Effect of air pollution on athlete health and performance.

Unfavourable effects on the respiratory and the cardiovascular systems from short-term and long-term inhalation of air pollution are well documented. Exposure to freshly generated mixed combustion emissions such as those observed in proximity to roadways with high volumes of traffic and those from ice-resurfacing equipment are of particular concern. This is because there is a greater toxicity from freshly generated whole exhaust than from its component parts. The particles released from emissions are considered to cause oxidative damage and inflammation in the airways and the vascular system,

[New parameters "average flow" and "acceleration wave": comparison with current parameters in the flow volume curve].

AIM: The flow volume curve is an essential test method for diagnosis and treatment of the respiratory diseases. However, this curve depends on patient's continuous effort toward optimal expiration and it has been reported that differences in this effort may possibly result in error in flow speed. To overcome the potential error, we devised the "average flow" and the "acceleration wave" that comprehends the overall data of the expiration flow speed, and have done comparative analysis with the current parameters. METHODS: The average flow is derived by taking the integration from the beginning

Improvements in symptoms and quality of life following exercise training in older adults with moderate/severe persistent asthma.

BACKGROUND: Some individuals with moderate/severe persistent asthma develop irreversible airway obstruction. These individuals present with dyspnoea, exercise intolerance and impaired quality of life (QOL), all of which could potentially be alleviated with exercise training. OBJECTIVE: To investigate whether exercise training improves functional exercise capacity and QOL in middle-aged and older adults with fixed airway obstruction asthma (FAOA). METHODS: 35 subjects aged 67.8 ± 10.6 years, with FEV(1) 59 ± 16% of predicted, were randomised to a 6-week 'intervention' period of supervised exe

Rheumatoid lung disease.

Rheumatoid arthritis (RA) is a common, functionally disabling disease with genetic and environmental contributors. It occurs in approximately 1% of the population and adversely affects quality of life, functional status, and survival. Beyond its impact on the joints, pulmonary involvement occurs regularly and is responsible for a significant portion of the morbidity and mortality. Although pulmonary infection and/or drug toxicity are frequent complications, lung disease directly associated with the underlying RA is more common. The airways, vasculature, parenchyma, and pleura can all be involv

Long term effects of exposure to automobile exhaust on the pulmonary function of female adults in Tokyo, Japan.

AIMS: To investigate the chronic effects of air pollution caused mainly by automobiles in healthy adult females. METHODS: Respiratory symptoms were investigated in 5682 adult females who had lived in the Tokyo metropolitan area for three years or more in 1987; 733 of them were subjected to pulmonary function tests over eight years from 1987 to 1994. The subjects were divided into three groups by the level of air pollution they were exposed to during the study period. The concentrations of nitrogen dioxide and suspended particulate matter were the highest in group 1, and the lowest in group 3.

Brachial plexus neurotization with donor phrenic nerves and its effect on pulmonary function.

OBJECT: To examine possible side effects of neurotizations in which the phrenic nerve was used, pulmonary function was analyzed pre- and pos … perienced pulmonary problems postoperatively. Nonetheless, pulmonary functional parameters showed a vital capacity in percent of the predicted value of 9.8 +/- 6.3% (mean +/- standard deviation [SD]) in all patients examined, which was a significant reduction (p = 0.0002). In right-sided phrenic nerve transfers this reduction was significant, at 14.3 +/- 3.3% (m …

2 Years' experience with inspiratory muscle training in patients with neuromuscular disorders.

PURPOSE: The aim of our study was to assess the long-term effects of specific inspiratory muscle training (IMT) in patients with neuromuscular disorders (NMDs) who have various degrees of respiratory impairment. PATIENTS AND METHODS: Twenty-seven patients with NMDs (Duchenne's muscular dystrophy, 18 patients; spinal muscular atrophy, 9 patients) underwent 24 months of IMT. Patients were divided into three groups according to their vital capacity (VC) values. VC was measured as the parameter for the respiratory system involvement of the disease. Maximal inspiratory pressure (PImax) was assesse…

Changes in FVC during methacholine-induced bronchoconstriction in elderly patients with asthma: bronchial hyperresponsiveness and aging.

STUDY OBJECTIVE: We evaluated whether aging may produce changes in bronchial hyperresponsiveness, risk of enhanced bronchoconstriction, and changes of bronchoconstriction perception. SETTING: Each subject underwent a methacholine bronchial challenge. Methacholine challenge was stopped when one of the following conditions occurred: (1) plateau of bronchoconstriction; (2) decrease of FEV(1) > 40%; (3) FEV(1) drop below 1 L; or (4) excessive respiratory discomfort. Methacholine dose-response curves were plotted both for FVC and FEV(1). The provocative dose of methacholine causing a 20% decrease

Relationship between chronic hypercapnia and inspiratory-muscle weakness in myotonic dystrophy.

