Rapid resolution of delusional parasitosis in pellagra with niacin augmentation therapy.

Pellagra is associated with low levels of vitamin B3 (niacin) and/or tryptophan and often involves other other B vitamins. Since the time Gasper Casal first described the disease in 1972, it was observed that the patients with pellagra were all poor, subsisted mainly on maize, and rarely ate fresh meat. Subsequent occurrences have been in the form of epidemic outbreaks, consequent to either introduction to maize as a major food or increased consumption of other niacin-deficient diets like Jowar (Sorgum vulgare). The virtual disappearance of pellagra as an endemic health problem in recent years

Acquired palmoplantar keratoderma and immunobullous disease associated with antibodies to desmocollin 3.

We present a case of immunobullous disease with an impressive acquired palmoplantar keratoderma (PPK) and unique antigenicity. The palms of the patient showed hyperkeratotic ridges with a tripe pattern that decreased with the amelioration of the immunobullous condition. The histopathology of perilesional skin (blister) demonstrated eosinophilic spongiosis and suprabasal blistering as in pemphigus vulgaris. In palmar skin, acantholysis, intraepidermal pustules, papillomatosis and marked hyperkeratosis were observed. Direct and indirect immunofluorescence displayed intraepidermal intercellular I

[Reduced vision in Wernicke's syndrome with symptoms of nystagmus].

A 42-year-old man presented in the Department of Ophthalmology of Holstebro Central Hospital with a history of alcohol abuse, vomiting and weight loss. The initial symptoms of beriberi were stiffness of the legs and a picture of Wernicke's syndrome. He had bilateral loss of horizontal eccentric gaze holding and upbeat nystagmus, and visual acuity was reduced to counting fingers. Visual acuity was tested in the down-gaze position and increased to 0.3 because of reduced nystagmus. The patient's oculomotor function improved dramatically after treatment with thiamine. Wernicke's encephalopathy and

[Beriberi after bariatric surgery: not an unusual complication. Report of two cases and literature review].

The number of patients submitted to bariatric surgery to treat morbid obesity is increasing, therefore, some nutritional deficiencies, with which many physicians are no longer familiarized, are reappearing. Postoperatively, many nutritional disorders may occur, one of them is thiamine deficiency (beriberi). The thiamine and/or vitamin B12 deficiency can correspond to 40% of the neuropathy cases after bariatric surgery. Two patients with the clinic of peripheral neuropathy and Wernicke-Korsakoff syndrome will be reported. Some months after the surgery, they presented prostration, depression, me

[Bilateral optic neuropathy from methanol intoxication. A case report].

Methanol poisoning generally occurs after voluntary or accidental ingestion and can be responsible for blindness by optic neuropathy. We describe two cases of methanol poisoning who developed bilateral optic atrophy despite a treatment combining corticosteroids and vitamin B. Through these observations we recall the physiopathology as well as the clinical and therapeutic aspects of this type of intoxication.

Mitochondrial decay, a major cause of aging, can be delayed.

Mitochondrial decay due to oxidant byproducts is a principal underlying contributor to aging, including the degenerative diseases of aging such as brain degeneration [15,23,32]. The energy for basic metabolic processes comes from mitochondria, and their decay with age impairs cellular metabolism and leads to cellular decline. Our progress over the last decade in delaying the mitochondrial decay of aging is briefly reviewed.

[Neuritis nervi optici--results of an etiological inquiry and therapeutical results (author's transl)].

The authors report on the results of an etiological inquiry in 71 patients from 1970 to 1977 suffering from neuritis nervi optici. The study includes also the therapeutical results. The findings demonstrate that modern examination techniques did not bring a definite progress of the clarification of the etiological relation: Approximately 50% stay without causality. The identificated causes are on all fours with those found by Rönne (1930) except the diseases following the application of modern drugs, e. g. contraceptives. Therapeutically a combination of systemic and parabulbar application of

Role of vitamins and minerals in prevention and management of type 2 diabetes mellitus.

