Sulfide-based, pro-regenerative, anti-inflammatory vascular grafts: Impregnation of slow-released sulfide signals into graft implants.

Hydrogen sulfide exhibits significant potential in vascular therapy due to its anti-inflammatory, antioxidant, and cytoprotective properties. However, clinical translation is hindered by the absence of biomaterial systems capable of sustained, physiologically relevant sulfide release. To address this, we have developed two coaxial electrospun systems as vascular grafts, which are impregnated with diallyl trisulfide, a stable sulfide donor, enabling controlled sulfide release through a core-shell fiber structure that enhances mechanical properties and cellular interactions. Results show that bo

Custom-Fenestrated Atrial Septal Occluder Devices in Borderline Patients With Pulmonary Arterial Hypertension or Restrictive Left Ventricular Physiology.

BACKGROUND: Operability of patients with atrial septal defects (ASD) and pulmonary arterial hypertension (PAH) is guided by various hemodynamic parameters obtained after acute vasodilator testing (AVT). Fenestrated closure is suggested in gray-zone of operability as well as restrictive left ventricular (LV) physiology. On-table fenestrations made in the catheterization laboratory close on follow-up. Nitinol wire braid and fabric are modified in custom-fenestrated occluders to create a permanent 5-8 mm communication. AIMS: This retrospective analysis studied patient characteristics and hemodyn

Management and outcomes for portal venous thrombosis after pancreaticoduodenectomy.

BACKGROUND: Postpancreatectomy portal venous thrombosis (PPVT) is an uncommon but significant complication that requires evolving management strategies. Furthermore, the actual effect of PPVT is underreported. This study aimed to summarize our institutional experience in managing patients with PPVT. METHODS: This was a single-institution database of patients who developed PPVT after pancreaticoduodenectomy or total pancreatectomy between 2011 and 2023. Patients who had preoperative PVT or underwent distal pancreatectomy were excluded from the study. RESULTS: Of 53 patients with PPVT, most we

Sheep models for evaluation of novel patch and prosthesis material in vascular surgery: tips and tricks to avoid possible pitfalls.

BACKGROUND: In vascular surgery, novel synthetic prosthesis materials for patch-angioplasties, interpositions, bypasses and shunts are continuously under development and optimization. The characteristics of an ideal vascular prosthesis would display long-term patency, biocompatibility, durability, low porosity, lack of stich hole bleeding, ease of handling, kink resistance, infection resistance and reasonable costs. The aim of this study was to establish and report a reliable sheep model including potential pitfalls where those parameters could be analyzed. Before surgery, sheep were acclimati

Rapid cellular luminal coverage of Dacron inferior vena cava prostheses in dogs by immediate seeding of autogenous endothelial cells derived from omental tissue: results of a preliminary trial.

Endothelial cell seeding methods might reduce the high failure rates of venous vascular prostheses, but low flow rates in venous vascular prostheses impose a need to protect early patency and to attain early endothelial cell coverage without waiting several weeks for relatively small endothelial cell innocula from autologous veins to form confluent linings. To obtain large number of autologous endothelial cells for high-density seeding, canine omental microvascular endothelial cells were harvested by collagenase digestion and density gradient centrifugation, with yields of 1.34 +/- 0.24 (SD) X

[Tubo-ovarian transplantation in the treatment of sterility. Experimental research].

Microsurgical transposition of fallopian tube and ovary has the potential of being an efficient therapeutic treatment in patients with tubal … aries and fallopian tubes. The microvascular techniques prove highly reliable in terms of immediate vascular patency rate but it is disappointing that 50% of the autografts has failed with blocked vessels by day 30. Perhaps this is due to the difficult techniques in anastomosing the ovarian vessels of small caliber. In spite of these outcomes the vascularized auto …

Pre-operative ultrasound mapping before arteriovenous fistula formation: an updated systematic review and meta-analysis.

BACKGROUND: Arteriovenous fistula represents the preferred vascular access for patients with kidney failure requiring hemodialysis. Surgeons have traditionally used physical examination to identify the most suitable vessels. This meta-analysis aims to evaluate whether ultrasound mapping should be routinely performed before arteriovenous fistula creation. METHODS: Medline, Scopus, Web of Science and CENTRAL were systematically searched from inception to November 1, 2022. Randomized controlled trials and cohort studies comparing routine ultrasound mapping to physical examination in terms of art

Long-Term Functional Outcomes After Surgery of Functional Popliteal Artery Entrapment Syndrome.

BACKGROUND: Functional popliteal artery entrapment syndrome (fPAES) is a subtype of PAES without anatomic abnormalities entrapment of the popliteal artery. One of the management of symptomatic fPAES is surgical exploration of the popliteal region with popliteal artery release with lysis of fibrous bands. There is a lack of data regarding the long-term functional results of this surgery, most of the studies focusing on vascular patency in anatomical PAES. The aim of this study was to assess the efficacy of surgery in functional PAES, focusing on long-term physical activity return after surgery …

[Single versus double antiplatelet therapy following coronary artery bypass grafting].

Coronary artery bypass grafting is the treatment of choice in complex obstructive coronary artery disease, particularly in diabetic patients. The long-term benefits of coronary artery bypass grafting depend on the successful execution of the anastomosis, on the appropriate choice of conduits (arterial rather than venous) but are also influenced, as for the coronary stent, by the pharmacological measures used to prevent occlusion both in the period immediately following the intervention and in the subsequent years. The use of aspirin at intermediate doses, before and after surgery, clearly impr

CIRSE Standards of Practice on Below-the-Knee Revascularisation.

The CIRSE Standards of Practice Committee established a writing group that was tasked with producing up-to-date recommendations for performing below-the-knee revascularisation, taking into account data on novel techniques, devices, and long-term outcomes that have emerged over the last decade. CIRSE Standards of Practice documents are not clinical practice guidelines or systematic reviews of the literature. This document is not intended to impose a standard of clinical patient care but recommends a reasonable approach to and best practices for performing below-the-knee revascularisation.

[Peculiarities of surgical treatment for true aneurysms of the popliteal artery].

A popliteal artery aneurysm is a rare disease and therefore it seems currently important to investigate peculiarities of surgical treatment of this pathology. Our study included a total of 60 patients (62 limbs) operated on for popliteal artery aneurysms with various complications. In order to assess the obtained findings the patients were subdivided into 3 groups depending on emergency of rendering surgical care. The spectrum of the operations performed varied from prosthetic repair of the popliteal artery from the posterior approach (graft length 7 cm) to femoroplantar bypass grafting (shunt
