Leiomyoma of the Foot: A Case Report and Literature Review.

Leiomyomas within the foot are rare occurrences that are difficult to diagnose clinically and radiographically. They are benign and tend to be slow growing, often with minimal or no pain. We present an unusual case of a worker's compensation patient who presented with a new-onset mass within his foot that was thought to be a fibroma, but was later discovered to be a fast-growing leiomyoma after surgical excision. We also present a review of the literature regarding leiomyomas within the foot and ankle.

The impact of occupational health service network and reporting system in Taiwan.

BACKGROUND: Underreporting occupational disease cases has been a long-standing problem in Taiwan, which hinders the progress in occupational health and safety. To address this problem, the government has founded the Network of Occupational Diseases and Injuries Service (NODIS) for occupational disease and injury services and established a new Internet-based reporting system. OBJECTIVES: The aims of this study are to analyze the possible influence of the NODIS, comprised of Center for Occupational Disease and Injury Services and their local network hospitals, on compensable occupational diseas

Prognostic factors for long-term work disability due to musculoskeletal disorders.

The objective of this study is to identify risk factors for permanent work disability (PWD) related to musculoskeletal disorders (MSDs). This is a secondary data analysis of a randomized controlled intervention study in Temporary Work Disability (TWD) due to MSDs. The association of PWD (claim submission and status recognition) with baseline clinical, sociodemographic, work-related administrative and occupational factors was analyzed by Cox proportional hazards models. Of 3,311 patients with TWD, 47 submitted a PWD claim, of whom 32 achieved PWD status. The main alleged causes of the PWD were

Lateral epicondylitis treatment by extensor carpi radialis fasciotomy and radial nerve decompression: is outcome influenced by the occupational disease compensation aspect?

INTRODUCTION: The etiology, treatment, and patient management in cases of chronic epicondylitis, within the legislation on occupational disease, remain highly controversial. HYPOTHESIS: Recognition as an occupational disease has a negative influence on the functional result of epicondylitis treated with aponeurotomy and neurolysis of the motor branch of the radial nerve. PATIENTS AND METHODS: Twenty-eight patients (30 cases of epicondylitis) were operated between January 2007 and January 2008. There were nine men and 19 women whose mean age was 46.1 years. A preoperative EMG found anomalies

[Paradigm change in the assessment of myeloid and lymphoid neoplasms associated with occupational benzene exposure].

Benzene-caused hematologic neoplasms may be recognized as an occupational disease (OD) according to the German ordinance on ODs. At present, the OD No. 1303 covers heterogeneous diseases and various chemical agents triggering these diseases. The members of the medical advisory board specializing in ODs within the Ministry of Employment and Social Affairs recently proposed excluding "diseases of the blood, the hematopoietic and lymphatic system caused by benzene" from OD No. 1303 and classifying them as a separate OD. Benzene is generally acknowledged as a cause of acute myeloid leukemia, prove

Results of isolated Lisfranc injuries and the effect of compensation claims.

The results of treatment of Lisfranc injuries are often unsatisfactory. This retrospective study investigated 46 patients with isolated Lisfranc injuries at a minimum of two years after surgery. Thirteen patients had a poor outcome and had to change employment, or were unable to find work as a result of this injury. The presence of a compensation claim (p = 0.02) and a delay in diagnosis of more than six months were associated with a poor outcome (p = 0.01). There was no association between poor functional outcome and age, gender, mechanism of injury or previous occupation. This study may have

Managing employee health problems--optimal use of independent medical evaluations.

The purpose of an IME is to obtain an impartial, objective evaluation of an employees health condition. Based on the evaluation, the examiner is asked to render an opinion to answer questions set forth in written communication to the examiner. The opinion, which is grounded in the examiners professional education, experience, and judgment, becomes a legal instrument. Occupational health nurses must be cognizant of the ramifications of an IME and the communications surrounding this examination. In addition, many factors can affect the quality of an IME. Occupational health nurses should be awar

Treating the injured worker in Brazil. The South American experience.

Despite appropriate laws to protect workers, the health conditions of Brazilian workers reflect the social and economic reality in Brazil. A statistical analysis of health actions in Brazil shows serious distortions caused by the immense discrepancy between the poverty of the Northern and Northeastern states and the richness, although concentrated, of the great Southern and Southeastern cities of the country. Valid quantitative measures of health care problems of workers are confounded by the presence of a large mass of informal workers within the country, which may not be included in standard

Occupational asthma. Practical points for diagnosis and management.

