A Window Into Vulnerability and Support: At-Risk Mothers' Perinatal Experiences and Perspectives on mHealth-Based Care.

AIM: To examine the perinatal experiences of at-risk mothers and their engagement with mobile-health-based care. DESIGN: A qualitative descriptive study. METHODS: One-to-one semi-structured interviews were conducted with 30 at-risk mothers, defined as those who were single, had low income, were at risk of depression, had adverse childhood experiences, gave birth to a baby with congenital disorders, or had a history of mental health conditions. Participants were purposively sampled at 6 months postpartum from a tertiary public hospital in Singapore between February and September 2024. Intervi

Improving viral load testing coverage among orphan and vulnerable children in Jinka Town, South Ethiopia: a quality improvement project implemented by the USAID FFHPCT activity team.

BACKGROUND: Orphan and vulnerable children (OVC) are at a heightened risk of poor health outcomes, particularly in areas with a high prevalence of HIV/AIDS. Viral load (VL) testing is a vital component of HIV care that enables early detection of treatment failure and improves health outcomes. During the first half of the US Agency for International Development Family Focused HIV Prevention, Care and Treatment activity implementation (October 2023-March 2024), OVC VL testing coverage in Jinka town was 89%. This shortfall affects the timely diagnosis and treatment adjustments for OVC, posing a c

Artificial intelligence: a useful tool in active tuberculosis screening among vulnerable groups in Romania - advantages and limitations.

INTRODUCTION: Despite advances in diagnostic technologies for tuberculosis (TB), global control of this disease requires improved technologies for active case finding in selected vulnerable populations. The integration of artificial intelligence (AI) into imaging modalities has been anticipated to assume a pivotal position in conjunction with traditional bacteriological diagnostic approaches, especially in the active diagnosis of vulnerable groups. METHODS: The study was conducted as a prospective investigation spanning from November 2019 to October 2023, in Romania's national TB screening pr…

Wastewater-based genomic surveillance of SARS-CoV-2 in vulnerable communities in Mumbai.

Background & objectives The global impact of COVID-19, with over 45 million cases and 533,300 deaths in India alone, necessitates effective surveillance methods. Traditional approaches face challenges in detecting pre-symptomatic and asymptomatic cases, prompting the exploration of wastewater-based epidemiology (WBE). This study focuses on Mumbai's vulnerable slums, aiming to assess the potential of WBE as an alternative surveillance method. Methods Genomic surveillance of SARS-CoV-2 was conducted in Mumbai's vulnerable settings (slums) for 11 months (August 2022 to June 2023). Wastewater samp

Supportive Care for Common Conditions in Small Vulnerable Newborns and Term Infants: The Evidence.

INTRODUCTION: Small vulnerable newborns (SVNs) are at an increased risk of early death and other morbidities. Essential interventions provided to SVN, and other high-risk newborns have been proven critical in improving their outcomes. We aimed to provide an update on the effectiveness and safety of these interventions in low- and middle-income countries (LMICs). METHOD: Following a comprehensive literature scope, we updated or reanalyzed LMIC-specific evidence for essential SVN care interventions. RESULTS: A total of 113 individual LMIC studies were identified. Most of them were of high risk

Telemedicine and Access to Elective Cholecystectomy for Socially Vulnerable Adults: A Pilot Randomized Clinical Trial.

IMPORTANCE: Socially vulnerable patients with symptomatic cholelithiasis are more likely to face barriers to accessing surgical care. This barrier to access can lead to delays in treatment, the need for emergent cholecystectomy, and worse outcomes. OBJECTIVES: To determine the effectiveness of telemedicine vs in-person surgical consultation on access to elective cholecystectomy in socially vulnerable populations and to evaluate the association of scheduling navigation with access to elective cholecystectomy in these populations. DESIGN, SETTING, AND PARTICIPANTS: This pilot randomized clinic…

CoViD-19 in Italy: homeless population needs protection.

Italy is one of the most affected countries by the new coronavirus (CoViD-19) pandemic. In the country, there are an estimated 49,000-52,000 homeless people. People experiencing homelessness are among the potentially most vulnerable groups to the CoViD-19. Despite this, in Italy there is a worrying delay in implementation of a national coordinated strategy to protect homeless people from the potentially devastating effects caused by CoViD-19. In order to contain the epidemic among the most vulnerable people, we propose a short operational agenda based on the field experience of the medical-hum

Disadvantaged neighborhoods, birth weight, and problem behavior in five- and six-year-old pre-school children: Evidence from a cohort born in Amsterdam.

