[Increased mortality and disability in a cohort of Mexican children with maple syrup urine disease].

INTRODUCTION: Maple syrup urine disease (MSUD) is a genetic disorder that produces ketoacidosis crises and neurological complications often leading to death. The age of diagnosis and treatment determine a child's adequate and healthy outcome. OBJECTIVE: Describe the characteristics of a pediatric Mexican cohort with MSUD. MATERIAL AND METHODS: Retrospective analysis of MSUD cases seen at our Metabolic Unit between 1991- 2006. RESULTS: We studied 36 patients; three were initially detected through neonatal screening, one of them done in Mexico and two in the United States. The latter were giv

A rational approach to employing high plasma levels of antipsychotics for violence associated with schizophrenia: case vignettes.

Forensic psychiatric settings contain a high prevalence of treatment-resistant violent schizophrenia patients. Clozapine therapy has the most robust data for the management of violence in patients with schizophrenia, but for those who cannot tolerate or refuse clozapine, high-dose antipsychotic treatment to high achieve high plasma levels remains a viable option despite limited evidence for efficacy in controlled trials. This article enumerates rational guidelines for employing high plasma level strategies, emphasizing the appropriate interpretation of, and reaction to high plasma antipsychoti

A case of neonatal gastric perforation misdiagnosed as neonatal necrotizing enterocolitis: A literature review.

RATIONALE: Neonatal gastric perforation (NGP) is a rare but life-threatening neonatal acute abdominal condition. Its clinical and imaging manifestations may overlap with those of necrotizing enterocolitis (NEC), making accurate preoperative localization of the perforation site challenging. This case highlights the diagnostic challenges and key points in the differential diagnosis of NGP when its initial presentation resembles NEC. PATIENT CONCERNS: A male infant born at 33 weeks and 5 days of gestation, with a birth weight of 2500 g, was admitted on the fourth day of life because of abdominal

[A collaborative study on children with mature B-cell non-Hodgkin's lymphoma in China].

OBJECTIVE: This study aimed to evaluate the efficacy of the CCCG-B-NHL 2010 protocol in children with mature B-cell non-Hodgkin's lymphoma (B-NHL) in China retrospectively. METHOD: Eight tertiary referral centers for childhood cancer participated in this study. From April 2009 to March 2012, 104 patients below 18 years with newly diagnosed, untreated B-NHL or mature B cell acute lymphoblastic leukemia (B-ALL) were enrolled. Six patients refused further staging work-up and treatment due to the expense were excluded. Diagnostic slides were not centrally reviewed in this retrospective study. Twe

Care at Burn Centers Is Associated With Reduced Discharge Against Medical Advice: A National, Multi-Database Analysis.

Discharge against medical advice (AMA) leads to worse outcomes in patients with burn injuries, including higher readmission rates, wound complications, and elevated costs, although protective and risk factors remain incompletely characterized. Burn encounters were identified in 2 national databases, the Nationwide Inpatient Sample (NIS), 2016-2021, and the Burn Care Quality Platform (BCQP), 2016-2022. Encounters were stratified by AMA discharge. The primary outcomes were the overall incidence of AMA discharge and the effects of inpatient psychosocial or behavioral interventions and burn center

Arguments against a "general and permanent" ban on pediatric intersex surgery: A response to Clune-Taylor.

The paper offers a critical response to the proposed "dis/analogy" between the restriction of Jehovah's Witness parental right to refuse life-saving blood transfusions for their minor children and a "general" and "permanent" ban on "unnecessary" pediatric intersex surgery. The main argument of the analogy is "securing the patient's future autonomy." Feinberg's theory of rights is used to demonstrate that the proposed analogy is untenable. A new category of developmental rights-in-trust is introduced to address specific needs of gender development in DSD situations. Both premises are disputed.

Paraneoplastic cerebellar degeneration secondary to ovarian carcinosarcoma: a cerebellar conundrum.

We present a case of an elderly female patient who presented with a 6-month history of progressive slurred speech, vertigo, unsteadiness and falls. She underwent an extensive battery of neurological and cardiovascular investigations, none of which demonstrated a diagnostic cause for her symptoms. She was referred to the stroke and neurology teams and was started on treatment for presumed anxiety. As her symptoms continued to progress, she was referred to the falls service. Following a multidisciplinary team discussion, she was reviewed by the consultant geriatrician who felt this may be due to

The struggle for schizophrenia treatment: A case study.

Individual of legal age with schizophrenia presenting anosognosia was abandoned, as a result of a court decision. Close family members were not allowed to provide medical follow-up, treatment, protection regarding his vulnerability, and preserve the dignity of their loved one. The issue was the court's prioritization of the autonomy of the individual over his mental health status. The purpose of this case study was to identify the pitfalls of a court case seeking medical follow-up and treatment for a family member with schizophrenia and anosognosia. The method was qualitative and the design wa

Vitamin K deficiency bleeding: a case study.

