Infantile nystagmus syndrome (INS), formerly referred to as congenital nystagmus, typically presents in early infancy. We report a rare case in which nystagmus consistent with INS became clinically apparent after a vertigo attack during adolescence. A 17-year-old female developed acute rotational vertigo. Although the vertigo gradually subsided, she continued to perceive rightward visual motion. Five months later, she was referred to our institution because of persistent spontaneous and gaze-evoked nystagmus. There was no prior history or documentation of nystagmus. Examination revealed jerk-t…

In this article, we review studies that bring video-oculography (VOG) to the emergency department and inpatient settings, with an emphasis on acute vestibular syndrome. We find that most current studies are research-based, rather than pilot clinical programs. Many focus on the use of VOG to differentiate central disorders (eg, ischemic stroke) from peripheral (eg, inner ear) mimics. We critically appraise the classes of eye movements studied by each, and then provide an update on recent work in leveraging artificial intelligence, telemedicine, and the use of consumer devices and smartphones fo

OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) frequently recurs, affecting quality of life. Although migraine and environmental factors such as atmospheric pressure and temperature have been proposed as predictors, their precise influence remains unclear. We aimed to evaluate whether migraine, thyroid disease, and environmental factors (atmospheric pressure and temperature) predict recurrence of BPPV.
METHODS: We retrospectively analyzed data from 374 patients diagnosed with BPPV between 2018 and 2024 at a university otolaryngology clinic. Clinical history, migraine and thyroid statu…

Dizziness and vertigo are frequent leading symptoms also in the ophthalmological practice and hospitals. They require a structured clinically oriented diagnostic approach. This article provides a practical approach based on a focused history taking using four key questions as well as a systematic examination of ocular motility (four static and four dynamic tests) and four tests assessing the vestibular system. The focus is on differentiating peripheral from central causes through structured history taking and clinical examination. In addition, the functional anatomy of the brainstem and cerebe…

INTRODUCTION: The diversity of underlying causes poses a medical challenge in the differential diagnosis of dizziness. For many patients, the time to reach a diagnosis is quite long and includes several tests. This imposes a significant financial burden on the healthcare system while delaying the recognition and treatment of some neurological emergencies. In this paper, we present a machine learning-based method to aid in the early recognition of vascular causes of vertigo, which require timely management.
METHODS: Using data such as age, gender, accompanying symptoms, comorbidities, and comm…

The clinical manifestation, physical and laboratory examination, electrophysiological, and imaging data of 2 female adult OMS patients with vertigo were analyzed at the Department of Neurology of the First Medical Center of Chinese PLA General Hospital from February 2021 to March 2022. The treatment strategy and clinical outcome were followed up. The two female patients were aged 42 and 66 years. Anti-NMDA receptor antibody and anti-GABAB receptor antibody were detected in serological screening, respectively. The two patients met the diagnostic criteria for OMS, and one was screened for breast…

(1) Background: Patients with vestibular migraine (VM) often present with positional vertigo. A portion of these patients have features of benign paroxysmal positional vertigo (BPPV). It is a challenge to rapidly identify the BPPV component of VM associated with positional vertigo. (2) Methods: Retrospective data collected from 60 VM and 47 VM + BPPV patients were used to build a diagnostic model, and then prospective data from 47 patients were used for the external validation. All patients had VM manifesting as positional vertigo, with or without accompanying BPPV. The clinical manifestations…

Although dizziness in VS is chronically progressive in nature, VS may present as an acute vestibular syndrome that mimics VN. Central vertigo is a result of vestibular structure dysfunction in the central nervous system. Currently, misdiagnoses between peripheral and central lesions are frequent, and diagnostic testing costs are high. Identifying the characteristics of these 2 conditions is challenging. We can provide better treatment if we can establish a diagnosis earlier. Cerebral cavernous malformation (CCM) at the cerebellum is a cerebellar lesion that causes symptoms of central vertigo. We report a patient, 20th years old, female, with vertigo for 1 month before being admitted. Vertigo was getting worse, and wh…

Benign paroxysmal positional vertigo is a rare vestibular disorder in the pediatric population. It is a vestibulopathy characterized by brief attacks of vertigo, which occur after specific movements. This review aims to provide the current evidence regarding benign paroxysmal positional vertigo in children. This is a narrative review of the available literature on benign paroxysmal positional vertigo in children. The studies were retrieved from systematic searches on PubMed and by cross referencing. Few studies have focused on pediatric benign paroxysmal positional vertigo, and most are retros…

BACKGROUND: Menière's disease (MD) is an inner ear disorder due to raised endolymphatic pressure (hydrops), characterized by cochlear symptoms associated with episodic vertigo. In delayed hydrops, vertigo attacks begin long after the onset of a hearing loss. Few were published on MD in which the onset of vertigo precedes cochlear symptoms by several months. Vestibular migraine (VM) is also a cause of episodic vertigo and an association between migraine and MD was proposed. Purpose of our retrospective work was to assess clinical features associated with MD in which vertigo precedes by months c…

BACKGROUND: In the present study, we investigated the relationship between caroticovertebral Doppler USG measurement results and Vertigo Symptom Scale-Short Form (VSS-SF) in patients with idiopathic vertigo.
METHODS: Fifty patients with idiopathic vertigo and 30 healthy subjects were included into the study. Ear, Nose & Throat (ENT) examination, audiological examination, routine hemogram, biochemichal tests and temporal magnetic resonance imaging were performed to diagnose "idiopathic vertigo". By carotico-vertebral Doppler ultrasonography (USG), common carotid artery (CCA) area, intima media…

Ménière disease is a disorder of the inner ear, characterized by rotational vertigo, hearing loss, tinnitus, and vegetative symptoms. The aim of the present research is to examine the effectiveness of betahistine and piracetam in the reduction of vertigo attacks in Ménière disease. To verify our hypothesis, 105 (31 male and 74 females, mean age [standard deviation], 57.4 [11.05]) adult patients with definite Ménière disease were enrolled in this investigation. Beside the analysis of the hospital records, the subjective complaints of the patients and the completed vertigo diaries were taken int…

Cerebellar dizziness and vertigo account for approximately 10% of diagnoses in a tertiary dizziness center. This term summarizes a large group of disorders with chronic (degenerative, hereditary, acquired cerebellar ataxias), recurrent (episodic ataxias), or acute (stroke, inflammation) presentations. Key to the diagnosis is a comprehensive examination of central ocular motor and vestibular function. Patients with cerebellar dizziness and vertigo usually show a pattern of deficits in smooth pursuit, gaze-holding, saccade accuracy, or fixation-suppression of the vestibulo-ocular reflex. Central…

INTRODUCTION: Physicians are found to have a great and increasing amount of patients suffering of vertigo in their everyday practice. For the treatment of the balance disorders, a vestibular training was introduced which played an influential role based on our study.
AIM: Our aim was to create an easy and comprehensible training program which can be performed even with a family member.
METHODS: Our clinical treatment consists of intravenous vasoactive medicine administration, combined with vestibular training. For the investigation of the improvement we used the Jacobson and Newman questionn…
