Clinical correlations to distinguish severe from milder forms of obstructive sleep apnoea syndrome using overnight oximetry for prioritising adenotonsillectomy in a limited-resource setting.

BACKGROUND: In resource-poor settings with limited surgical services, it is essential to identify and prioritise children with severe and very severe obstructive sleep apnoea syndrome (OSAS) to expedite surgery. McGill's Oximetry Score (MOS) has been validated against polysomnography for OSAS and is affordable and easy to use. AIMS: The aim of this study was to assess the correlation of tonsillar size and clinical symptoms with MOS grade 3 or 4, to identify who requires overnight oximetry and who to prioritise for adenotonsillectomy. METHODS: Children with suspected OSAS were recruited from

Pre- Versus Post-Tonsillectomy Intraoperative Bupivacaine Injection in the Pediatric Population: An Age- and Surgical Indication-Stratified Analysis.

OBJECTIVES: Postoperative respiratory depression is of concern in children undergoing adenotonsillectomy receiving postoperative opioids and may be mitigated with intraoperative bupivacaine. This study aims to compare the impact of bupivacaine on postoperative pain and sedation in various pediatric age and surgical indication subgroups. METHODS: This is a case series with chart review of 181 patients <18 years old undergoing adenotonsillectomy at a tertiary care center (2013-2016). Postoperative outcomes were compared between those who received intraoperative bupivacaine before (pre-tonsillec

Hot tonsillectomy for paediatric obstructive sleep apnoea.

Obstructive sleep apnoea is a common presentation in paediatric ear, nose and tongue (ENT) outpatients. The use of sleep studies is controversial however once a diagnosis has been made, frequently treatment is surgery. Should these patients be operated on as urgent cases? A 5-year-old boy was admitted under the paediatric team with difficultly breathing and desaturations to 77%. The patient had previously been seen by ENT as an outpatient with an 8-month history of obstructive sleep apnoea and was listed for an adenotonsillectomy with the standard waiting time. During this admission he had an

Posterior plica perforation: rare complication of adenotonsillectomy.

Tonsillectomy is one of the most common operations performed by otolaryngologists. Some extraordinary complications of this surgery are massive bleeding, taste perception disorders due to glossopharyngeal nerve damage, hematoma of the mouth floor, jugular vein thrombosis, Grisel syndrome, cervical osteomyelitis, nasopharyngeal stenosis, pulmonary edema, infection, and lingual artery pseudoaneurysm. In this clinical report, an 8-year-old girl presented with a posterior plica perforation after adenotonsillectomy performed 1 week previously and this complication has led to velopharyngeal insuffic

Current Belgian adenotonsillectomy practice: a survey among Belgian ENT specialists.

OBJECTIVES: Tonsillectomies and adenotonsillectomies (TE and ATE) are among the most frequently performed operations in the western world. Despite the lack of generally accepted clinical guidelines about indications, the beneficial role of the procedure has been established. The aim of this study was to identify current practice relating to indications for tonsillectomies and adenotonsillectomies among Belgian ENT specialists. METHODS: An anonymous questionnaire was sent to all Belgian ENT specialists. Part I of the questionnaire included questions about the ENT specialist himself/herself and

An 8-year-old girl with a recurrence of obstructive sleep apnea syndrome caused by hypertrophy of tubal tonsils 4 years after adenotonsillectomy.

Obstructive sleep apnea syndrome affects 1% to 2% of children. It is caused mainly by upper airway obstruction and manifests as snoring and sleep disturbance. Adenotonsillectomy can improve quality of life because airway obstruction occurs when both tonsils and adenoids are enlarged. We describe an 8-year-old girl with a recurrence of obstructive sleep apnea syndrome caused by hypertrophy of the tubal tonsils 4 years after adenotonsillectomy. The findings from this case highlight the importance of 1) identifying hypertrophy of the residual adenoid and compensatory hypertrophy of the tubal tons

Extra-esophageal acid reflux induced adenotonsillar hyperplasia: case report and literature review.

The etiology of adenotonsillar hyperplasia is not well understood. A 3-year-old child presented with obstructive sleep apnea believed to be secondary to enlarged tonsils and adenoids. Subglottic stenosis was encountered during intubation; the adenotonsillectomy was cancelled. Severe extra-esophageal reflux was identified and treated. At follow-up endoscopy 3 weeks later, the tonsils and adenoids were no longer enlarged or obstructing the airway. The role of extra-esophageal reflux in the pathogenesis adenotonsillar hyperplasia is discussed.

Relapsing infective-phlogistic pathology of Waldeyer's ring and its relationship with secretory otitis media.

In our previous studies on the physiopathology of the rhinopharyngeal-tubal unit, we found and reported an unmistakable correspondence between rhinopathy and otitis media. However, we have often asked ourselves how and why severe cases of otitis media were often present in subjects without an evident obstructive naso-tubal pathology. With research that we have already published, we have been able to reveal how alteration of the ventilatory function of the tube is not the only factor that provokes affections of the middle ear. In our opinion, the phlogistic event is the main cause of otitis. Ou

[On the physiopathologic reactions of pharyngeal tonsil from the clinical standpoint (author's transl)].

On the basis of personal experiences the authors isolate two forms of pharyngeal tonsil hypertrophy: a physiological and a pathological one. The first one corresponds to the requirements of the organism which is not completely mature immunologically, it may be quite considerable but causes no disturbances in breathing through the nose or in the function of Eustachian tubes. The pathological hypertrophy of pharyngeal tonsil is due to a parenchymal inflammation caused by viruses with a typical pathogenesis. The authors describe six diagnostic criterias of the pathological hypertrophy of pharynge

Tonsillectomy for periodic fever, aphthous stomatitis, pharyngitis and cervical adenitis syndrome (PFAPA).

BACKGROUND: Periodic fever, aphthous stomatitis, pharyngitis and cervical adenitis (PFAPA) syndrome is a rare clinical syndrome of unknown cause usually identified in children. Tonsillectomy is considered a potential treatment option for this syndrome. This is an update of a Cochrane review first published in 2010. OBJECTIVES: To assess the effectiveness and safety of tonsillectomy (with or without adenoidectomy) in children with PFAPA. SEARCH METHODS: We searched the Cochrane Ear, Nose and Throat Disorders Group Trials Register; the Cochrane Central Register of Controlled Trials (CENTRAL); …

Choanal Narrowing in Children With Down Syndrome: A Retrospective Case Series.

A retrospective case series was conducted on pediatric patients with Down syndrome who underwent choanal narrowing repair at a tertiary pediatric center between May 2017 and July 2024. Of the 84 patients who underwent repair, 13 children with Down syndrome (mean age: 2.54 years) were identified, representing 15.5% of the cohort. All had choanal narrowing (30%-80% stenosis, mean: 62%) and underwent concurrent airway procedures including adenoidectomy (n = 9), turbinate reduction (n = 7), and adenotonsillectomy (n = 3). Caregivers reported improvement in nasal breathing and obstructive sleep apn…
