Optic disc swelling in a patient with tuberculous meningitis: a diagnostic challenge.

We discuss the diagnostic challenge in an adult patient presented with purely ocular symptoms diagnosed with tuberculous meningitis (TBM). A 38-year-old woman presented with bilateral painless blurring of vision. There were bilateral sixth cranial nerve palsy and bilateral optic disc swelling. Optic nerve function tests were normal. Patient was lucid with no signs of meningism. Brain imaging were normal. She had a positive Mantoux test, high erythrocyte sedimentation rate but no clinical evidence of active pulmonary tuberculosis infection. Her Quantiferon-TB Gold in-tube test was negative. Cer

Vision screening of older drivers for preventing road traffic injuries and fatalities.

BACKGROUND: Demographic data in North America, Europe, Asia, Australia and New Zealand suggest a rapid growth in the number of persons over the age of 65 years as the baby boomer generation passes retirement age. As older adults make up an increasing proportion of the population, they are an important consideration when designing future evidence-based traffic safety policies, particularly those that lead to restrictions or cessation of driving. Research has shown that cessation of driving among older drivers can lead to negative emotional consequences such as loss of independence and depressio

Perceived Visual Distortions in Juvenile Amblyopes During/Following Routine Amblyopia Treatment.

PURPOSE: To establish the point prevalence of perceived visual distortions (PVDs) in amblyopic children; the association between severity of PVDs and clinical parameters of amblyopia; and the relationship between PVDs and amblyopia treatment outcomes. METHODS: Perceived visual distortions were measured using a 16-point dichoptic alignment paradigm in 148 visually normal children (aged, 9.18 ± 2.51 years), and 82 amblyopic children (aged, 6.33 ± 1.48 years) receiving or following amblyopia treatment. Global distortion (GD; vector sum of mean-centered individual alignment error between physical

The Near Mallett Unit Foveal Suppression Test: a cross-sectional study to establish test norms and relationship with other optometric tests.

When the binocular system comes under prolonged stress in decompensated heterophoria, a small suppression area can develop at the fovea. This foveal suppression (FS) may be an adaptation to eliminate symptoms. The near Mallett unit (NMU) is a hand-held instrument that contains a binocular status test for detecting and quantifying FS. We measured FS in a cross-sectional study of 131 patients to investigate the normal range of responses. A ratio scoring method was devised which considers the number of letters read with dichoptic viewing when binocularly fused (b) and under monocular (m) conditio

Visual impairments in people with severe and profound multiple disabilities: an inventory of visual functioning.

BACKGROUND: The prevalence of visual impairments in people with severe and profound multiple disabilities (SPMD) is the subject of considerable debate and is difficult to assess. METHODS: In a typical Dutch care organization, all clients with SPMD (n = 76) participated in the study and specific instruments adapted to these clients (requiring a minimum of cooperation) were used to measure visual acuity, the visual field, binocular vision, contrast sensitivity, refractive errors and visual functioning behaviour. RESULTS: We found an unexpected 92% of clients with SPMD to have visual impairment

[Traffic ophthalmology. Assessment of ability to drive in road traffic].

Assessment of an individual's ability to drive in road traffic is an important part of the ophthalmologist's work. It fulfils the ophthalmologist's duty to care for the patient and increases general road traffic safety. This assessment requires testing of visual acuity, visual field, mesopic vision and glare, motility, binocular vision and colour vision. Also the optical media and the fundus have to be examined. If an optical aid is necessary, it should be suitable for use in road traffic. In some cases restrictions are necessary, which should be noted in the driver's licence. Testing of visua

Prevalence of non-strabismic binocular vision anomalies and age-related changes in binocular vision among middle-aged and older adults: a systematic review.

BACKGROUND: With the global aging population, binocular vision anomalies in middle-aged and older adults have gained increasing attention. Non-Strabismic Binocular Vision Anomalies (NSBVAs) are particularly prevalent in this population, yet clinical research remains insufficient. To bridge this gap, we conducted a systematic review to evaluate NSBVAs prevalence and subtypes in middle-aged and older adults and analyze age-related changes in binocular vision. The aim was to clarify how aging affects binocular function and inform clinical management. METHODS: Studies were identified using PubMed

Line bisection in unilateral homonymous visual field defects.