RATIONALE: Low birth weight has been found to increase the problem behavior of children. Yet, little attention has been given to adequately account for the impact of the child's neighborhood on this relation. The residential neighborhood is a choice, based on factors that are usually not observed that may also influence birth weight and problem behavior. OBJECTIVE: Using a model that accounts for such endogeneity of both neighborhood choice and birth weight, we have analyzed behavioral problems in 4210 pre-school children between the ages of 5 and 6, birth weight, and neighborhood status, sim

Impact of COVID-19 on women and children and the need for a gendered approach in vaccine development.

The COVID-19 pandemic has imposed unprecedented health and socioeconomic challenges on public health, disrupting it on a global scale. Given that women and children are widely considered the most vulnerable in the times of emergency, whether in war or during a pandemic, the current pandemic has also severely disrupted access to reproductive and child health services. Despite this, data on the effect of the pandemic on pregnant women and newborns remain scarce, and gender-disaggregated indicators of mortality and morbidity are not available. In this context, we suggest the implementation of a g

FIGO Statement: Vaccination in pregnancy.

Pregnant women and their fetuses are among the vulnerable populations that can be severely affected by communicable diseases. As such, some vaccines such as the influenza and the Tdap (tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis) vaccines are strongly recommended in each pregnancy, with generally safe profiles. Other vaccines can be offered based on risk factors, and only when the benefits of receiving them outweigh the risks. Development of vaccines against group B streptococcus infection and respiratory syncytial virus infection are of great importance. In this paper, …

Views on the use of the term 'elder orphans': A qualitative study.

Older adults living alone in the community with no immediate family network or support are referred in the literature as 'elder orphans'. The grey literature has a growing emphasis on the importance of supporting this vulnerable group, particularly with health and social care. However, there is a dearth of empirical research on 'elder orphans', and definitions remained semantically complicated and unknown to the public, healthcare professionals and those being referred as elder orphans. This research explored the views of older adults on the use of the terminology 'elder orphans', and the impl

Leaving no one behind? Social inclusion of health insurance in low- and middle-income countries: a systematic review.

BACKGROUND: One way to achieve universal health coverage (UHC) in low- and middle-income countries (LMIC) is the implementation of health insurance schemes. A robust and up to date overview of empirical evidence assessing and substantiating health equity impact of health insurance schemes among specific vulnerable populations in LMICs beyond the more common parameters, such as income level, is lacking. We fill this gap by conducting a systematic review of how social inclusion affects access to equitable health financing arrangements in LMIC. METHODS: We searched 11 databases to identify peer-…

Effect of Structured Physical Activity on Overall Burden and Transitions Between States of Major Mobility Disability in Older Persons: Secondary Analysis of a Randomized Trial.

BACKGROUND: The total time a patient is disabled likely has a greater influence on his or her quality of life than the initial occurrence of disability alone. OBJECTIVE: To compare the effect of a long-term, structured physical activity program with that of a health education intervention on the proportion of patient assessments indicating major mobility disability (MMD) (that is, MMD burden) and on the risk for transitions into and out of MMD. DESIGN: Single-blinded, parallel-group, randomized trial. (ClinicalTrials.gov: NCT01072500). SETTING: 8 U.S. centers between February 2010 and Decem

Reducing Underserved Children's Exposure to Tobacco Smoke: A Randomized Counseling Trial With Maternal Smokers.

INTRODUCTION: Addressing maternal smoking and child tobacco smoke exposure is a public health priority. Standard care advice and self-help materials to help parents reduce child tobacco smoke exposure is not sufficient to promote change in underserved populations. We tested the efficacy of a behavioral counseling approach with underserved maternal smokers to reduce infant's and preschooler's tobacco smoke exposure. DESIGN: A two-arm randomized trial: enhanced behavior counseling (experimental) versus enhanced standard care (control). Assessment staff members were blinded. SETTING/PARTICIPANT

Changing homelessness services: revanchism, 'professionalisation' and resistance.

This paper argues that the increasing international salience of homelessness can be partially explained by reference to the revanchist thesis (involving processes of coerced exclusion and abjection), but the situation on the ground is more complex. It reports on interviews with 18 representatives of 11 homelessness service providers in one city in England. As Cloke et al. found, these providers tended to be either larger, more 'professional', 'insider' services or smaller, more 'amateur', 'outsider' services. However, this does not mean that the former were necessarily more revanchist and the