Vitamins and minerals play an important role in glucose metabolism, so understanding the impact of vitamin and mineral deficiencies and the potential utility of supplementation is relevant to the prevention and/or management of type 2 diabetes mellitus (DM). This review investigates current evidence for relationships between selected nutrients - vitamin B complex, antioxidants (vitamin A, C, E and carotenoids), calcium, vitamin D, vitamin K, magnesium, sodium, and potassium - and glucose metabolism. The investigation reveals current evidence is not strong enough for supplementation with minera…

Intestinal absorption in health and disease: micronutrients.

The main theme of this chapter concerns the precise biochemical mechanisms involved in stages up to, and including, gastrointestinal absorption of vitamins and certain selected minerals. Essential data regarding sequential events, immediately following absorption of these micronutrients, are also included. There is reference to water-soluble vitamins that are, in general, either coenzymes in various metabolic reactions or carriers of certain biochemical groupings. In contrast, fat-soluble vitamins frequently function as integral components of cell membranes; they, too, receive ample attention.

Vitamin B supplementation enhances the efficacy of non-steroidal anti-inflammatory drugs in patients with painful foot and ankle conditions: A multicenter, prospective, randomized controlled trial.

OBJECTIVE: The management of foot disorders often involves anti-inflammatory drugs for chronic inflammation and pain. Combining anti-inflammatory drugs with vitamin B complex may have a synergistic effect, but its efficacy in foot disorders remains unclear. This study aimed to investigate the effects of combining vitamin B complex with anti-inflammatory drugs in patients with foot disorders. METHODS: This study enrolled 201 patients aged ≥ 19 years with foot disorders of Achilles tendinitis, foot or ankle arthritis, Civinini-Morton syndrome (Morton's neuroma), or plantar fasciitis from four h…

Biomimetic thiyl radical formation from diphenyl disulfide with the low valent Ni(i) state of a cofactor F430 model.

Cofactor F430 is a nickel-containing hydrocorphinato complex that plays important roles in the enzymatic formation and oxidation of methane. In methanotrophic bacteria, F430-dependent methyl-coenzyme M reductase (MCR) catalyses the endergonic conversion of the heterodisulfide adduct of coenzymes M and B with methane to methyl-coenzyme M and coenzyme B. In a radical mechanism, the Ni(i)-induced formation of a transient thiyl radical of coenzyme B from the heterodisulfide has been proposed. Herein, we introduce a new semi-artificial Ni-complex derived from vitamin B12 as functional model of F430

The role of vitamins in dementia prevention and cognitive health: A comprehensive review.

Dementia is a progressive neurodegenerative disorder characterized by cognitive decline that interferes with daily functioning, particularly in the elderly. As one of the leading causes of disability among older adults, dementia remains a significant public health concern. Emerging evidence suggests that certain vitamins may play a crucial role in the prevention and management of cognitive impairment. This review explores the potential impact of key micronutrients specifically water-soluble vitamins such as vitamin C and B-complex (B1, B2, B3, B5, B6, B9, B12), and fat-soluble vitamins A, D, E

Myeloradiculoneuropathy due to vitamin B12 deficiency: an unusual clinical and radiological presentation.

A 42-year-old man from rural India presented with asymmetric progressive paraparesis mimicking compressive dorsal myelopathy, followed by distal upper limb, truncal and neck-flexor weakness, further complicated by acute urinary retention. His sensory deficits were marked by loss of joint position sense (JPS) and graded loss of vibration sense, along with a definite sensory level. Deep tendon jerks were hypo-to-areflexic, plantar was bilaterally extensor. He had become less attentive and occasionally failed to keep track with conversations. A syndromic diagnosis of myeloradiculoneuropathy with

Effects of maternal B12 supplementation on neurophysiological outcomes in children: a study protocol for an extended follow-up from a placebo randomised control trial in Bangalore, India.

INTRODUCTION: Vitamin B12 deficiency is highly prevalent in pregnant Indian women. Neuropsychological tests have shown an association between low maternal vitamin B12 status and poorer cognitive performances in the offspring, although findings from these studies have been inconsistent. Vitamin B12 has an important role in the formation of myelin which is important for the transmission speed of neural impulses and myelination in the central nervous system has been linked to cognition. Assessing neurophysiological measures using event-related potentials (ERPs) in children may provide additional