The contralesional line bisection error in unilateral homonymous hemianopia is a frequent but neglected clinical phenomenon. Our knowledge about this bisection error is based on small samples of hemianopic patients. Moreover, horizontal line bisection has never been investigated in other unilateral visual field defects. The present study is the first to examine line bisection in a large, representative sample of patients with unilateral homonymous visual field defects. We investigated horizontal line bisection in 129 patients with left- or right-sided homonymous hemianopia (60.5%), upper and l

Impact of visual impairment on measures of cognitive function for children with congenital toxoplasmosis: implications for compensatory intervention strategies.

OBJECTIVES: The purpose of this work was to determine whether visual impairment caused by toxoplasmic chorioretinitis is associated with impaired performance of specific tasks on standardized tests of cognitive function. If so, then we worked to determine whether there are patterns in these difficulties that provide a logical basis for development of measures of cognitive function independent of visual impairment and compensatory intervention strategies to facilitate learning for such children. METHODS: Sixty-four children with congenital toxoplasmosis with intelligence quotient scores > or =

Dual Sensory Impairment: Impact of Central Vision Loss and Hearing Loss on Visual and Auditory Localization.

PURPOSE: In the United States, AMD is a leading cause of low vision that leads to central vision loss and has a high co-occurrence with hearing loss. The impact of central vision loss on the daily functioning of older individuals cannot be fully addressed without considering their hearing status. We investigated the impact of combined central vision loss and hearing loss on spatial localization, an ability critical for social interactions and navigation. METHODS: Sixteen older adults with central vision loss primarily due to AMD, with or without co-occurring hearing loss, completed a spatial

Improvement of Color Vision Following Posterior Cranial Vault Distraction for Crouzon Syndrome.

Crouzon syndrome (CS) is one of the craniosynostosis syndromes that leads to early fusion of cranial sutures and increased intracranial pressure. Intracranial hypertension is a serious complication that may lead to vision loss and cognitive impairment. Early detection and management are necessary to prevent complications. The authors present a patient with CS who underwent posterior cranial vault reconstruction with internal distraction after multiple episodes of headache and papilledema. The patient was unaware of any loss of color vision before the surgery; however, he noted an improvement i

The dichoptic contrast ordering test: A method for measuring the depth of binocular imbalance.

In binocular vision, the relative strength of the input from the two eyes can have significant functional impact. These inputs are typically balanced; however, in some conditions (e.g., amblyopia), one eye will dominate over the other. To quantify imbalances in binocular vision, we have developed the Dichoptic Contrast Ordering Test (DiCOT). Implemented on a tablet device, the program uses rankings of perceived contrast (of dichoptically presented stimuli) to find a scaling factor that balances the two eyes. We measured how physical interventions (applied to one eye) affect the DiCOT measureme

Overestimation of stereo thresholds by the TNO stereotest is not due to global stereopsis.

PURPOSE: It has been repeatedly shown that the TNO stereotest overestimates stereo threshold compared to other clinical stereotests. In the current study, we test whether this overestimation can be attributed to a distinction between 'global' (or 'cyclopean') and 'Локально' (feature or contour-based) stereopsis. METHODS: We compared stereo thresholds of a global (TNO) and a local clinical stereotest (Randot Circles). In addition, a global and a local psychophysical stereotest were added to the design. One hundred and forty-nine children between 4 and 16 years old were included in the study. RES

Duane retraction syndrome type 1 with Usher syndrome type 2: an unreported association.

Duane retraction syndrome is characterized by globe retraction and palpebral fissure narrowing on adduction, with restriction of abduction, adduction, or both. Usher syndrome type 2 consists of congenital bilateral sensorineural hearing loss and retinitis pigmentosa. The authors present a case with a yet unreported association between Duane retraction syndrome type 1 and Usher syndrome type 2.
