Surgical management of univentricular heart with subaortic obstruction.

In children with a univentricular heart and a rudimentary subaortic chamber, surgical relief of subaortic obstruction caused by a restrictive bulboventricular foramen is associated with high morbidity and mortality. A 6-year-old child with a univentricular heart of the left ventricular type, a rudimentary subaortic chamber, and atresia of the left-sided atrioventricular valve had pulmonary artery banding in infancy. Severe subaortic obstruction subsequently developed. At operation, the pulmonary artery was transected and the stump was anastomosed directly to the posterior aspect of the ascendi

Ventricular septal defect and left sided obstructive lesions in infants.

Infants born with ventricular septal defect and left sided obstructive lesions display a variety of pathophysiologic features. Some patients have relatively straightforward afterload- or preload-induced congestive heart failure, whereas others have a mixed pattern. The presence of patent ductus arteriosus may add to the left-to-right shunt, or when the physiology is that of preductal coarctation, may be associated with a small left ventricular chamber. Over the years, various management schemes have been devised for infants with ventricular septal defect and left sided obstructive lesions. The

Permanent Left Ventricular Assist Devices: Long-Term Management and Outcomes.

This article outlines the historical development, physiologic principles, and contemporary clinical management of durable left ventricular assist devices, with a primary focus on the HeartMate 3 era. An understanding of centrifugal pump physiology, pulsatility index, power, and flow estimation is essential for effective long-term patient management. While major complications such as pump thrombosis and stroke have declined, gastrointestinal bleeding, right ventricular failure, infection, arrhythmias, and outflow graft obstruction remain a significant clinical challenge. Nonetheless, left ventr

Ross-Konno Procedure With Cylinder Mitral Valve Replacement in 49 Days Old Infant.

A neonate with antenatally diagnosed severe aortic valve stenosis, left ventricular outflow tract obstruction, left ventricular endocardial fibroelastosis, and severe mixed mitral valve disease was admitted to our center for balloon aortic valvuloplasty. On day 49, we performed a Ross-Konno procedure and mitral valve replacement on the baby, whose weight was 3.4 kg. Right ventricle-to-pulmonary artery continuity was restored with a handmade trileaflet Gore-Tex conduit. We used a handmade cylindrical prosthesis made from decellularized equine pericardium in the mitral position. We observed impr

Fabry's disease with LVOT obstruction: diagnosis and management.

A 46-year-old male was diagnosed with hypertrophic cardiomyopathy (HCM) and severe left ventricular outflow tract (LVOT) obstruction. Genetic testing revealed that this was a "mimic of HCM" and the true diagnosis was Fabry's disease, although there were no other clinical features of Fabry's. Despite maximal medical therapy he remained symptomatic from the outflow tract gradient and required surgical myectomy for symptom relief. This case illustrates the diagnosis of an HCM mimic by genetic testing.

Visualization of 3D Models Through Virtual Reality in the Planning of Congenital Cardiothoracic Anomalies Correction: An Initial Experience.

We present the case of an nine-year-old girl with double outlet right ventricle with noncommitted ventricular septal defect and malposition of the great arteries who had undergone repair at the age of seven months. Six years later, the patient presented with right ventricular failure, conduit calcification with obstruction, and obstruction of the left ventricular outflow tract. Three-dimensional models reconstructed by Digital Imaging and Communications in Medicine (DICOM) images of the patient were visualized in a virtual reality system to help plan the surgical correction of the intracardiac

Management of Left Ventricular Intramural Hematoma- an Unusual Complication of Complete Atrioventricular Septal Defect Repair in a Child.

Postoperative survival for complete atrioventricular septal defect (cAVSD) is excellent. Common complications of surgery include valvular stenosis/regurgitation, left ventricular outflow tract obstruction, arrythmias, and residual defects. We report a challenging case of a 7-month old girl with Trisomy 21, preoperative obstructive sleep apnea with severe pulmonary hypertension, who underwent AVSD repair and required veno-arterial extracorporeal membrane oxygenation (V-A ECMO) while unable to come off cardiopulmonary bypass and developed left ventricular intramural hematoma during the course. A case of hypertrophic cardiomyopathy with previous aortic valve replacement.

We describe a 45-year-old patient who was diagnosed with hypertrophic obstructive cardiomyopathy (HOCM) after the aortic valve replacement surgery. Enlarged left atria, thickened ventricular septum, left ventricular outflow tract stenosis, moderate mitral regurgitation and mild tricuspid regurgitation in the echocardiography were found. We offered the patient the new minimally invasive treatment modality: percutaneous intra-myocardial septal radiofrequency ablation (PIMSRA). We demonstrate the safety and efficacy with pictures. One month after surgery, the patient recovered well with improved

Long-term follow-up of operated congenital heart disease.

The long-term follow-up status of patients operated upon for the more common forms of congenital heart disease is reviewed. Among patients with large left-to-right shunts the long-term prognosis for those with ASD and VSD is generally excellent. With rare exceptions, these patients can look forward to a normal adulthood. Residual atrioventricular valve disease complicates the long-term course of infants and children operated upon for atrioventricular canal, but nevertheless, most patients have remained stable for many years. Among patients after relief of right ventricular outflow tract obstru

Sub-aortic tunnel stenosis complicated by infective endocarditis: a case report.

Tunnel sub-aortic stenosis is a very unusual form of obstruction to left ventricular outflow. Endocarditis involving this congenital anomaly is extremely rare and not described in literature. We report the case of a 45-year-old woman who was diagnosed with a severe left ventricle outflow obstruction because of sub-aortic tunnel and was proposed for elective surgery. One month after diagnosis of her heart disease, she presented with fever and deterioration of general condition. Diagnosis of infective endocarditis secondary to a community acquired streptococcus viridans was confirmed on the basi

[Two-dimensional echocardiography in subvalvular aortic stenosis. Apropos of 20 cases].

