Computed tomography angiography of congenital left-sided obstructive heart lesions: a pictorial essay.

Congenital left-sided obstructive heart lesions (LSOLs) are a group of rare but serious congenital heart diseases. They encompass a spectrum of structural abnormalities that can obstruct blood flow at any point from the left atrium to the aortic arch. They are broadly divided into two main categories. The first is left ventricular inflow obstruction, including cor triatriatum sinister and congenital mitral stenosis. The second is left ventricular outflow obstruction, including subvalvular aortic stenosis, bicuspid aortic valve, supravalvular aortic stenosis, coarctation of the aorta, aortic ar…

Post-carvedilol myocardial function in cats with obstructive hypertrophic cardiomyopathy.

INTRODUCTION: Hypertrophic cardiomyopathy (HCM) is the most prevalent cardiac disease in cats, and one phenotype includes obstructive HCM with dynamic left ventricular outflow tract obstruction (DLVOTO). Myocardial function has been reported to be lower in cats with obstructive HCM than in non-obstructive HCM. Carvedilol, because of its pharmacological action, is expected to reduce the pressure gradient associated with DLVOTO, but no previous reports have studied its effects on myocardial function. This study aimed to evaluate myocardial function in cats with obstructive HCM with left ventricu

[Discrete Subaortic Stenosis in an Adult; Report of a Case].

Discrete subaortic stenosis (DSS) is a well-described cause of isolated left ventricular outflow tract obstruction( LVOTO) in children. But prevalence, rate of progression and postoperative data in adults are limited. We report a case of a 30-year-old woman, who was referred to our institution because of chest pain and loss of consciousness. Echocardiography revealed DSS with LVOTO (peak gradient 81 mmHg) and mild aortic regurgitation. Increased age at the time of diagnosis, female sex and preoperative left ventricular outflow tract(LVOT) gradient ≥80 mmHg were thought to be predictors for reo

Hypertrophic cardiomyopathy and caesarean section: intraoperative use of transthoracic echocardiography.

A pregnant woman with severe hypertrophic cardiomyopathy was monitored with transthoracic echocardiography to guide management of fluids and vasopressors during elective caesarean section. After insertion of intravenous, arterial and central venous cannulae, a transthoracic echocardiogram was performed noting left ventricular cavity size and contractility, systolic anterior motion of the mitral valve and left ventricular outflow tract obstruction. Following combined spinal-epidural anaesthesia, serial examinations were made intraoperatively. Short-lived haemodynamic instability ensued just bef

Back to normal: super-response to mavacamten in a patient with obstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a primary myocardial disorder characterized by left ventricular hypertrophy, often leading to left ventricular outflow tract (LVOT) obstruction and secondary mitral regurgitation (MR) due to systolic anterior motion (SAM) of the mitral valve leaflets. Mavacamten, a cardiac myosin inhibitor, is a modern treatment option aimed at reducing myocardial contractility, SAM and LVOT obstruction. We present the case of a patient with HCM responding not only with a marked reduction in LVOT obstruction and mitral regurgitation, but also with normalization of left vent

Extensive Right Ventricular Involvement With Thrombus and Myocarditis: An Unanticipated Discovery in Advanced Hepatocellular Carcinoma.

BACKGROUND: Cardiac involvement in malignancies is rare, with right ventricular tumoral thrombus in hepatocellular carcinoma (HCC) without right atrial extension being exceptionally uncommon. CASE SUMMARY: A 54-year-old man with stage IV HCC experienced recurrent chest discomfort. Stress echocardiography detected an ill-defined inhomogeneous mass, and cardiac magnetic resonance identified right ventricular outflow tract obstruction, left ventricular septal infiltration, and acute-to-subacute myocarditis. Abdominal imaging showed tumor recurrence along with a large thrombus in the retrohepatic

Transcatheter right ventricular outflow tract stenting in children with postoperative infundibular stenosis and preserved pulmonary valve function.

Recurrent or residual right ventricular outflow tract obstruction after early surgical repair of congenital heart disease is one of the most frequent indications for either surgical or transcatheter reintervention. Transcatheter stent implantation across the stenotic right ventricular outflow tract or conduit is a safe and effective alternative to surgical reintervention. However, chronic deleterious effects of pulmonary regurgitation can potentially counterbalance the early improvement in clinical and hemodynamic parameters, sometimes necessitating further intervention. While there are severa

Myocardial infarction induced by oral terazosin in a patient with predisposing structural cardiomyopathy: case report.