Vitamin K deficiency bleeding (VKDB), formerly known as hemorrhagic disease of the newborn (HDN), is a bleeding disorder in neonates that is caused by inadequate serum levels of vitamin K. Vitamin K is a nutrient essential for adequate function of the coagulation cascade. Certain internal and external factors place newborn infants at higher risk for VKDB. Therefore, vitamin K prophylaxis has become the standard of care for newborns. Although the American Academy of Pediatrics recommends the administration of vitamin K to newborns, some parents are choosing to withhold vitamin K administration

A quadricuspid aortic valve causing moderate aortic regurgitation.

Quadricuspid aortic valve (QAV) is a rare cause of aortic regurgitation. Most cases are detected incidentally during echocardiography, angiography, autopsy or surgery. It may also be associated with other congenital anomalies of the heart. A 70 year-old man was admitted to our hospital with a five-month history of dyspnea. Echocardiographic examination showed a QAV leading to moderate aortic regurgitation, severe mitral regurgitation, left ventricular dysfunction and aortic root dilatation. Surgical treatment was considered, but the patient refused. We describe a case of QAV leading to aortic

Habitual anterior dislocation of the hip in cerebral palsy: a case report.

Habitual anterior dislocation of the hip joint is extremely rare. We believe this is the first case reported of a habitual anterior dislocation of the hip occurring in a child with cerebral palsy. The dislocation was painless, voluntary and used to occur every time the child would extend and externally rotate the limb. Operative intervention in the form of a varus derotation femoral osteotomy and anterior capsulorraphy was planned, but the caregivers of the patient were unwilling for the same. Hence, the patient was given an adduction and internal rotation brace and was counselled not to perfo

Kaposi sarcoma and paracoccidioidomycosis in the same fragment of oral mucosa biopsy: a rare association in human immunodeficiency virus-positive patient. A case report.

The immunossuppression caused by HIV infection makes the affected individuals more susceptible to some diseases including infections, neoplasms, or even the association between them. Kaposi sarcoma (KS) is the most common AIDS-related neoplasm, featured as an angioproliferative disorder. Its cause seems to be related to the human herpesvirus type 8 and it is usually associated with lower CD4+ T cell count. Oral involvement is frequent, presenting red to blue-purplish plaques, maculaes, and nodules. On the other hand, paracoccidioidomycosis (PCM) is a systemic mycosis, endemic in Latin America,

Massive obstetric hemorrhage in a Jehovah's Witness: intraoperative strategies and high-dose erythropoietin use.

A massive obstetric hemorrhage resulting in an extremely low hematocrit (5.9%) required innovative intraoperative and postoperative management. We used a combination of rapid intraoperative volume replacement, aortic compression, chemical coma, high-dose erythropoietin, and enteral iron replacement to elevate the hematocrit in a patient refusing blood transfusions.

Fatal outcome in bronchus-associated lymphoid tissue lymphoma.

Primary pulmonary lymphoma is rare. The most common histological type is the bronchus-associated lymphoid tissue lymphoma. This type of lymphoma has an indolent course and excellent response to therapy. One-third of all cases are diagnosed incidentally. However, due to the rarity of this disease, little is known about its natural history in terms of dissemination and evolution. Herein, we report the unusual case of a 61-year-old man who refused treatment after being diagnosed with bronchus-associated lymphoid tissue lymphoma and died 2 years later from massive lung infiltration without dissemi

Vaccine safety controversies and the future of vaccination programs.

In the years following the hepatitis B vaccination/multiple sclerosis controversy, a number of new issues regarding vaccine safety have been raised, in some cases leading to more debate and confusion. Against this background, an international group of experts was convened to review the current points of view concerning the use of thimerosal as a preservative and its potential risks; the suggested link between thimerosal-containing vaccines and acute lymphoblastic leukemia; the alleged association between aluminum-containing vaccines/macrophagic myofasciitis and general systemic complaints; a p

Public fear of vaccination: separating fact from fiction.

During the last two centuries, the world has seen a substantial increase in the number and availability of vaccines for the prevention of infectious disease. Smallpox vaccine remains the most celebrated vaccine-related achievement in human history, but worldwide reductions in many other diseases including measles, mumps, rubella, polio, diphtheria, and whooping cough (Bordetella pertussis) also illustrate the power of vaccination in controlling outbreaks of contagious diseases. Ironically, as advances in vaccination successfully limit disease outbreaks, the impact that these infectious agents

A case illustration of resistance from a cognitive perspective.

Brian, a 36-year-old, single, white male, entered cognitive therapy in response to a depressive episode precipitated by the loss of a job. In addition to his Major Depression, the client met diagnostic criteria for Dysthymia and Personality Disorder Not Otherwise Specified. The first three sessions focused on Brian's unemployment crisis and related dysphoria, as well as his passive-avoidant approach to life. Brian collaborated with the therapist in formulating a treatment plan and quickly found a new job, whereupon he abruptly withdrew from therapy. Shortly thereafter, Brian contacted the ther