Asthma is a common chronic illness characterized by episodes of reversible airflow obstruction. A cornerstone of asthma management is identifying and avoiding agents that cause bronchospasm. The workplace is an important potential source of respirable exposures that can cause or trigger asthma. Identification of an occupational factor in asthma is important: early diagnosis and removal of the worker from the exposure is associated with improved prognosis; the diagnosis of occupational asthma may lead to compensation for work-related impairment and disability; and the diagnosis of occupational

Congenital spine deformity, congenital stenosis, diastematomyelia, and tight filum terminale in a workmen's compensation patient: a case report.

STUDY DESIGN: This case report concerns a patient injured at work who was denied adequate evaluation and treatment by a Workman's Compensation Commission, resulting in nearly complete paraplegia. OBJECTIVES: For a patient with a congenital spine deformity, a diastematomyelia, a tight filum terminale, and a congenital stenosis, denial of magnetic resonance imaging evaluation and appropriate surgery cannot be justified. SUMMARY OF BACKGROUND DATA: This 35-year-old man injured his knee and back in a fall at work but was able to work for 1 week. Progressive neurologic deterioration was documente

Occupational injury. Fact, fantasy, or fraud?

The preceding describes some of the confusion which abounds in practically all areas of clinical medicine and particularly in occupational injuries. In essence, this confusion devolves to a primary failure on our part as clinicians; the failure to differentiate fact from fantasy. In the clinical arena, this is the failure to differentiate between that which is clinical pathology in the peripheral neuromusculoskeletal system and that which is no less real for the patient, a fantasy arising from image-driven, adaptively-initiated activation of specific neuronal groups in the somatosensory or som

[Intervertebral disk-induced diseases (BK Nr. 2108, 2109, 2110). The so-called exacerbation and assessment of work disability].

Two problems are discussed from a multitude of problems arising with the assessment of the "new" occupational diseases classified under the heading "Vertebral Column" which are of essential importance in determining the amount of compensation payment under German statutory health insurance legislation. One problem concerns differentiation of damage from already existing damage or dividing the total disease into preliminary i.e. already existing damage on the one hand and worsening or exacerbation on the other. Pathogenesis of disk-related disease does not usually allow splitting the disease in

[Taste disorders in forensic medicine].

Taste disorders can a rise from lesions of peripheral receptors, taste control pathways or cortical area involvement. Among peripheral lesions, trauma of the tongue and oropharynx are the most common. Iatrogenic lesions of facial and glossopharingeal nerves are very important in Forensic Medicine, while there are different opinions about taste alterations due to head injuries; hypogeusia associated to smell disorders are found in 0.4-0.5% of patient after head trauma with good prognosis (90% healing) while qualitative disorders are more common (30%). The Authors describe clinical methodologies

Disability, direct cost, and payment issues in injuries involving woodworking and wood-related construction.

Treatment cost and payment patterns and disability for work, home, and recreation activities were examined for 495 consecutive persons seen as primary care patients over one year in Northern Vermont with injuries associated with woodworking, wood related construction, and home repair activities and materials. These primary care patients were from a larger sample of 601 persons with such injuries who received either primary or tertiary care. Mean total charges were $530 and $342 respectively for work and nonwork related injuries, with highest costs for back and arm injuries, and injuries involv

Compensation, pain complaints, and psychological disturbance.

This study compared 47 patients with chronic low back pain who were involved in personal injury litigation with 33 patients not seeking compensation who were also complaining of low back pain. Pain was assessed using a visual analogue scale and the adjectival check-list of the McGill Pain Questionnaire. Psychological state was assessed using the Zung Depression Scale, the State-Trait Anxiety Inventory, the Eysenck Personality Inventory and the Hostility and Direction of Hostility Questionnaire. The group involved in litigation contained significantly more males, and the duration of pain was si

Working in interventional cardiology laboratories: The perceived impact of radiation exposure as a health and gender hazard. A NEXT generation ANMCO initiative.

BACKGROUND: Gender-related discrepancies in personal and professional life have been reported among radio-exposed workers. We assessed this topic among cardiac catheterization workers in Italy, with a focus on gender and working position. METHODS: Radio-exposed workers affiliated with the Italian Association of Hospital Cardiologists were invited to answer an online survey, which included 41 questions formatted as multiple choice. RESULTS: Overall, 237 workers responded. The proportion of males was significantly higher than that of females in the population aged >50 years. A greater portion