We describe a 71-year-old male patient who developed acute myocardial infarction (AMI) due to a dynamic left ventricular outflow tract obstruction induced by terazosin. After receiving terazosin, the patient had a syncope followed by angina. The electrocardiogram showed Q waves and ST segment elevation in the precordial and inferior leads. Coronary angiography evidenced a chronically occluded left anterior descending artery. Doppler-echocardiography revealed apical akinesia, hyperdynamic basal segments, systolic anterior motion of the mitral valve (SAM) and dynamic left ventricular outflow tra

Left Atrial Volume Index, Left Ventricular Outflow Tract Gradient, and Incident Atrial Fibrillation in Hypertrophic Cardiomyopathy.

BACKGROUND: Atrial fibrillation (AF) is a common complication of hypertrophic cardiomyopathy (HCM). Whether left atrial volume index (LAVI) or resting left ventricular outflow tract (LVOT) gradient better identifies patients who develop AF is unresolved. OBJECTIVES: To compare baseline LAVI and resting LVOT gradient as predictors of incident AF in HCM. METHODS: We retrospectively studied 777 patients with HCM identified by ICD-10 code I42.1 (obstructive hypertrophic cardiomyopathy). Patients with pre-existing AF or atrial flutter were excluded, leaving 547 patients. The primary exposure was

Efficacy and Safety of Aficamten in Children and Adolescents With Obstructive Hypertrophic Cardiomyopathy: Study Design and Rationale of CEDAR-HCM.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is an important cause of morbidity and mortality in children, but treatment options are limited. Aficamten, a next-in-class cardiac myosin inhibitor, directly targets the hypercontractility underlying HCM. Aficamten improved exercise capacity, health status, and symptoms in adults with obstructive HCM in the pivotal, phase 3 SEQUOIA-HCM trial (Safety, Efficacy, and Quantitative Understanding of Obstruction Impact of Aficamten in HCM; NCT05186818). Dynamic outflow tract obstruction in congenitally corrected transposition of the great arteries.

Congenitally corrected transposition of the great arteries (CCTGA) is a rare form of congenital heart disease, constituting 0.5% of all congenital heart defects. The incidence of left ventricle (non-systemic ventricle) outflow tract obstruction ranges between 44 and 57%. Herein, we present the case of a 45 year old woman with CCTGA with progressively worsening dyspnea who had been referred for surgical correction of severe systemic ventricle (morphologic right ventricle) atrio-ventricular valve (tricuspid valve) regurgitation. Cardiac magnetic resonance imaging (CMR) and transesophageal imagin

Beta blocker therapy in recipients of twin-to-twin transfusion syndrome.

Recipients of severe twin-to-twin transfusion syndrome (TTTS) may suffer from low cardiac output caused by myocardial hypertrophy and sudden postnatal drop in preload. Our hypothesis was that selective beta-1 adrenergic blockers improve cardiac function in TTTS recipients with left ventricular outflow tract obstruction. We analysed data from two TTTS recipients treated with esmolol/metoprolol. Despite intense circulatory support, both patients showed severe hypotension and tachycardia before therapy. Echocardiographic findings included hypertrophic ventricles with thickened intraventricular se

Concomitant septal myectomy at the time of aortic valve replacement for severe aortic stenosis.

BACKGROUND: Left ventricular outflow tract obstruction may be unmasked after a successful aortic valve replacement (AVR) for severe aortic stenosis in the setting of asymmetrical basal septal hypertrophy (ABSH). The quantitative assessment of the obstructive potential of ABSH adjacent to a severely stenotic valve can be challenging. We reviewed our experience with patients who underwent concomitant septal myectomy at the time of AVR for severe aortic stenosis. METHODS: During the 10-year period ending January 2009, 3,523 patients underwent AVR for the primary indication of severe aortic steno

Transesophageal echocardiography guidance of percutaneous mitral valve replacement in failed annuloplasty ring: A case report.

Severe mitral regurgitation and stenosis due to failed mitral annuloplasty ring can be managed with percutaneous mitral valve in ring in high surgical risk patients. A 66-year-old male underwent coronary artery bypass surgery and mitral valve ring annuloplasty 7 years previously. He started to have shortness of breath with minimal effort in the past 2 years. Transthoracic echocardiogram revealed a new severe mitral regurgitation and severe mitral stenosis. The patient was turned down from surgery due to high surgical risk. The transcatheter mitral valve in ring implantation was decided. In thi
