Modified Burch colposuspension was performed upon 186 women having clinically and urodynamically verified stress urinary incontinence (SUI). After two years, 91 percent of the patients were cured or markedly improved. The success rate was as much as 95 percent among the patients who underwent Burch colposuspension as a first operative treatment for incontinence. The relationship between age and urodynamic parameters was studied cross-sectionally in a group of 28 women in whom clinically and urodynamically both stress incontinence and detrusor instability were excluded (no proven incontinence, NPI) and in a group, matched for age, with genuine stress incontinence (GSI). We assessed the prevalence of urinary incontinence and contributory factors such as senile dementia, impaired mobility, medication, associated disorders and urinary tract infection in 98 elderly residents of a nursing home. Pre- and postoperative clinical, urodynamic and morphological data from 91 cured and uncured patients with Burch colposuspension and from 54 patients with vaginal repair, were analysed. The results were expected to show possible reasons for the failure of surgery and to demonstrate prognostic criteria for successful surgery. An AMS 800 sphincteral prosthesis was implanted in 20 patients (18 men and 2 women) with total urinary incontinence, with results that were superior to those of the previously used procedures: success in 18 cases (90%), failure in 2 cases (1 incontinence, 1 explantation). A change of material was necessary in 3 patients. The AMS 800 prosthesis constitutes a major advance in the treatment of iatrogenic (post-prostatectomy) incontinence in man, of recurrent stress incontinence in women and of some cases of neurogenic or malformative urinary incontinence.

A modification of Kaufman's urinary incontinence device was implanted unsuccessfully in 5 patients. This device achieved passive urinary continence in each of these patients; however, all patients continued to experience moderate to severe stress incontinence.
