Six myxoid leiomyosarcomas of the uterus are described. Four were located within the myometrium and two lay predominantly within the broad ligament but appeared to have arisen in the outer myometrium. These tumors were characterized grossly by a gelatinous appearance and an apparently circumscribed border. The activity of oestrogen-induced peroxidase in sections along the uterus of normal and mammary tumour-bearing adult rats was measured. Oestradiol increased the activity of this enzyme in the cervix as well as in other parts of the uterus in ovariectomized or immature rats. The first case of primary Wilms' tumor of the uterus is described. It occurred in a 14-year-old girl. Microscopically, embryonal tubules, glomeruli, and blastema were identified within a myxomatous stroma. No teratomatous elements were found. The tumor arose in the uterus, permeated the endocervical canal, filled the vagina, and infiltrated the Douglas cul-de-sac and the left broad ligament through the posterior vaginal fornix. Comments are made on the histogenesis and histologic pattern of the tumor.

A placenta percreta must be expected in 7% of all cases of abnormal implantation of the placenta. Placenta percreta is therefore a rare but serious complication of pregnancy. In 15% of the cases spontaneous rupture of the uterus occurs. Depending on the type of delivery, the maternal mortality ranges from 6-30%. Early hysterectomy is the method of choice to prevent serious complications of hermorrhage. Since the degree of infiltration of the myometrium with placenta can not be determined with accuracy, excision of the site of perforation to preserve the uterus does not appear to be indicated. …

The clinicopathologic features of 4 cases of Müllerian adenosarcoma of the uterine body and 1 of the cervix are reported. They showed the characteristic pattern of benign epithelial growth within sarcomatous stroma. In the cervical tumor, malignant heterologous stromal cells were also present. All the patients are alive and well, confirming the good prognosis of this neoplasm. The authors think that the presence of a malignant heterologous component probably does not imply a worse prognosis.

Eighteen cases of acute and subacute puerperal inversion were studied to identify important predisposing factors and to assess management and postpartum morbidity. The study patients did not differ from case-matched controls in age, parity, duration of labor, type of delivery, or management of the third stage. The most common signs noted were hemorrhage (94%) and shock (39%). All inversions were recognized immediately and manually replaced within 60 minutes. Two thousand seven hundred ninety two contraction stress tests (CST) were analysed. Oxytocin was administered in 1730 of these to stimulate uterine contractions and the hyperstimulation which occurred in 48 tests (2.8%) was studied extensively. Seven patients went into labor within 24 hours of the hyperstimulation. Two infants weighed less than 2500 g. A 22 year old primigravida at 40+ weeks gestation had an i.v. induction of labour with 5-10 mU/min oxytocin for 3 h which resulted in fetal distress without bleeding at 5 cm dilatation at station -3, vertex transverse. At emergency Caesarean section an occult rupture of the uterus was found. Maternal height 168 cm, weight 71.0 kg. Fetal length 53 cm, weight 4080 g. The problems of spontaneous rupture of the uterus without detectable prior damage of the uterine wall are discussed.

A case of clostridial uterine infection following prolonged labor and vaginal delivery is presented. Uterine myonecrosis with gas formation was diagnosed by radiologic studies. Antimicrobial therapy with megadosage penicillin was initiated and total abdominal hysterectomy performed. Three episodes of cardiac arrest occurred during surgery. Resuscitation was carried out with recovery.

Laparoscopic ovarian biopsies performed on 11 patients with congenital absence of the uterus revealed the presence of follicles at different stages of development, with corpora lutea in 2 patients and cystic follicles in 1 patient who had cyclic patterns of serum gonadotropins and plasma progesterone.

Combined regimen contraceptive steroids produce precocious endometrial secretion, followed by involution leading to a suppressed endometrium with small tubular glands and failure of spiral arterioles to develop; decidua-like stromal changes are dose related as is dilatation of venules. Lipids in the uterine endometrium of mice during early pregnancy were investigated by histochemical techniques.
