Autoimmune polyglandular syndromes are combinations of various endocrine and nonendocrine autoimmune diseases, as well as the presence of elevated organ-specific antibody titers. We present a clinical case of a 41-year-old pregnant patient with type 2 autoimmune polyglandular syndrome, combining Addison's disease, Hashimoto's thyroiditis and hypogonadism. The pregnancy was achieved after the use of assisted reproductive technology. During the pregnancy the patient was strictly monitored. Glucocorticoid and mineralocor-ticoid replacement therapy was adjusted according to the electrolyte profile

Six families were studied which included 11 members with dermatitis herpetiformis (DH) and three with coeliac disease (CD). Proximal jejunal biopsies performed on 20 relatives revealed villous atrophy in eight. Of these eight, two, both siblings of patients with DH, had a history of juvenile CD. Determinations of histocampatibility (HLA) antigens showed that HLS-B8 occurred in all six families although two patients with DH and one relative with a history of juvenile CD lacked this antigen. In one family the haplotype A1,B8 was associated with DH, villous atrophy, juvenile diabetes and Addison'

CONTEXT: Lipoid congenital adrenal hyperplasia is a severe disorder of adrenal and gonadal steroidogenesis caused by mutations in the steroi … order, nonclassic lipoid congenital adrenal hyperplasia, and represent a new cause of nonautoimmune Addison disease (primary adrenal failure).

Primary adrenal insufficiency (PAI) consists in a lack of adrenal hormones, and particularly of cortisol and aldosterone. It typically presents with fatigue, weakness, loss of appetite, increased thirst and skin hyperpigmentation. While most cases stem from an autoimmune etiology, rare instances of PAI have been attributed to infection, adrenal hemorrhage and medication disrupting steroidogenesis pathways. This report presents two patients with hematologic malignancies who developed primary glucocorticoid deficiency due to posaconazole. Both received allogeneic stem-cell transplantation and us

Primary adrenal insufficiency (PAI) is a critical condition that requires prompt diagnosis and initiation of treatment. Diagnosis can be challenging due to various underlying causes, including defects in adrenal gland development, resistance to adrenocorticotropic hormone, autoimmune causes, and metabolic problems. A specific diagnosis is essential for developing a treatment plan and identifying other possible accompanying pathologies. Biochemical studies, genetic analyses, and imaging techniques are helpful in establishing a specific diagnosis. This evidence-based guideline includes the speci

The adrenal glands are small endocrine glands located on top of each kidney, producing hormones regulating important functions in our body like metabolism and stress. There are several underlying causes for adrenal insufficiency, where an autoimmune attack by the immune system is the most common cause. A number of genes are known to confer early onset adrenal disease in monogenic inheritance patterns, usually genetic encoding enzymes of adrenal steroidogenesis. Autoimmune primary adrenal insufficiency is usually a polygenic disease where our information recently has increased due to genome ass

An adrenal crisis (Addisonian crisis) is an acute life-threatening complication of adrenal insufficiency. It occurs when hydrocortisone demand is not met by supplementation in the context of an infection - often gastrointestinal, fever, trauma, acute psychological or physical stress. Symptoms of weakness, nausea, muscle/joint pain and drowsiness may develop out of robust health within few hours. If overlooked, treated too late, with insufficient dose or route of application, there exists a considerable risk of mortality. Treatment consists of immediate parenteral hydrocortisone (100 mg bolus)

The association of primary adrenal insufficiency with antiphospholipid antibodies is usually reported in the context of adrenal thrombosis or hemorrhage. We describe a 35-year-old woman who developed a primary antiphospholipid syndrome (spontaneous abortion, thrombocytopenia, and cerebrovascular occlusion) in association with primary adrenal insufficiency without evidence of suprarenal hemorrhage or thrombosis.

OBJECTIVE: To assess the ability of desmopressin administration to stimulate ACTH/cortisol secretion in patients with Cushing's disease, either before or after surgery, and in patients with other states characterized by ACTH hypersecretion, and to compare the results with those obtained after CRH testing.
DESIGN AND SUBJECTS: Plasma ACTH and serum cortisol levels were evaluated after the administration of desmopressin (10 micrograms i.v.), CRH (1 microgram/kg i.v.) and saline on different days in 17 patients with Cushing's disease, 1 with occult ectopic ACTH syndrome, 5 with Addison's disease

Primary adrenal insufficiency is rare but serious; it puts patients at risk of acute decompensation and adrenal crisis due to insufficient cortisol and aldosterone production. Further, its diagnosis is often delayed, or it is mistaken for secondary adrenal insufficiency, which can have life-threatening consequences. Early recognition and appropriate treatment can greatly improve patient outcomes and quality of life.

During the beta 1-24 corticotropin stimulation test, it appears the increase of Aldosterone Blood level is more important (170,91% +/- 35,89 (S. E.)) and more significant (alpha less than 0,001) than for cortisol (+/- 54,47% +/- 9,45 (S. E.) - alpha less than 0,10). The authors suggest aldosterone assays have to be done in order to obtain a better estimation on adrenal response to synthetic A.C.T.H.
