Ethical issues in dysphagia: when patients refuse assessment or treatment.

The primary goal of intervention for patients with dysphagia is to restore oral feeding. When patients are unable to achieve adequate nutrition, hydration, or safety with oral feeding, then nonoral approaches are often recommended. Although patients' rights to accept or refuse clinical recommendations are widely recognized, when a patient refuses tube feeding or other recommendations, dysphagia specialists are left with a host of practical questions about their role in caring for the patient. We review the criteria for assessing patients' capacity to make informed choices, approaches to decisi

Specialist palliative care in different intensive care unit populations: a retrospective longitudinal study.

BACKGROUND: Integration of specialist palliative care (PC) in intensive care units (ICUs) is recommended but remains underutilized. Understa … ood (mSMD: 0.552) and dyspnea (mSMD: 0.468) differed between ICU populations. 28.6% of patients had advance directives (p = 0.16 between ICUs), 48.1% a power of attorney (p = 0.22 between ICUs). CONCLUSIONS: In this pandemic-era cohort, ICU patients receiving specialist PC showed high mortality and symptom burden but differed regarding PC triggers and symptom asses …

"It seems enormously valuable to me." Perspectives of Dutch (potential) carriers of genetic FTD on onset-predictive biomarker testing.

BACKGROUND: Onset-predictive biomarker tests (OPBT) in genetic frontotemporal dementia (FTD) may be used to recruit mutation carriers into preventive clinical trials before symptoms manifest. This would require disclosure of OPBT results to potential participants. This study investigates the perspectives of Dutch presymptomatic mutation carriers and individuals at 50% risk of genetic FTD on disclosure of OPBT results. It focuses on their willingness to receive OPBT results, what impacts they foresee from disclosure, and their preferences for the process of disclosure. METHODS: Semi-structured

Nigeria's mental health and substance abuse bill 2019: Analysis of its compliance with the United Nations convention on the rights of persons with disabilities.

Countries are struggling with reconciling their national mental health legislation with the CRPD approach, which stresses equality as the fo … closest relatives, and non-prohibition of coercive practices. The bill does not make provisions for advance directives and is silent on informed consent to participate in research. Despite the bill's deficiencies, it would be a significant step forward for the country, whose current mental health legislation is the Lunacy Act of 1958. Although the CRPD has left it u …

STUDY OBJECTIVE: To determine the feasibility of bilevel positive airway pressure (BiPAP) support for acute respiratory distress (ARD) in th … erate BiPAP, and two others were considered ED treatment failures but were not intubated because of advance directives. CONCLUSION: As has been reported from other critical care settings, use of BiPAP is feasible and has potential utility in the management of ARD in the ED.

Do professional experience and qualification influence knowledge about law concerning informed consent and end-of-life decisions? A quantitative online survey among German intensive care physicians.

BACKGROUND: Recently, legal questions have increasingly arisen in intensive care medicine (ICM), especially when it comes to end-of-life dec … wer legal questions related to end-of-life policies, informed consent, surrogate decision making or advance directives. We tested pure factual knowledge in five questions. The other five questions tested situational knowledge using case vignettes. Every question could be answered with 'yes', 'no' or 'do not know'. False memories for end-of-life decisions.

OBJECTIVE: To examine people's false memories for end-of-life decisions. DESIGN: In Study 1, older adults decided which life-sustaining treatments they would want if they were seriously ill. They made these judgments twice, approximately 12 months apart. At Time 2, older adults and their self-selected surrogate decision makers tried to recall the older adults' Time 1 decisions. In Study 2, younger adults made treatment decisions twice, approximately 4 months apart. At Time 2, younger adults tried to recall their Time 1 decisions. Views of potential subjects toward proposed regulations for clinical research with adults unable to consent.

OBJECTIVE: The authors' goal was to assess healthy individuals' attitudes toward five of the most prominent proposed safeguards regarding the consent process for research with adults unable to consent. METHOD: Telephone interviews were conducted with 246 individuals with a family history of Alzheimer's disease who had participated in clinical research. RESULTS: The majority of respondents said that they were willing to participate in research if they lost the ability to consent. Few completed a research advance directive. Many had discussed their preferences with their families, and the majo

Patient Perspectives on Preoperative Code Status Discussions: A Qualitative Analysis.

CONTEXT: Professional guidelines from the American Society of Anesthesiologists (ASA) and American College of Surgeons (ACS) emphasize the importance of preoperative communication regarding code status, particularly for patients with limitations on resuscitation. However, little is known about how surgical patients with DNR/DNI or MOLST orders perceive these conversations. OBJECTIVES: To understand patient perspectives to improving perioperative communication and aligning care with patient values. PARTICIPANTS AND SETTING: This study included 12 adult surgical patients aged 65 and older with

Solution structure of the variable-type domain of the receptor for advanced glycation end products: new insight into AGE-RAGE interaction.

Diabetes is defined by chronic hyperglycemia due to deficiency in insulin action. It has been found that the amount of advanced glycation end products (AGE) from the Maillard reaction between proteins and sugar molecules increases in blood of diabetic patients and furthermore that AGE binding to their cell surface receptor (RAGE) triggers both macrovascular and microvascular impairments to cause diabetic complications. Due to the clinical significance of the vascular complications, RAGE is currently a focus as an attractive target for drug discovery of candidates which interfere with AGE-RAGE

Physician Approaches to Determining Goals of Stroke Care for Patients Living With Disability or Dementia: Results from the SEED Mixed-Methods Study.

BACKGROUND: Despite remarkable advances in stroke management, there is a continued lack of evidence to guide care for the 1 in 3 stroke patients living with disability or dementia (PLWD). To help inform best practices, this study sought to understand how physicians approach the complex issue of determining goals of care for PLWD and what challenges they encounter. METHODS: In a mixed-methods investigation, we invited physicians involved in stroke care to participate in semistructured interviews and an online survey, enquiring into perspectives on stroke management in PLWD. Interviews were ana
