Cutaneous leucocytoclastic vasculitis associated with omeprazole.

Omeprazole is a potent proton pump inhibitor and usually is well tolerated. Adverse effects of this drug have been reported in up to 5% of patients, most of which are trivial and disappear rapidly on discontinuation of the drug. Skin adverse reactions attributed to omeprazole are uncommon and include rashes, urticaria, angio-oedema, acute disseminated epidermal necrolysis, lichen spinulosus, and contact dermatitis. Cutaneous leucocytoclastic vasculitis (CLV) has not been previously reported in association with omeprazole. The development of CLV in an elderly patient four weeks after starting t…

[Allergic vasculitis following ingestion of celecoxib?].

A 76-year-old patient developed cutaneous vasculitis on the lower legs on the 8th day of treatment with the selective cox-2-inhibitor celecoxib (Celebrex((R))) and the proton-pump inhibitor omeprazole. The patient had no history of allergic reactions. The patient had already been treated previously with omeprazole without any side effects. A cutaneous biopsy confirmed the diagnosis of leucocytoclastic vasculitis. The purpuric skin lesions regressed within 3 weeks after withdrawing the two newly introduced drugs. We discuss the potential role of Celecoxib in triggering this vasculitis.

New treatments of ANCA-associated vasculitis.

The ANCA-associated systemic vasculides are, in many cases, life-threatening conditions. Combined treatment with corticosteroids and cycloph … everal of those new treatment modalities are now being tested in controlled studies by the European Vasculitis Study Group (EUVAS).

[Intestinal vasculitis--a diagnostic-therapeutic challenge].

Intestinal vasculitis is a rare cause of mesenteric ischemia. It results in chronic arterial insufficiency in most cases, sometimes in acute mesenteric ischemia. Abdominal symptoms like postprandial intestinal angina, diarrhea, anorexia, and perforation are nonspecific and do not allow for differentiation between vasculitic and noninflammatory causes of mesenteric ischemia. Conventional radiography and endoscopy can not prove the underlying process either. Therefore, extraintestinal symptoms of vasculitis must be observed carefully for diagnosing a systemic vasculitis with potential involvemen…

Leukocytoclastic vasculitis complicating Klebsiella pneumoniae bacteremia.

A 79-year old woman, with a history of hypertension, presented with clinical features of congestive heart failure, fever, a purpuric rash, and left lower quadrant abdominal tenderness. Contrast computed tomography scan of the abdomen showed features of acute diverticulitis, and blood culture was subsequently positive for Klebsiella pneumoniae. Histological examination of a biopsy of the rash confirmed a diagnosis of leukocytoclastic vasculitis (LCV). The bacteremia responded to intravenous amoxycillin/clavulanic acid and gentamicin and the rash subsided. This case represents the first case of …

Antiproteinase 3- and antimyeloperoxidase-associated vasculitis.

Antiproteinase 3- and antimyeloperoxidase-associated vasculitis. Wegener's granulomatosis, microscopic polyangiitis, and idiopathic pauci-immune necrotizing crescentic glomerulonephritis (NCGN) are strongly associated with antineutrophil cytoplasmic autoantibodies (ANCAs) directed against either proteinase 3 (anti-PR3) or myeloperoxidase (anti-MPO). This has led some investigators to prefer combining these diseases under the common heading of ANCA-associated vasculitides. However, it is increasingly recognized that there are characteristic differences between patients with anti-PR3 and those w…

Do vasculitis categorization systems really matter?

The diagnosis of systemic vasculitides is challenging for many reasons. The etiology and pathogenesis of most vasculitides are unknown or in … ayasu arteritis, polyarteritis nodosa, Kawasaki disease, Henoch-Schönlein purpura, cryoglobulinemic vasculitis, Wegener's granulomatosis, microscopic polyangiitis, and Churg-Strauss syndrome. Evidence is provided that the categorization for systemic vasculitis really does matter.

Uveitis and central nervous system vasculitis.

Vasculitis confined to the central nervous system (CNS) is a rare disease usually characterized by headache and focal neurologic symptoms. Patients with primary vasculitis of the CNS may have symptoms and laboratory findings of systemic disease such as fatigue and elevated erythrocyte sedimentation rate, but by definition, focal inflammation should not be present outside the CNS. We describe 3 patients with uveitis in association with this diagnosis. The recognition of this association adds to the complex differential diagnosis of uveitis in association with CNS disease, and indicates that "is…

Chronic immunosuppressive therapy for systemic vasculitis.

