[The development of techniques served to widen the lateral velopharyngeal space for treating OSAHS].

Since Fujita first described uvulopalatopharyngoplasty(UPPP)in 1981,UPPP and its modified procedures have been widely used to treat obstructive sleep apnea and hyponea syndrome(OSAHS).However, despite of its wide application, the success rates was uncertain, ranging from 20% to 80%,with patients of varing Friedman stages. It is well known that the principle of UPPP is to remove the redundant tissue of palate, elongated uvula and hypertrophic tonsils in order to widen the anteroposterior space at the level of palate. But recently, surgeons have found that not only the collapse of soft palate but also …

Preoperative marijuana inhalation--an airway concern.

PURPOSE: Cannabis Sativa (marijuana) may cause a variety of respiratory disorders including uvular oedema. This case illustrates that uvular oedema secondary to marijuana inhalation may cause a potentially serious postoperative clinical problem. CLINICAL FEATURES: A healthy 17-yr-old man who inhaled marijuana prior to general anaesthesia. In the recovery room, after an uneventful general anaesthetic, acute uvular oedema resulted in post operative airway obstruction and admission to hospital. The uvular oedema was treated successfully with dexamethasone. CONCLUSION: Recent inhalation of marij…

Sleep apnoea. Laser therapy for OSAS.

The standard treatments for obstructive sleep apnoea syndrome (OSAS) to date have been postural treatment, which involves the patient sleeping on the stomach; continuous positive airway pressure, using a nasal mask during the night to maintain a positive pressure to prevent collapse of the airway; and surgical uvolopalatopharyngoplasty (UPPP). This is a radical surgical procedure where the soft palate and uvula are excised. There are drawbacks to all of these procedures, either because patients may regard as unacceptable the position they must maintain in bed or the equipment required to be wo…

Uvulopalatopharyngoplasty: strategies for success and safety.

Management of snoring and/or obstructive sleep apnea patients may include uvulopalatopharyngoplasty, a surgical operation that occasionally generates complications. The most serious are perioperative airway problems and palatal dysfunction. Strategies to minimize risks and maximize efficacy are discussed.

Obstructive sleep apnea syndrome: pathophysiology, upper airway evaluation and surgical treatment.

This paper describes the pathophysiology of the upper airway and the methods of evaluation which include mainly the Mueller maneuver, and cephalometrics. The management of Obstructive Sleep Apnea Syndrome (OSAS) is determined by the level of daytime dysfunction, the degree of cardiopulmonary complications, age, co-existing medical problems, life style, and the extent of the upper airway obstruction and abnormality. Continuous positive airway pressure (C-PAP), uvulopalatopharyngoplasty (UPPP), and permanent tracheostomy are procedures indicated depending on the severity of the case. Midline las

Anatomy and physiology related to cleft palate: current research and clinical implications.

The most important areas for future research related to the anatomy and physiology of cleft palate are thus: (1) the role of the tensor tympani muscle in auditory tube clearance; (2) the specific etiology of auditory tube dysfunction associated with cleft palate; (3) the nature and extent of variability in the muscular deformities associated with various types of cleft palate; (4) the nature and extent of vascular abnormalities associated with clefts of the palate; and (5) the investigation of cleft palate as one aspect of a craniofacial syndrome.

Duplication of (12)(pter-q13.3) combined with deletion of (22)(pter-q11.2) in a patient with features of both chromosome aberrations.

We report a patient with multiple dysmorphic signs and congenital malformations, representing a combination of clinical features of duplication (12p) and deletion (22)(q11.2) syndromes. The girl had overgrowth at birth, showed abnormal cranio-facial findings, cleft uvula, a complex conotruncal heart defect, a polycystic right kidney, and an umbilical hernia. She died at the age of 6 months of cardio-respiratory failure. Cytogenetic examination demonstrated a derivative chromosome 12 replacing one of the two chromosomes 22. The paternal karyotype was normal 46,XY while the mother's karyotype wa…

Cleft lip and/or palate in two cases of 46,X,i(Xq) Turner syndrome.

Cleft lip (CL) and/or palate (CP) are uncommon anomalies in Turner syndrome (TS) series. We report two unrelated sporadic 46,X,i(Xq) patients exhibiting orofacial clefts and a peculiar facial appearance masking the clinical diagnosis. CL, and CP in case 1 and CP in case 2, though non-specific of TS, may not be fortuitous findings. The increased frequency of CP and bifid uvula in poly X syndromes, the dermatoglyphic similarities between iXq TS and X polysomies, and the occurrence of Klinefelter phenotype when extra Xq material is present in a male, are all indirect evidences suggesting that Xq …

[Massive hyperplasia of the arytenoid mucosa with sleep apnea and stridor. Endoscopic resection by CO2 laser].

A case is presented of a 48 year-old woman with symptoms of severe rhoncopathy and obstructive sleep apnea, followed by middle inspiratory stridor and dyspnea to any phisial effort. Redundant hyperplasia of the arytenoid mucosa was discovered during exploration, with vibration during inspiration and closure of the lumen as it goes closer to the epiglottis. Also, severe septal deviation, hypertrophy of the pharyngeal tonsil, soft palate and uvula, and macroglossia, were diagnosed. Neither CPAP/BIAP or triple anti-reflux therapy were effective, and laser endoscopic excision resolved the laryngea…