Twenty cases of discrete subvalvular aortic stenosis in children aged from 8 months to 23 years were examined by 2-dimensional echocardiography and cineangiography. Fifteen of these patients subsequently underwent open heart surgery to relieve the obstruction. In 18 cases the obstacle was a fixed stenosis (13 type I, 3 type II and 2 type III) and in two cases, the obstacle was provoked by accessory tissues of the anterior mitral leaflet. The echocardiographic incidences used were the left parasternal long axis, the apical LAO-equivalent, and longitudinal and transverse subcostal views. In type

A rare case of biventricular non-compaction associated with ventricular septal defect and descendent aortic stenosis in a young man.

Non-compaction of the ventricular myocardium is a cardiomyopathy caused by the arrest of normal embryogenesis of the ventricles. It is classified in isolated non-compaction of the ventricles (most frequently of the left one) and in ventricular non-compaction associated with other congenital anomalies of the endocardium and myocardium, such as obstruction of the right or left ventricular outflow tracts, complex cyanotic congenital heart disease, and coronary artery anomalies. There are controversies regarding the right ventricle non-compaction due to the normally trabeculated shape of its walls

A Case Report of Spinal Anesthesia for Cerclage Placement in the Setting of Severe Hypertrophic Obstructive Cardiomyopathy.

Neuraxial anesthesia is preferred over general anesthesia in obstetric patients to avoid airway manipulation, aspiration, and maternal-fetal transfer of medications; however, a sudden sympathetic block is generally avoided in patients with hypertrophic obstructive cardiomyopathy (HOCM). The case of a 31-year-old G2P0010 with HOCM with severe resting left ventricular outflow tract (LVOT) obstruction and systolic anterior motion of the mitral valve undergoing a cerclage under choroprocaine spinal anesthesia is presented. Risks and benefits of general versus neuraxial anesthesia, and epidural ver

Outcome of septal myectomy in patients with Fabry's disease.

Fabry's disease has cardiac manifestations that may mimic hypertrophic cardiomyopathy, including left ventricular outflow tract (LVOT) obstruction. It is important to distinguish hypertrophic cardiomyopathy from Fabry's disease, which can be treated with enzyme replacement therapy (ERT). However, after adequate ERT, some patients have no resolution of cardiac symptoms; thus, septal myectomy might be considered. We present 2 cases of extended septal myectomy in patients with LVOT obstruction secondary to Fabry's disease.

Postoperative evaluation of the Fontan procedure by radionuclide angiography.

Seven patients were evaluated by radionuclide angiography (RNA) following Fontan-like procedures for complex cyanotic congenital defects. Both first-pass and ECG gated equilibrium angiography were performed in each. Residual right-to-left shunts, right atrial (RA) outflow and pulmonary arterial obstruction, and left ventricular dysfunction were demonstrated by these techniques. In addition, RA ejection fraction (RAEF) by gated equilibrium scan was measured in each patient. Relatively low RAEF was seen in conjunction with residual obstruction to pulmonary flow. RNA is useful in the postoperativ

Managing a mavacamten non-responder in obstructive hypertrophic cardiomyopathy: A case report.

UNLABELLED: We report the case of a woman in her 40s with obstructive hypertrophic cardiomyopathy who remained symptomatic despite optimal medical therapy. A prior attempt of alcohol septal ablation was not performed due to unsuitable septal branch anatomy. Echocardiography demonstrated severe left ventricular outflow tract (LVOT) obstruction, systolic anterior motion (SAM) of the mitral valve (MV), and moderate-to-severe mitral regurgitation. Biomarkers indicated significant hemodynamic stress, and magnetic resonance imaging revealed myocardial fibrosis. Treatment with the cardiac myosin inhi

The Ross procedure in patients aged less than 18 years: the midterm results.

OBJECTIVE: This study reviews a single-center experience with the Ross procedure in infants and young children. METHODS: From November 1993 to March 2012, 55 children aged less than 17 years underwent a Ross procedure. The patients ranged in age from 2 days to 17 years (median, 5.9 years). Thirteen patients were infants, and 18 patients were preschool children. The predominant indication for the Ross procedure was aortic stenosis. Twenty-seven patients (49%) with left ventricular outflow tract obstruction underwent a modified Ross-Konno procedure. Twenty-five patients (45%) had undergone 40 p

Significance of brain natriuretic peptide in the evaluation of symptoms and the degree of left ventricular diastolic dysfunction in patients with hypertrophic cardiomyopathy.

INTRODUCTION: Brain natriuretic peptide (BNP) correlates well with left ventricular wall hypertrophy and the severity of left ventricular outflow tract (LVOT) obstruction in patients with hypertrophic cardiomyopathy (HCM). The purpose of this study was to investigate the correlation of BNP levels with clinical status and left ventricular function in HCM patients. METHODS: In 32 patients with HCM (age 58 +/- 14 years) we evaluated: (a) the severity of dyspnea and angina; (b) BNP plasma levels; (c) left ventricular wall thickness, sphericity index and mitral regurgitation; (d) LVOT obstruction;

Surgical Ventricular Septal Myectomy in the Developing World.

Surgical septal myectomy is the preferred treatment strategy in hypertrophic cardiomyopathy (HC) to reverse progressive heart failure symptoms due to left ventricular outflow obstruction. However, open-heart surgery and the ventricular septal myectomy option in particular have limited accessibility in many parts of the world, including developing countries such as India, where skilled surgeons familiar with the complex left ventricular outflow tract anatomy of obstructive HC are rare. Indeed, septal myectomy, which is currently performed in only a limited number of countries, can become availa
