Sucralfate attains a healing rate of about 79% for duodenal ulcer in 4 weeks, which is similar to the effects of cimetidine and ranitidine. A 26-yr-old male of Ecuadorian descent developed epigastric pain and bleeding per rectum, necessitating hospitalization. Upper endoscopy revealed an irregularly shaped gastric ulcer which was biopsied. Because of persistent bleeding and hemodynamic compromise, the patient underwent an exploratory laparotomy. A case of a 49-year-old woman affected by wide ulcerations on the face which have been caused by the patient herself, as she has admitted, is reported. The lesions were strictly limited to the area of innervation of the first and second branch of the trigeminal nerve. The patient presented parasthesias and analgesia in the trigeminal area as the consequence of a neurosurgical operation. She also suffered from important psychosis. All these elements enable us to diagnose "trigeminal trophic ulcers". The pathogenesis of this syndrome is discussed.

A 59-year-old Chinese man presented with a three month history of penile ulcers and abdominal pain. Syphilis and a gastric malignancy were diagnosed clinically. Biopsy of the genital and stomach ulcers revealed histiocytic granulomata containing numerous intracellular fungal bodies. Histoplasma capsulatum was subsequently cultured from the penile ulcer. Antifungal therapy was instituted. However the patient's general condition deteriorated rapidly and he died within two weeks of admission. A post-mortem liver biopsy showed numerous intracellular fungi.

A 14-year-old black male with sickle cell anemia developed a duodenal ulcer that masqueraded as sickle cell-related abdominal pain crisis on multiple occasions. Malingering and poor therapeutic compliance aggravated the ulcer in this patient, who ultimately succumbed to a catastrophic bleed. Between 1979 and 1989, 92 patients were treated at St George Hospital, Kogarah, for perforated duodenal or prepyloric ulcer. Mortality rate at 28 days was 18%. This is due to the fact that the differential diagnosis between gastric ulcer and ulcerative carcinoma is the most important goal of biopsy. The patient suffering from gastric ulcer disease needs more attention than one suffering from duodenal ulcer disease, especially regarding the necessity of endoscopic bioptic control of healing, which is vital to ensure that malignancy is not missed. Greater patience is also necessary during therapy, since a gastric ulcer is generally larger than a duodenal ulcer and more time is thus necessary for complete healing. Otherwise the natural course of the disease and the therapeutic requirements for acute and maintenance treatment are similar. This also relates to the indication for surgery.

A group of 23 non-ulcer dyspepsia patients were compared with controls drawn from relatives of psychiatric outpatients. The level of hostility in both groups was high, but not significantly different. There was also no significant difference between the 2 groups on measures of extroversion, neuroticism, psychoticism and lie scores, but the ulcer group was significantly more depressed and more were diagnosed as suffering from a neurotic depression and generalized anxiety disorder.

The seasonal periodicity of perforated peptic ulcer of the stomach was analysed among 296 patients operated at seven departments of gastrointestinal surgery in Denmark between 1975 and 1984. Admissions were significantly higher in August and September with a tendency, although not significant, in March also. This pattern was unaffected by age and sex.

Following an injury to the right eye, a corneal ulcer with hypopyon developed in a leprosy patient. Direct examination of the corneal scrapings on three occasions showed septate, branched, dematiaceous hyphal elements. When scrapings were cultured on Sabouraud's glucose and brain heart infusion agars. Bipolaris hawaiiensis was isolated repeatedly. The patient responded successfully to treatment with nystatin ointment, although the central opacity of the cornea remained and visual acuity did not improve.
