Possible mechanisms in the induction of vitiligo-like hypopigmentation by topical imiquimod.

The pathogenesis of vitiligo was examined for clues to the pigmentary changes that may occur after treatment with topical imiquimod. The literature varies on the pigmentary changes induced by topical use of imiquimod. The US Food and Drugs Administration lists 68 reports of pigmentary changes out of a total of 1257 reports related to imiquimod lodged from 1997 to 2003. Some studies describe vitiligo-like hypopigmentation associated with imiquimod treatment of genital warts (as with the patient described in this report), molluscum contagiosum, basal cell carcinoma, extramammary Paget's disease

The role of platelet activating factor and the efficacy of apafant ophthalmic solution in experimental allergic conjunctivitis.

Platelet-activating factor (PAF) may be an important mediator in allergic conjunctivitis. In this study, apafant, a potent PAF antagonist, was evaluated for topical ocular anti-PAF activity in PAF and antigen stimulated conjunctivitis models. PAF, when injected into parpebral conjunctiva, provoked an acute increase, measured as dye leakage, in conjunctival vascular permeability. Apafant inhibited this response in a dose-related manner, and the inhibitory action of 0.1% apafant lasted for at least 6 hours duration. PAF, when instilled into the conjunctival sac, induced itch-scratching behavior

Carbonic anhydrase inhibitors: synthesis and inhibition against isozymes I, II and IV of topically acting antiglaucoma sulfonamides incorporating cis-5-norbornene-endo-3-carboxy-2-carboxamido moieties.

Sulfonamides incorporating cis-5-norbornene-endo-3-carboxy-2-carboxamido moieties in their molecules were prepared by reaction of cis-5-norbornene-endo-2,3-dicarboxylic anhydride with aromatic/heterocyclic sulfonamides possessing free amino, hydrazino, or imino groups. Some of these compounds showed very good CA II and CA IV inhibitory properties, with affinities for the enzymes in the low nanomolar range. Some of the most active CA II inhibitors reported here have been formulated as aqueous solutions for topical administration as antiglaucoma agents in normotensive rabbits. Some of the deriva

[Topical, intratumor and intravenous nitrogen mustard in the treatment of mycosis fungoides (author's transl)].

Topical nitrogen mustard (NH2) is recommended in mycosis fungoides stages 1 or 2 (eczematous premycotic phase and plaque stage), intratumor infiltration in stage 3 (tumor stage) and intravenous administration in stages 4 and 5 (widespread disease). Following these criteria ten patients with mycosis fungoides have been treated with NH2. Six cases were included in stages 1 or 2 and complete remission was achieved in four of them. Therapy had to discontinued in one case due to allergic hypersensitivity. The remaining patient experienced clinical improvement after 27 months of continued treatment.

Topical Cantharidin in the Management of Molluscum Contagiosum: Preliminary Assessment of an Ether-free, Pharmaceutical-grade Formulation.

Topical application of cantharidin, a vesicant that is naturally derived from the blister beetle, has a long track record of being used to treat primarily cutaneous molluscum contagiosum and Verrucae. Although not approved by the United States Food and Drug Administration, cantharidin has been available through a variety of compounding sources without standardization of manufacturing, formulation, or method of application. Randomized, controlled studies assessing safety and efficacy of topical cantharidin are limited, to date, with the majority of published evidence gleaned from collective cli

The use of topical tacrolimus (FK506/Protopic) in cutaneous manifestations of autoimmune diseases.

The cutaneous manifestations of autoimmune diseases, including systemic lupus erythematosus, are common and often recalcitrant to treatment. Unfortunately, therapy for lupus and other autoimmune skin diseases has not advanced and relies heavily on the use of oral and topical corticosteroids. Frequently, treatments prove less than ideal, either from toxicity or lack of efficacy. A topical form of the immunomodulating transplant medication, tacrolimus (FK-506, Protopic), has recently been developed and approved for use in treating atopic dermatitis. Its mechanism of action and local route of adm

Patient compliance with short-term topical aural antibiotic therapy.

Little is known about patient compliance with topical aural antibiotic regimens. The compliance of 50 patients with unilateral otitis externa attending an otolaryngology clinic was studied by comparing the weight of dispensed topical ear preparations before and after completion of a 7-day-course of treatment. A standard was obtained from controlled administration of the preparation under laboratory conditions and the performance of different delivery systems evaluated. Thirty-seven patients re-attended for review with their medication. A total of 34 of 50 patients entering the study (70%) sati

Thieno[2,3-b]furan-2-sulfonamides as topical carbonic anhydrase inhibitors.

