risk. With program maturation, we achieved improved readmission rates for specific patient populations. METHODS: TTCC is a nursing driven program that supports patients at high risk for 30-day readmission. The TTCC interventions include calls to patients within 72 hours of discharge, complete medication reconciliation, coordination of medical appointments, and individualized problem solving. "We Are Saving Their Bodies and Destroying Their Souls.": Family Caregivers' Experiences of Formal Care Setting Visitation Restrictions during the COVID-19 Pandemic.

This study aims to explore the experiences of family caregivers during the COVID-19 pandemic-imposed visitation restrictions at formal care settings (FCS) such as assisted living centers and traditional nursing homes. Participants (N = 512) were recruited from an international caregiving social media site that was developed at the beginning of the COVID-19 pandemic. Descriptive data was collected on the family caregivers, the care recipient and facility. Respondents also provided a single feeling word describing their experience and an open-ended question allowed for further exploration. Careg

Pressure ulcers in maturing skin - A clinical perspective.

While the problem of Pressure Ulcers (PU) in adults has received a great deal of attention, far less is known about PUs in neonates and children. The overall health status of children is generally better and multi-morbidity is limited to a small percentage of patients, like very low term neonates (born before 32 weeks of gestation age), newborns with congenital abnormalities, genetic disorders, perinatal distress syndrome or children with a limited immunity. Survival rates of both critically and chronically ill neonates, infants and children have improved dramatically in recent years, introduc
