Normal Gain with Corrective Saccades in the Video Head Impulse Test: Clinical Implications and Diagnostic Considerations.

Background: The video head impulse test (vHIT) is a standard tool for assessing semicircular canal function through vestibulo-ocular reflex (VOR) gain and corrective saccades (CS). Although reduced gain accompanied by CS typically indicates vestibular hypofunction, a paradoxical pattern-normal gain with CS-is occasionally observed, and its clinical significance remains unclear. Methods: We retrospectively analyzed 1573 patients who underwent both vHIT and bithermal caloric testing between September 2022 and September 2025. Among them, 1174 subjects with normal bilateral horizontal VOR gains (0

Capturing nystagmus during vertigo attacks using a smartphone: adherence, characteristics, pearls and pitfalls.

OBJECTIVE: To investigate adherence, characteristics, and first clinical experiences of capturing ictal nystagmus at home, which can be performed to complement the diagnostic process in patients with episodic vestibular symptoms. METHODS: Patients were recruited at a tertiary referral center in case capturing ictal nystagmus could contribute to the diagnostic process (e.g., to detect or rule out BPPV). They were asked to capture ictal nystagmus with their own smartphone at home, using a smartphone-based adapter (Nystagmocatcher, Balansdiagnos, Stockholm, Sweden). All recordings were analyzed

Vestibular compensation.

Regulation of the human equilibrium differs from the other, mostly linear, human senses. It consists of a multitude of receptors and an interrelated network that is necessary for data processing as well as for regulatory processes. Thus, the human equilibrium system represents a mixture of hardware and related software interactions. The input is also named the "equilibrium tetrade" (1-3), and comprises vision, vestibular inertia acceleration perception, proprioception and hearing. In the case of various types of lesion, one has to deal with a variety of morphological as well as mostly function

[Equilibrium function in Menière's disease].

In order to assess objectively the equilibrium function in Ménière's disease, stabilographic measurements were performed in combination with a battery of provocative tests. In 55 patients, static and dynamic equilibrium was investigated on a differential basis and compared with the data calculated for healthy people of the same age. In Ménière's disease, patients showed a decline of the equilibrium function (P less than 0.001) between attacks, the decline enhancing with the duration of the disease. It was found that one of the early signs of functional insufficiency of the equilibrium function

The dizzy patient. A practical approach to management.

Dizziness can generally be divided into true vertigo and pseudovertigo (giddiness or light-headedness). The most common causes of pseudovertigo are hyperventilation, orthostatic hypotension, and multisensory deficits of older patients. Of the many types of true vertigo, only a few are caused by serious structural disorders of the brainstem, and these can usually be recognized by their temporal profile and concomitant symptoms and signs. Most cases of vertigo are caused by peripheral vestibular disorders that are self-limiting. Treatment is directed toward control of the acute autonomic symptom

[Electronystagmograhy in disseminated sclerosis. Uses and limits (author's transl)].

A review of the literature shows the extreme frequency of nystagmus and changes in vestibular reflexes in disseminated sclerosis. During the course of this disease, it is possible to detect nystagmus in at least two-thirds of cases and in a similar proportion there are changes in vestibular reflexes, usually a hyperreflexia, especially in the early phases of disseminated sclerosis. Subjectively the patients rarely complain of vertigo and such symptoms are in any case part of the early form of the disease. On the other hand, disorders of balance are much more frequent but the factors involved a

Measurement of eye deviation under the nonfixative condition in patients with peripheral labyrinthine disorders, especially Meniere's disease.

Among 232 Japanese patients with peripheral disorders, 98 had a unilateral Meniere's disease. In these patients and 20 healthy adults, recordings of eye deviation were made utilizing DC-ENG under two conditions, i.e. eyes closed (EC) and eyes open in the absolute darkness (AD). (1) 90 % tolerance interval of AD. was significantly smaller than that of EC in controls. (2) Appearance of nystagmus in 5 groups with labyrinthine lesions was compared with the direction of deviation toward and against the affected side in both conditions. (3) Concerning unilateral Meniere's, the relationship among dev

[Sensory components of optokinetic nystagmus in exposure to the action of angular accelerations].

The relationship between the vestibular and optokinetic nystagmus was investigated using a special vestibulooptokinetic trainer. A correlation between the objective index of the angular velocity of the slow component of the optokinetic nystagmus and concomitant sensory reactions responsible for the visual perception of optokinetic stimuli was examined. It was shown that tolerance of the optokinetic system can be characterized by the differential criterion +/-deltaw that is the difference between the angular velocity of the slow component of the optokinetic nystagmus and the angular velocity of

Effects of middle ear pressure on the inner ear.

It was demonstrated in experiments on normal subjects that moderate ambient pressure changes, creating overpressure in the middle ear, may induce a vestibular reaction. In other experiments on subjects suffering from acute attacks of Meniere's disease, relief of symptoms was achieved by means of ambient pressure changes of the same magnitude.

Diagnostic accuracy of bone-conducted cervical vestibular- evoked myogenic potentials in early infancy: a prospective cohort study.

BACKGROUND/OBJECTIVES: Vestibular function assessment in infants is demanding. To develop age-appropriate methods for vestibular investigation early in infancy and in newborns we studied whether Bone-Conducted cervical Vestibular-Evoked Myogenic Potentials (BCcVEMP) is an accurate method. We also outlined BCcVEMP wave values in early infancy and compared them with values obtained at one year of age in the same cohort. MATERIALS AND METHODS: A total of 53 infants and newborn were included in the study (median age 6.4 weeks, IQR 8.29). Referrals were made to a university-affiliated audiology an
