Understanding Barriers to Uptake of TB Preventive Treatment Among People Living with HIV in Zimbabwe: A Qualitative Assessment of Healthcare Workers' Perspectives.

People living with HIV (PLHIV) are at an increased risk of progressing from Mycobacterium tuberculosis (TB) infection to TB disease. Tuberculosis preventive treatment (TPT) is recommended for PLHIV; however, its uptake remains suboptimal. In Zimbabwe, three months of weekly rifapentine and isoniazid (3HP) and six months of daily isoniazid (6H) are the most commonly prescribed TPT regimens. We explored barriers to TPT uptake among PLHIV from the perspective of healthcare workers (HCWs) in Zimbabwe. Facility nurses who had been implementing TPT for at least six months were invited to participate…

Creating complete life histories of individual female tsetse (Glossina spp) to study the effects of meteorological conditions on fly size in Zimbabwe.

Using novel methodologies and ovarian dissection, we constructed life histories for ~90,000 female Glossina pallidipes and G. m. morsitans collected in Zimbabwe (1988-1999). Temperature-dependent development rates were used to estimate, for each fly, the dates spent as an oocyte, egg, larva, pupa - and adult in each ovarian category. This enabled analysis of relationships between wing and egg lengths of field-captured adults - and mean environmental conditions prevailing when these lengths were being determined, during oogenesis for eggs, and oogenesis plus pregnancy for wings. Lengths all osc…

Reduction in overtreatment of gonorrhoea and chlamydia through point-of-care testing compared with syndromic management for vaginal discharge: a modelling study for Zimbabwe.

BACKGROUND: Zimbabwe's national guidelines for sexually transmitted infection (STI) management recommend that high-risk women presenting with vaginal discharge syndrome (VDS) are prescribed antibiotics for gonorrhoea (Neisseria gonorrhoeae (NG)), chlamydia (Chlamydia trachomatis (CT)), trichomoniasis (Trichomonas vaginalis (TV)) and bacterial vaginosis (BV). The performance of this approach depends on its clinical interpretation and implementation. Here, we investigate the potential relative impact of an NG/CT/TV point-of-care (POC) test on undertreatment, overtreatment and disease burden in t…

Developing and evaluating a community-driven intervention to promote uptake of HIV and contraception services among students enrolled in colleges and universities in Zimbabwe.

INTRODUCTION: There is a growing appreciation that community-led interventions are key to sustaining the HIV response and achieving HIV prev … promotion/distribution of HIVST, PEP, EC and condoms. The agreed intervention was piloted in three Zimbabwean colleges/universities from December 2023 to February 2024. Student peers distributed HIVST and condoms directly, and vouchers for PEP and EC that were redeemed at college/nearby clinics. During co-development, students strongly preferred peer dist …

Higher loss of livelihood and impoverishment in households affected by tuberculosis compared to non-tuberculosis affected households in Zimbabwe: a cross-sectional study.

INTRODUCTION: Tuberculosis (TB) disproportionally affects poor people, leading to income and non-income losses. Measures of socioeconomic impact of TB, e.g. impoverishment and patient costs are inadequate to capture non-income losses. We applied impoverishment and a multidimensional measure on TB and non-TB affected households in Zimbabwe. METHODS: We conducted a cross-sectional study in 270 households: 90 non-TB; 90 drug-susceptible TB (DS-TB), 90 drug-resistant TB (DR-TB) during the COVID-19 pandemic (2020-2021). Household data included ownership of assets, number of household members, inco…

Early implementation learnings on acceptability and feasibility of "V": a multi-level PrEP intervention designed with and for adolescent girls and young women in Zimbabwe.

INTRODUCTION: Adolescent girls and young women (AGYW) remain disproportionately affected by HIV in Zimbabwe. Several HIV prevention options are available, including oral tenofovir-based pre-exposure prophylaxis (PrEP), however AGYW face unique barriers to PrEP uptake and continuation and novel approaches are therefore needed to empower AGYW to use PrEP. The objective of this study was to characterize early learnings from implementing a multi-level intervention consisting of fashionable branding (including a "V Starter Kit"), service integration, and peer education and support throughout a youn…

Integrating 3HP-based tuberculosis preventive treatment into Zimbabwe's Fast Track HIV treatment model: experiences from a pilot study.

INTRODUCTION: Tuberculosis (TB) causes one-third of HIV-related deaths worldwide, making TB preventive treatment (TPT) a critical element of HIV programmes. Approximately 16% of people living with HIV (PLHIV) on antiretrovirals in Zimbabwe are enrolled in the Fast Track (FT) differentiated service delivery model, which includes multi-month dispensing of antiretrovirals and quarterly health facility (HF) visits. We assessed the feasibility and acceptability of utilizing FT to deliver 3HP (3 months of once-weekly rifapentine and isoniazid) for TPT by aligning TPT and HIV visits, providing multi-…

'I was trying to get there, but I couldn't': social norms, vulnerability and lived experiences of home delivery in Mashonaland Central Province, Zimbabwe.

Increasing facility-based delivery rates is pivotal to reach Sustainable Development Goals to improve skilled attendance at birth and reduce maternal and neonatal mortality in low- and middle-income countries (LMICs). The translation of global health initiatives into national policy and programmes has increased facility-based deliveries in LMICs, but little is known about the impact of such policies on social norms from the perspective of women who continue to deliver at home. This qualitative study explores the reasons for and experiences of home delivery among women living in rural Zimbabwe.…

Knowledge and perceptions of schistosomiasis, a water-borne disease, in two semi-arid rural areas of South Africa (Ndumo) and Zimbabwe (Ntalale).

Schistosomiasis is a serious water-borne disease of public health importance in southern Africa and is characterised by high morbidity and negative socio-economic repercussions. Limited knowledge on the mode of transmission and treatment contribute towards increase in the risk of infection. This study assessed community knowledge levels, perceptions and the co-constructed realities of social actors with regards to schistosomiasis in rural resource-poor communities of Ndumo in South Africa and Ntalale in Zimbabwe. We hypothesised that there was association between community knowledge level on s…

Accuracy of different diagnostic techniques for Schistosoma haematobium to estimate treatment needs in Zimbabwe: Application of a hierarchical Bayesian egg count model.

BACKGROUND: Treatment needs for Schistosoma haematobium are commonly evaluated using urine filtration with detection of parasite eggs under a microscope. A common symptom of S. haematobium is hematuria, the passing of blood in urine. Hence, the use of hematuria-based diagnostic techniques as a proxy for the assessment of treatment needs has been considered. This study evaluates data from a national survey in Zimbabwe, where three hematuria-based diagnostic techniques, that is microhematuria, macrohematuria, and an anamnestic questionnaire pertaining to self-reported blood in urine, have been i…
