Periocular contact dermatitis with use of topical voriconazole 1% in mycotic keratitis.

We present two cases of culture-proven fungal keratitis on natamycin treatment which developed periocular erythema, oedema, burning sensation and pruritus within 48 hours of the addition of topical voriconazole. On clinical examination, periocular erythema with induration was noted. A diagnosis of orbital cellulitis was suspected, but the absence of pain and tenderness refuted the diagnosis on clinical grounds. A dermatology consultation was sought, and a diagnosis of periocular contact dermatitis with voriconazole was made. A skin patch test was performed with the same medication; however, it

Assessment of corneal and conjunctival metaplasia by impression cytology during the treatment of canine keratoconjunctivitis sicca.

The purpose of this study was to assess the condition of cells in the conjunctiva and corneal epithelium prior to and during cyclosporine- or tacrolimusbased treatment of keratoconjunctivitis sicca (KCS). The study was performed on 40 dogs with KCS. The dogs were divided into two groups of 20 animals each. In Group I, 0.75% cyclosporine eye drops were administered three times a day, while in Group II 0.02% tacrolimus eye drops were administered twice daily. Additionally, each group was subdivided into three subgroups based on the results of the Schirmer I tear test (STT I). Evaluation of cellu

Brimonidine and brinzolamide for treating glaucoma and ocular hypertension; a safety evaluation.

Brimonidine tartrate and brinzolamide eye drops are often used as third and fourth line treatment options to reduce intraocular pressure (IOP) in the management of glaucoma and ocular hypertension. Better tolerated, more effective topical agents requiring once daily instillation including prostaglandin analogues and beta-blockers usually are preferred as initial therapy, unless there are contraindications. Brimonidine and brinzolamide are often required owing to progressive glaucoma or intolerances to or ineffectiveness of front-line agents. Areas covered: We review the safety of formulations

Topical beta-blocker treatment for migraine.

Beta-blockers are a well-known prophylactic treatment for migraine; however, treatment by the ocular route has not been widely considered. This case illustrates the resolution of a visual field defect associated with migraine and improvement of symptoms possibly due to administration of a topical beta-blocker. This novel method of treatment especially when visual field defects are present may have a place in the management of migraine.

A primer on topical antibiotics for the skin and eyes.

A thorough knowledge of all topical antibiotics treatment options is important as more pathogens become resistant to standard therapies. As resistance to mupirocin increases, it seems that triple antibiotic ointments and retapamulin (the newest FDA-approved agent) will have greater roles in treating impetigo and skin infections. Awareness of all these agents provides a dermatologist additional tools with which to treat skin infections. As a wide variety of topical antibiotics, combination antibiotics, and combination anti-inflammatory preparations used by ophthalmologists are not part of the t

The early efficacy of topical levocabastine in patients with allergic conjunctivitis.

BACKGROUND: We investigated the early efficacy of topical levocabastine, an H(1) histamine-receptor antagonist, in improving the clinical symptoms of allergic conjunctivitis. METHODS: Thirty-six patients with allergic conjunctivitis were enrolled. One drop of levocabastine was instilled in one eye and one drop of artificial tears in the contralateral eye. Clinical examinations were performed before, and 15 and 30 minutes after instillation. Symptoms of itching and signs of injection were assessed at each time point. RESULTS: Both levocabastine and artificial tears resulted in a statistically

Systemic side effects of ophthalmic drops.

Topical application of ocular drugs may cause serious adverse ocular or systemic side effects. Children are at greater risk for systemic side effects because ocular dosing is not weight-adjusted, and infants are especially vulnerable as a consequence of the inability to efficiently metabolize the drug, and/or an immature blood-brain barrier. Four pediatric patients are reported in whom treatment with topical brimonidine, a selective alpha-2 adrenoceptor agonist, indicated for the treatment of glaucoma, produced somnolence. Anti-glaucoma eyedrops that are not approved for use in children are st

Topical application of integrin antagonists inhibits proliferative retinopathy.

The expression of alphav-integrins is highly selective for angiogenic endothelial cells; ligation inhibition by cyclic RGD peptides prevents pathological neovascularization in tumor or retinopathy models to a large extent. We have previously demonstrated that proliferative retinopathy in a mouse model of retinopathy of prematurity (ROP model) can be reduced by more than 70%. To minimize systemic side effects and unwanted interference with responsive angiogenesis, we investigated topical application of cyclic RGD-peptides. In preliminary experiments, we could exclude any inhibiting effects of t

Iris cyst associated with topical administration of latanoprost.

PURPOSE: To report an adverse side effect associated with topical latanoprost usage. METHOD: Case report. A 76-year-old woman with primary open-angle glaucoma developed an iris cyst 5 weeks after beginning treatment with latanoprost. Clinical examinations and slit-lamp photographs were performed. RESULTS: Latanoprost was discontinued. Periodic examinations disclosed that the iris cyst gradually diminished and finally disappeared within 3 weeks. CONCLUSION: The formation of an iris cyst is a possible complication of topical latanoprost therapy.

Ocular hypotension induced by topical dopaminergic drugs and phosphodiesterase inhibitors.

The aim of this work was to investigate the ocular hypotensive activity of some topically administered cAMP-phosphodiesterase inhibitors alone and in combination with dopaminergic compounds. Experiments were performed with ocular normotensive rabbits and during transitory induced ocular hyper- or hypotension. An ocular hypotensive effect was observed after instillation of aminophylline, dyphylline, pentoxifylline, caffeine, and iso-caffeine, but not following topical hydroxypropyl-1,3-dimethylxanthine. Dopaminergic compounds were also studied in order to be combined with phosphodiesterase inhi

Double-masked comparative study of UF-021 and timolol ophthalmic solutions in patients with primary open-angle glaucoma or ocular hypertension.

A double-masked comparative clinical study of UF-021 (Rescula), a unique prostaglandin-related compound, was carried out at 18 centers with timolol maleate 0.5% ophthalmic solution as an active reference drug. After a wash-out period, UF-021 (0.12%) or timolol (0.5%) was given topically twice a day for 12 weeks to 158 patients with primary open-angle glaucoma or ocular hypertension, in a randomized double-masked manner. Both groups showed a significant reduction in intraocular pressure from the second week to the end of the study. In overall improvement rating, 91.4% of the cases (64/70) in th

Multiple-drop study of topically applied 1% delta 9-tetrahydrocannabinol in human eyes.

Twenty-eight male volunteers were given either 1% delta 9-tetrahydrocannabinol (delta 9-THC) or vehicle alone (light mineral oil). They used the medication four times per day for one week. Five volunteers, four of which used vehicle alone, discontinued the study because of burning sensation and lid swelling. In the 23 volunteers who completed the study, there was no difference in intraocular pressure between eyes treated with 1% delta 9-THC and controls.

Experimental stroma herpes simplex keratitis. Influence of treatment with topical bromovinyldeoxyuridine and trifluridine.

Two compounds, bromovinyldeoxyuridine ([E]-5-[2-bromovinyl]-2'-deoxyuridine) and trifluridine (5-trifluoromethyl-2' deoxyuridine) were compared for their efficacy in the topical treatment of experimental stroma herpetic keratitis produced by the injection of live herpes simplex virus, type 1, into the corneal stroma of rabbits. The trifluridine was used as 1% eyedrops, whereas bromovinyldeoxyuridine was used as either 0.1% or 0.5% eyedrops. All three treatment regimens caused a substantial healing of stroma disease in comparison with placebo treatment. The bromovinyldeoxyuridine (whether used