We studied 134 patients with Steinert's myotonic dystrophy (MD) in order to determine the prevalence of chronic hypercapnia, the level of muscle weakness and forced expiratory volume at which hypercapnic respiratory failure is likely to occur, and how clinical assessment might help predict hypercapnic respiratory failure. Subjects were divided into five classes with a muscular disability rating scale (MDRS): 0 = no clinical impairment (n = 9), I = minimal signs of impairment (n = 11), II = distal weakness (n = 41), III = moderate proximal weakness (n = 62), and IV = nonambulatory (n = 11). Motor neurone disease--a study of prevalence and disability.

We conducted a point prevalence study of motor neurone disease in the counties of South Glamorgan, Mid Glamorgan and Gwent, whose combined p … d (< 10 ml/s) in 67% (33/49), with three requiring feeding through a percutaneous gastrostomy tube. Vital capacity was < 70% of predicted value for age, sex and height in 67% (29/43). One patient was receiving domiciliary ventilation. These results give a quantitative indication of the dependency in a population of patients with motor neurone disease.

Bronchial rupture secondary to blunt chest trauma.

Bronchial injury due to blunt chest trauma is rare, and its rarity and the fact that it has two distinct modes of presentation may considerably delay diagnosis. Two recent cases illustrate the two main types of injury and presentation. In the first the rupture is intrapleural and air escapes into the pleural space; insertion of a chest drain leads to a continuous air leak. In the second type the rupture is largely extrapleural with little communication with the pleural cavity; initially symptoms may be mild or absent but complications may occur later.

Nasal positive pressure ventilation in patients with acute respiratory failure. Difficult and time-consuming procedure for nurses.

Intubation and mechanical ventilation are well-established techniques in the management of patients with acute respiratory failure; however, there are situations in which these procedures cannot be used safely for various reasons. A recently described noninvasive technique, nasal positive-pressure ventilation (NPPV), has been developed for home ventilation of certain patients with chronic ventilatory insufficiency. We hypothesized that NPPV could be used in selected patients in whom intubation and mechanical ventilation were clearly indicated, but not immediately possible, or even contraindica

Smoking cessation and acute airway response to ozone.

Pulmonary function and symptom responses of asymptomatic smokers were measured during chamber exposures to ozone or filtered air. Subjects were evaluated while habituated to smoking and after 6 mo of smoking abstinence. At the onset of treatment, subjects (n = 18) who had a history of smoking 33.4 +/- 15.6 packs/y and who had almost normal pulmonary function (forced vital capacity [FVC], forced expiratory volume in 1 s [FEV1.0], and mid-maximal flow [MMF] were greater than 82% of predicted) were unresponsive to ozone exposure (0.4 ppm for 2 h). A matched group of control smokers (n = 16) did n…

A study of the prevalence of ventilatory obstruction in textile workers exposed to cotton dust.

A study was conducted in a cotton spinning mill to assess the prevalence of respiratory impairment in 189 asymptomatic workers and 133 byssinotics drawn from various sections of the plant exposed to airborne cotton dust in the work environment. Eight-four healthy controls belonging to the same socioeconomic status and ethnic group having never been exposed to cotton dust or any other pollutant was also studied for the purpose of comparison. Of the 189 asymptomatic workers, 13 (6.8%) suffered from bronchial obstruction whereas only 2 (2.3%) among the control showed obstructive pulmonary impairm

The effect on airway function of inspired air conditions after isocapnic hyperventilation with dry air.

The magnitude of postexercise or posthyperventilation bronchoconstriction in patients with asthma is related to the temperature and the water content of the inspired air during the exercise or hyperventilation period. Recent studies have suggested that the inspired air conditions during recovery from exercise may also be important in determining the magnitude of postexercise airway narrowing. In the present study, normal subjects (n = 8) and patients with asthma (n = 12) were studied on separate days. On day 1 the subjects performed isocapnic hyperventilation with warm dry air and recovered br

The relation among pulmonary function, chest roentgenographic abnormalities, and smoking status in an asbestos-exposed cohort.

To clarify the clinical value of the International Labor Office (ILO) roentgenographic classification of pneumoconiosis in assessing asbestos-exposed persons, we determined the relationships among spirometric values, radiographic evidence of parenchymal pulmonary fibrosis and/or pleural thickening, and cigarette smoking history in 684 consecutively enrolled male plumbers and pipefitters participating in a larger cross-sectional prevalence study. The subjects were mainly marine pipefitters (35%), plumbers (24%), and steamfitters/welders (23%); 28% had never smoked, 39% were ex-smokers, and 33%