Treatment with glucocorticoids and cytotoxic agents has markedly improved the outcome in patients with systemic vasculitis. As more patients survive with these disorders, the long-term risks of these therapies have become increasingly apparent. An understanding of therapeutic toxicities is important because in some instances they can be minimized by the use of treatment strategies and monitoring techniques. Pneumocystis carinii pneumonia is associated with a high mortality rate in immunosuppressed patients but can be prevented by prophylaxis. Glucocorticoid-related avascular necrosis can incur…

Hyperimmunoglobulinemia D in idiopathic retinal vasculitis.

BACKGROUND: We examined four patients who exhibited both idiopathic retinal vasculitis and elevated serum IgD levels. Uveitis caused by Behçet's disease is also associated with high levels of serum IgD. Therefore, the clinical features of these patients were investigated and the possible relationship between retinal vasculitis and elevated serum IgD was examined after undertaking a study of increased IgD levels in patients diagnosed with uveitis. METHODS: The study population was composed of 110 patients: 49 with Behçet's disease, 15 with sarcoidosis, 10 with Vogt-Koyanagi-Harada disease, and…

Respiratory failure in ANCA-associated vasculitis.

OBJECTIVE: To assess the prevalence, clinical manifestations, and course of respiratory failure in all patients who tested positive for antineutrophil cytoplasmic autoantibodies (ANCA) in our clinics in the period between January 1985 and January 1993. DESIGN: Case-series analysis. SETTING: Three teaching hospitals in the Netherlands. PATIENTS: Two hundred twenty consecutive patients suspected of having vasculitis and/or glomerulonephritis who tested positive for ANCA by indirect immunofluorescence and enzyme-linked immunosorbent assay. RESULTS: Sixty-two patients had pulmonary involvement…

MR of vasculitis-induced optic neuropathy.

PURPOSE: To describe the MR characteristics of optic neuropathy caused by vasculitis. METHODS: Nine cases of optic neuropathy with diagnosis of vasculitis (six with systemic lupus erythematosis and one each with rheumatoid arthritis, Sjögren disease, and radiation vasculitis) were reviewed retrospectively. Patients were 31 to 62 years old, and all but one were women. All patients had MR imaging through the orbits and anterior visual pathways, five with fat suppression, with and without gadopentetate dimeglumine. Five patients also had MR imaging of the entire brain. The size and enhancement o…

Vasculitis masquerading as neurologic decompression illness.

An adult male diver developed limb pains and peripheral neurologic signs following a series of dives. He was treated for acute neurologic decompression illness (DCI) and responded well, but subsequently he deteriorated and developed features of a progressive multisystem disease. Investigation confirmed a clinical diagnosis of polyarteritis nodosa complicated by a mononeuritis multiplex. Vasculitis is uncommon and its masquerade as neurologic DCI may be unique. However, this case empha-sizes the importance of careful clinical assessment and illustrates the potentially wide differential diagnosi…

Sepsis and tumor emboli mimic vasculitis.

An unequivocal diagnosis of vasculitis typical of polyarteritis was made in a sixty one year old male. At post mortem no vasculitis was found, but there were widespread microabscesses and tumour emboli from an occult adenocarcinoma of the caecum. The importance of considering other pathologies which may mimic vasculitis is emphasised.

Cranial MRI in idiopathic retinal vasculitis.

Ten patients with clinically isolated idiopathic retinal vasculitis who had a positive family history for multiple sclerosis (MS) or positive typing for HLA B7 underwent magnetic resonance imaging (MRI) of brain and optic nerves in order to establish the frequency of clinically silent lesions. Brain MRI was normal in seven and abnormal in three: one had a single small white matter lesion, two had extensive white matter abnormalities resembling those seen in MS. In two patients a lesion was shown in the optic nerve. These findings suggest that a minority of patients with idiopathic retinal vasc…

[CT & MRI findings of refractory vasculitis].

We described CT and MRI findings of various refractory vasculitis. Refractory vasculitis must be divided into two categories including one group with major vascular involvement and another one with small or microvascular involvement. In cases with major vessel involvement such as Takayasu's arteritis, Kawasaki disease or vasculo-Behçet disease, CT and MRI depicted direct findings of vessels itself including stenosis, obstruction, aneurysm, and wall thickening. On the other hand, CT and MRI showed various extravascular findings in cases with small or microvascular involvement, such as CNS invol…
