Multiple oral ulcers leading to diagnosis of pulmonary tuberculosis.

We report a 47 year old man who presented with painful non-healing tongue ulcers of 3 months duration. Examination revealed an additional buccal ulcer that he was unaware of. Histopathology of the ulcers showed caseation necrosis. Following this report, chest radiography and sputum microscopy performed revealed pulmonary tuberculosis. From this case-study, one should be aware of coexisting pulmonary tuberculosis in patients with chronic non-healing oral ulcers, both for diagnosis as well as prevention of transmission through respiratory droplets.

Extra-pulmonary tuberculosis developing at sites of previous trauma.

We describe five individuals in whom extra-pulmonary tuberculosis appeared to localise at a site of previous blunt injury. We review other similar case reports where preceding trauma was blunt and non-penetrating, and discuss a possible mechanism involving transport of mycobacteria in monocytes to sites of injury during "latent" tuberculosis infection. This challenges the conventional model proposed for mycobacteria dissemination in tuberculosis disease.

Rasmussen's aneurysm with aspergilloma in old, healed pulmonary tuberculosis.

Rasmussen's aneurysm is rare sequelae of pulmonary tuberculosis. The hemoptysis caused by rupturing of it is usually massive and needs early and aggressive management. To our knowledge, we are the first to report a case of Rasmussen's aneurysm with aspergilloma proved pathologically. The accomplishment of aspergilloma changes the first choice of treatment from endovascular embolisation to surgery.

[Optimisation of diagnostics and differential diagnostics disseminated pulmonary tuberculosis].

One of the reasons of dramatic situation with tuberculosis in Russia is untimely diagnostics of tuberculosis. The aim of the study was to identify the causes of diagnostic mistakes when we deal with disseminated pulmonary tuberculosis at current stage and to modernize the diagnostic process. The analysis of the diagnostic activity of the consultative diagnostic center of Central Tuberculosis Research Institute of Russian Academy Medical Sciences for 2011 was performed with special attention on the results of the survey of 505 patients with pulmonary dissemination. The frequency of discrepancie

Detection of Pulmonary tuberculosis: comparing MR imaging with HRCT.

BACKGROUND: Computer Tomography (CT) is considered the gold standard for assessing the morphological changes of lung parenchyma. Although novel CT techniques have substantially decreased the radiation dose, radiation exposure is still high. Magnetic Resonance Imaging (MRI) has been established as a radiation- free alternative to CT for several lung diseases, but its role in infectious diseases still needs to be explored further. Therefore, the purpose of our study was to compare MRI with high resolution CT (HRCT) for assessing pulmonary tuberculosis. METHODS: 50 patients with culture-proven p

Neuromyelitis optica and pulmonary tuberculosis: a case-control study.

SETTING: Neuromyelitis optica (NMO), or Devic's disease, is a rare acute inflammatory disease characterised by demyelination affecting the spinal cord and optic nerves. Although NMO usually occurs in isolation, the association between NMO and active pulmonary tuberculosis (TB) has been suggested by a number of case reports and series. OBJECTIVE: To investigate the possible association between NMO and TB. DESIGN: A retrospective case-control study. We included adult patients admitted to our institution with a nal diagnosis of NMO based on accepted criteria between January 1995 and February 20

Nontuberculous mycobacteria isolated during the treatment of pulmonary tuberculosis.

BACKGROUND: The significance of nontuberculous mycobacteria (NTM) isolated from a patient during therapy for pulmonary tuberculosis (TB) is uncertain. We investigated the frequency and clinical significance of NTM isolated from patients receiving anti-TB treatment. METHODS: We conducted a retrospective cohort study of all patients with culture-confirmed pulmonary TB, and identified patients with respiratory cultures positive for NTM during therapy for pulmonary TB. RESULTS: From January 2003 to December 2005, 958 patients were diagnosed with culture-confirmed pulmonary TB. NTM were isolated

[Cellular immunity characteristics in drug abusers with pulmonary tuberculosis].

OBJECTIVE: To explore the characteristics of cellular immunity in drug abusers with pulmonary tuberculosis. METHODS: Sixty drug abusers with pulmonary tuberculosis and 60 non-drug abusers with pulmonary tuberculosis (control) were enrolled in this study. Three days after establishment of a definite diagnosis, peripheral blood was taken from the patients for lymphocyte subgroup (CD3(+), CD3(+)/CD4(+), CD3(+)/CD8(+) T lymphocyte subgroups and NK cells) examination by flow cytometry, and the CD4(+)/CD8+(+) ratio was calculated. The difference of cellular immunity between the drug abusers and con

Case report: extra pulmonary tuberculosis in sickle cell disease.

In apparently healthy individuals, tuberculosis (TB) affects mainly the lungs however, worsening immune status tend to predispose to an increased tendency for extra-pulmonary tuberculosis (EPTB). We report the case of a 22 year old known sickle cell anaemia (HBS) female student with three-month history of bilateral hip pain, weight loss and swelling of the left hip with multiple discharging sinuses, paraplegia and recurrent fever. There was no preceding history of trauma. Full Blood Count (FBC) revealed leukocytosis with neutrophilia, monocytosis, thrombocytosis and Packed Cell Volume (PCV) of

Multifocal tuberculosis of the chest wall without pulmonary involvement.

Skeletal tuberculosis (TB) is usually seen in association with a primary pulmonary focus. However, it is being increasingly recognized that the former may be encountered without the latter, particularly in children. Sternal lesions have been frequently reported in infants, presumably secondary to Bacillus Calmette-Guerin (BCG) vaccination. Herein we report a case of rib TB and a cold abscess in a child who had previously had sternal TB ; that diagnosis had been missed at the time. Timely treatment of apparently solitary skeletal lesions may reduce the number of multifocal cases of skeletal TB

Rifabutin-associated panuveitis with retinal vasculitis in pulmonary tuberculosis.

INTRODUCTION: Rifabutin-associated uveitis has been reported frequently in AIDS patients and more rarely in immunocompetent patients. It is characterized clinically by anterior acute uveitis. Only a few poorly documented cases of rifabutin-induced panuveitis with retinal vasculitis have been reported. Here, we report four cases of rifabutin-associated panuveitis with retinal vasculitis. CASE REPORTS: We describe four patients with active tuberculosis, treated with a multidrug regimen including rifabutin for at least 1.5 months before presentation. The first patient was immunocompetent, the th

Intracranial tuberculomas developing while on therapy for pulmonary tuberculosis.

We describe the case of a south Asian man who developed symptomatic intracranial tuberculomas while on therapy for pulmonary tuberculosis. The development or progression of intracranial tuberculomas during the course of appropriate antituberculous therapy has been recognised previously. We review the epidemiology, pathophysiology, diagnosis, and treatment of this paradoxical phenomenon. Although the aetiology of this reaction is unknown, it may be related to an enhanced immune response during the course of therapy. Routine brain imaging for all patients diagnosed with tuberculosis is not recom

Patient mortality of active pulmonary tuberculosis requiring mechanical ventilation.

Mortality remains high among patients with pulmonary tuberculosis requiring mechanical ventilation (TBMV). This study was carried out to establish the mortality rates of TBMV and to identify factors that contribute to in-hospital mortality. From January 1996-April 2001, there were 825 patients with active pulmonary tuberculosis at the National Taiwan University Hospital, Taipei, Taiwan. Of these, 41 suffered acute respiratory failure and required mechanical ventilation in the intensive care unit (ICU). Of these 41 patients, 38 were followed up for 180 days. In-hospital deaths were documented i