Novel 5-[(alkylamino)methyl]thieno[2,3-b]furan-2-sulfonamides were prepared and evaluated in vitro for inhibition of human carbonic anhydrase II (CA II) and ex vivo for their ability to inhibit Ca II in the albino rabbit eye after topical administration. Compound 11a was found to lower intraocular pressure (IOP) in both the alpha-CT ocular hypertensive albino rabbit and the normal albino rabbit, but was ineffective at lowering IOP in a hypertensive, pigmented monkey model. Since 11a was highly bound to ocular pigment, a series of less basic analogs was prepared. Examples in this series were bo

Sucralfate retention enemas in solitary rectal ulcer.

The conservative treatment of solitary rectal ulcer is generally unsatisfactory. Six patients, aged 27-54 years, with recurrent solitary rectal ulcer were treated with topical administration of sucralfate in a daily dose of 2 g twice a day for 6 weeks. Four patients experienced complete relief of symptoms and the remaining two patients had marked improvement. Although macroscopic healing of the ulcer was apparent in all, histologic improvement was not appreciable. Five of the six patients remain in remission during a follow-up period of 4-14 months (mean, 8 +/- 1.5 months). Recurrence was obse

Traumatic hyphema.

Multiple modalities of treatment for traumatic hyphema have been advocated in the past. Therapy should be directed at reducing the risk of secondary hemorrhage and the potentially devastating complications of corneal blood staining and optic atrophy. Therapeutic regimens proven successful include: a patch and shield to the traumatized eye; daily visual acuity and slit-lamp biomicroscopy, including intraocular pressure, evaluation of corneal clarity, and size of hyphema; topical atropine; the systemic administration of aminocaproic acid; and topical and systemic antiglaucomatous medications wit

Evaluation of dose escalation and drug combinations of ivermectin and fipronil as strategies for improving control of resistant Rhipicephalus microplus populations.

The objective of this study was to compare the therapeutic and pharmacological performance of ivermectin (IVM) and fipronil (FIP) administered as single drugs, combined in a same formulation, or co-administered individually, as potential strategies to improve treatment efficacy against multi-resistant Rhipicephalus microplus populations. The effect of IVM dose escalation was also evaluated. Naturally infested heifers were allocated into six groups (G) and treated with: injectable IVM 1% at 0.2 mg/kg (G-I) or 0.4 mg/kg (G-II); FIP pour-on at 1 mg/kg (G-III); pour-on formulation of IVM/FIP (0.5

Complete atrioventricular block due to timolol eye drops: a case report and literature review.

BACKGROUND: Timolol Maleate is a non-selective beta-adrenergic blocker that is commonly used to treat open-angle glaucoma. Despite its topical administration, ophthalmic timolol enters systemic circulation and produces a systemic beta-adrenergic blockade. We report a case of long-term timolol use that uncovered and worsened an underlying cardiac conduction defect demonstrated as a third degree atrioventricular (AV) block. CASE PRESENTATION: A 62-year old male with a 13-year history of glaucoma was hospitalized due to shortness of breath, dizziness, and amaurosis. Electrocardiography indicated

Irritant Contact Dermatitis Induced by Compound Clobetasol Propionate in a Patient with Undiagnosed Type 2 Diabetes Mellitus.

Irritant contact dermatitis (ICD) is a common inflammatory skin condition caused by direct cytotoxic effects of irritants on the epidermis. Topical corticosteroids, while effective for treating inflammatory dermatoses, may paradoxically induce adverse skin reactions when misused. This case highlights ICD triggered by self-administration of compound clobetasol propionate ointment, exacerbated by undiagnosed type 2 diabetes mellitus (T2DM).

Benzocaine-induced methemoglobinemia: a potentially fatal complication of transesophageal echocardiography.

We describe the cases of 2 patients who developed benzocaine-induced methemoglobinemia after the administration of benzocaine as premedication for transesophageal echocardiography. The use of intravenous methylene blue resolved the cyanosis in both patients. Physicians who perform procedures involving the application of topical anesthesia need to be aware of this side effect to prevent morbidity and mortality.
